BLUE CARD SURVEY FORM - GUIDANCE NOTES


Important : The surveyor must settle the survey fee with the vessels Owners directly, without
involvement of RaetsMarine Insurance B.V.
We request surveyors to take good notice of below guidelines prior to attending on board.
With effect from the 21st of November 2008 the International Convention on Civil Liability for Bunker Oil
Pollution Damage, 2001 will be in force. This convention requires registered owners of vessels over 1,000
GRT registered in a state which is party to the Convention to maintain insurance (or alternative security such
as the guarantee of a bank or similar financial institution) to cover pollution damage up to the limit of liability
of the relevant limitation regime (but no more than the limit applicable under the current version of the 1976
Convention of Limitation of Liability for Maritime Claims as amended).
Ships registered in a State party or entering or leaving a port in the territory of a State party are to carry on
board a CLC Certificate issued by the flag state attesting that the registered owner maintains insurance in
accordance with the requirements as set out in the International Convention on Civil Liability for Bunker Oil
Pollution Damage, 2001.
To obtain a CLC Certificate from the relevant flag state authorities the owners of vessels insured with
RaetsMarine will need to obtain a Bunker Convention Blue Card from RaetsMarine.
This Bunker Convention Blue Card confirms that the insurance maintained for the insured vessel meets the
requirements of the Bunker Convention.
Given the nature of confirmation provided by the Bunker Convention Blue Card it is imperative that the P&I
insurance is not subject to any provisions which may prejudice cover in the event of an incident, such as a
defects warranty clause or reservations mentioned in a RaetsMarines Risk Assessment.
It is therefore strictly necessary that an insured vessel has undergone a satisfactory Blue Card
survey in accordance with our Blue Card survey guidelines, by an approved surveyor
Complete the Blue card survey report with the information available on site but dont forget that this report format
is only a guide to assist with the reporting.
If a test or inspection could not be carried out, clearly state reason. This non-accomplished inspection
must be stated on the deficiency list in the nominated section.
Nowadays the majority of claims are caused, direct or indirectly, by human errors. The surveyor is asked to
have special attention for the human element on board and in the relevant section carefully assess crews
competence, both in operation of the vessel and in crew following procedures.
The full Blue Card Survey report with relevant photographs and test results must be faxed / emailed to
RaetsMarine Insurance B.V. in Rotterdam WITHIN 48 HOURS after completing the survey.
Please note that RaetsMarine requires only ONE ORIGINAL of the report. We kindly ask to provide the report
with staple, ring binder or loose sheets and ask not to prepare reports with a hard cover / glued (due to
administrational reasons). Please also send a copy of the report to the Owners.
The condition rating section on page 1 is used to assist RaetsMarine to make an assessment for insurance
purposes. Considerable objectivity is required from the surveyor. To ensure a common basis in completing this
section, the following brief guide should be followed. Whole numbers with decimal digits can be used.

Page one also includes a summary section. This should be a brief summary of the vessels condition.
The last page of the form (page 8) should list any deficiencies, clearly stating the nature and extent of these
defects. Surveyors should only mention the deficiency; they should refrain from providing a solution in their
comment.
The Master or other Owners representative must sign the last page(s) of the form (page 8), with the list of
defects and any non-accomplished items. If a signature is refused or is unobtainable this should be stated with
the reason.

be corrected within a period of 6 calendar weeks from date of survey, if
and when these deficiencies are not lifted within this period the Blue
Card might not be issued or might be revoked.
