A descriptor for a saw-tooth pattern of high temperature 'spikes', a finding typical of pyogenic
hepatic abscesses, often accompanied by chills, sweating, N&V, anorexia, pain
Sigmoid sinus thrombophlebitis may be completely asymptomatic or can be
associated with signs of toxemia, torticollis and septic embolization. If the thrombus
propagates far enough, it can be associated with intracranial hypertension or otic
hydrocephalus. Clinically, the patient will present with a picket fence fever pattern,
headache, lethargy, papilledema and an ear infection. Septic thrombus can
propagate to involve the cavernous sinus. If there is involvement of the cavernous
sinus, signs include bilateral orbital involvement, sever chemosis and
opthalmoplegia, retinal engorgement, and high fever. MRI with gadolinium is the
imaging technique of choice for sigmoid sinus thrombophlebitis. MRI with
MRA(magnetic resonance angiography) is also an ideal diagnostic tool. Conventional
angiography requires vessel puncture, use of IV contrast agents, and uses ionizing
radiation. Therefore, angiography is less desirable. The treatment is to surgically
expose the diseased dura and remove the excess granulation tissue. If otic
hydrocephalus is present, one must treat or monitor the intracranial hypertension. If
a septic emboli is present , it may require sigmoid sinus decompression or ligation
of the internal jugular vein to avoid further embolization of the tissue. Alternative
forms of treatment include anticoagulation therapy and IV antibiotics. However,
anticoagulation is not usually recommended as it can be associated with spreading
septic emoboli or hemorrhage.

Airline Passengers Are Sprayed for Bugs

An airline flight to the tropics may involve greater health risks than a dose of airline food--
pesticides are routinely sprayed in aircraft cabins by U.S. airlines sometimes over the heads of
passengers during flight. "Disinsection" is the industry term for this practice, which continues
despite clear evidence of risk to passengers and crew. People more vulnerable to the effects of
pesticides, such as infants, pregnant woman or asthmatics are informed, if at all, only just prior to
spraying. Airline flight attendants unions argue that chemical spraying is unnecessary because
mechanical methods could be applied instead.
No U.S. agency requires pesticide use on planes. The US Department of Transportation website
lists the countries that require in-flight spraying, and those that will accept the "residual"
treatment as an alternative. Six countries currently require pesticide spraying on all inbound
flights: Grenada, India, Kiribati, Madagascar, Trinidad and Tobago and Uruguay. The
application method varies by country and airline. Typically, a pressurized spray containing 2%
phenothrin is sprayed over the passengers' heads during the flight (also called "top-of-descent")
or upon arrival, but while the doors are closed. Alternatively, cabin crew may spray the occupied
cabin prior to departure after the doors have been closed ("blocks away"). A member of the crew
will announce the procedure shortly before they spray.
Another six countries: Australia, Barbados, Fiji, Jamaica, New Zealand and Panama require the
use of residual pesticides. In this case applicators board the aircraft and spray every surface in
the cabin with a solution that contains 2% permethrin. This process takes place shortly before
crew and passengers board, without their knowledge. Babies and children are said to be more
sensitive to the effects of permethrin. Once an aircraft has been residually treated, foreign
quarantine officials will allow it to land without additional pesticide treatment for the next 56
days.
Passengers flying on US domestic flights may find themselves on an airliner that has recently
been sprayed. United Airlines, for example, treats all of its 747-400 aircraft in Hong Kong.
These aircraft are not restricted to the South Pacific routes; they are simply scheduled to fly to
Australia or New Zealand during the next 56 days, but in the meantime, can be flown on both
international and domestic routes.
The International Civil Aviation Organization reports that most airlines use permethrin and
pyrethroid, both are suspected endocrine disruptors, and permethrin may be a carcinogen. The
Northwest Coalition for Alternatives to Pesticides (NCAP) points out that pesticides cause even
greater harm on airplanes, where up to 50% of the air in the cabins is recycled. "Pesticides break
down slowly in the enclosed, poorly ventilated aircraft," says a NCAP spokesperson.
The airlines are not required to inform passengers at ticket purchase of flight sprays, and there is
also no control over how much pesticide is applied on the aircraft. The Association of Flight
Attendants reported in 2001 that one airline used 50-60% more pesticide than the maximum
recommended by the World Health Organization. Between 2000 and 2001, one cabin crew union
received complaints of pesticide-related illness on more than 200 flights. Many complaints cite
damp surfaces and pesticide odors in crew rest compartments. Crews and passengers have
reported sinus problems, swollen and itchy eyes, cough, difficulty breathing, hoarseness, skin
rashes/hives that vary in intensity, severe headaches and fatigue, and heightened sensitivity to
other chemicals. Some crew members have medical documentation of reactions consistent with
nerve gas exposure, such as blood, optic nerve, and nervous system abnormalities.
Alternative methods to control insects on aircraft are already in use. Since the 1980s, the U.S.
Department of Agriculture (USDA) has used curtains made of overlapping strips of plastic to
successfully keep Japanese Beetles off aircraft destined for the western states during the summer.
Chemically treated mosquito netting and blowers in jetways may also be used as alternatives. A
variety of mechanical means should be tested.
The Association of Flight Attendants suggests that passengers contact the airline to find out if
pesticides will be sprayed on their flight, or if they will be boarding a "residually sprayed" craft.
The U.S. Department of Transportation website also lists countries that require spray.

Acyclovir “I see clover”
I see a three-leaf clover (this is a prodrug that gets converted into a triphosphate – it resembles
dNTP and inhibits DNA Polymerase). Imagine putting clovers over herpes sores and genital
warts (this is used to treat those).

Amantadine “Homan to Dean”

In case you haven’t heard, Dr. Homan is our new SOM dean. His new policy: he won’t let us
come to school without our white coats (this drug blocks uncoating, stage 2, in viral infection).
Other interesting facts about him: he can pee for 12 hours straight! (The half-life is 12h and it’s
excreted unchanged in the kidney.)
Dr. Homan can’t take the pressure of being a new dean so he becomes a nervous drunk with

You have to inflate your man doll and you get an upper respiratory infection. If you “use” this
anatomically correct man doll, he will give you pelvic inflammatory disease (This drug is
indicated for URI and PID). He’s also so real that he bleeds!!! (Adverse effect: It affects Vit-K
dependent clotting function)

Cephazolin “…Soul in”

You want to keep your soul in and not die if you go to surgery (this drug is used prophylactically
for surgery).

Cephoperazone “…Opera zone”

Going to the opera makes me want to get drunk to the point of illness. These drugs, when mixed
with ethanol interfere with ALDH, affecting alcohol metabolism (you end up with increased
acetaldehyde – a DISULFIRAM-LIKE REACTION).
Also, if you see Phantom of the Opera, expect to see some blood! (This affects Vit-K dependent
clotting function.)

Cephotaxime “…Tax time”

IRS is hard to kill off – they are MULTIPLE DRUG RESISTANT (Use this drug against MDR
strains.).

Clindamycin “Clean the mice”

You’re cleaning the mice because they are covered with ticks (this drug is used to treat Lyme
Disease). You have to use SOS kitchen cleaner to clean the mice (SOS = 50 S ribosomal unit –
where this drug acts).
Richard Gere likes to put mice in “where the sun don’t shine” and this is a very FRAGILE
procedure (Clindamycin may cause Pseudomembranous enterocolitis and it’s also used to treat
B. fragilis infections).

Fluconazole “Flu cone”

Imagine an emaciated AIDS patient eating an ice cream cone made of the flu and he gets an oral
and esophageal fungal infection. (This is used to treat oral and esophageal candidiasis in AIDS
patients).

Foscarnet “Fox cARRR net”

If you catch foxes in a net, they won’t reproduce (this drug is a stage 3 inhibitor). ARRR… it
inhibits RNA polymerase and Reverse transcriptase to block Reproduction. You’re going to put
the foxes into a car that’s made out of bones instead of metal (this drug binds to bones and
decreases Ca2+ and Mg2+).
Also, a foxy lady is HOT (this drug will cause a fever) and never wear fox urine to hunt (this is
excreted in the kidney and causes renal insufficiency).

Griseofulvin “Greasy…”
If you put grease on your skin you’ll sunburn. This drug binds to keratin in the skin blocking
fungal habitation. Also think about greasing up the mitotic spindle (it binds the mitotic spindle as
well).
Also, if you wear grease when you go to war, the bullets will slip off you and not hit you so you
won’t bleed! (This drug induces p450 which increases the metabolism of warfarin thus reducing
warfarin’s hypoprothrombinemic effects.)
Poor people have to use grease for their because they can’t afford Salon Selectives (this can
cause porphyria as an adverse effect).

Idoxuridine “Eye dogs…”

You need seeing-eye dogs because you have herpes simplex infection of the eyelid, conjunctiva,
and cornea (this is what this drug treats). Imagine your dogs attacking a passerby and
immediately chewing out his liver (this is rapidly metabolized by the liver).
You feed them the generic brand of Kibbles and Bits called “Kinase Inhibits” (This drug inhibits
DNA synthesis enzymes – Stage three).
Also, think Three Dog Night – it’s a prodrug that has to be turned into a triphosphate.

Miconazole “Microphone”
Imagine an athlete stuffing a microphone in his jock strap, a woman using a microphone for a
tampon, and a guy wearing mike’s for shoes. This drug is indicated for jock itch (tinea crurus),
yeast infections, and athlete’s foot.

Naladixic Acid “Dixie Chic”
