Self-Assessment Questionnaire
This questionnaire is designed to gather information regarding the mechanisms your facility management
has in place to manage its business conduct; human rights, labor and employment practices; worker health,
safety and well-being practices; sustainability and environmental responsiblity; and efforts associated with
transparency and disclosure. This information will be used to assure conformance to Johnson & Johnson's
Responsibility Standards for Suppliers and to determine if additional follow up may be necessary.
When answering the questions, "mechanisms" may include programs, policies, procedures, practices,
training, communications, meetings, internal inspections, internal audits, or management reviews. The
mechanism need not be documented or formalized to be considered effective. To be considered effective,
the mechanism should deliver the desired results on a continuous basis.
Please complete the questionnaire as thoroughly as possible. We encourage you to provide details in the
column marked "Please briefly explain all responses" since this will make the review process most efficient
for your organization and ours. Please return the file by email to your J&J contact. Your J&J contact will be
