ALUMINA SILOS
1. PURPOSE
This procedure has been developed to ensure that all site personnel who will, or
may, work at heights; the works on the Alumina Silo do so in a safe manner.
2. SCOPE
This procedure applies to all personnel employed by ASTALDI and subcontractors.
3. DEFINITIONS
High structures refer to any permanent or temporary above ground structure used
by personnel for any purpose during the course of the construction works.
4. INSTRUCTIONS
4.1

Personnel
Personnel shall be adequately trained and experienced in their work, specifically
as applied to operating above ground level. The training required shall be work
at height which will be provided by Qatalum and Astaldi Training departments.
ASTALDI and subcontractor Supervisors, adequate in number, training and
experience shall be provided for the control of work above ground level.
In order to minimize exposure to risk, the minimum practical number of
personnel shall be assigned to the job.
The operations above ground level shall be carried out in such a way that noninvolved personnel, in the vicinity of the activity, are not at risk.

The general protective measures must be preferred compared to


personal protective equipment.

All personnel working above ground level shall be provided with, and shall use,
appropriate personal protection including, but not limited to, the relevant
requirements of procedure Personal Protection Equipment
4.3

Means of Access / Egress


Stage 1 Use of full body harness with 100% tie-off
In order for alumina civil crews to safety access the dance floor for work or
emergency purposes, the following control measures shall be required to be in
place:

A staircase with landings for rest points shall be required to be erected on


alternate sides of the alumina foundation
Engineered points of anchorage for life line to be implemented to allow
workers to travel near the leading edges to place formwork boarding as
per the engineered design. All life line systems shall be required to have
an approved engineered drawing prior to the installation thereof. Refer to
engineered drawing attached.
An alternate option is to install scaffold tubing attached to the rebar on
the cured columns to act as a fall protection / fall restraint measure. This
system shall be required to have an approved engineered drawing prior
to the installation thereof. Refer to engineered drawing attached.
Warning signage shall be placed at strategic intervals from the base of
the staircases right up to the leading edge works to inform personnel of
the impending dangers and hazards that may be evident during the civil
works on alumina silos.
Only personnel trained in work at height may be authorized to access the
staircase to the dance floor of the alumina.
All personnel shall be required to don full body harness during the
installation of the dance floor.
During the installation of the dance floor all personnel shall practice
100% tie-off to the engineered life lines or scaffolding.
To prevent non authorized personnel accessing the staircase without a
full body harness a watch shall be present at the base of the staircase to
stop any personnel from accessing the staircase. All personnel shall

Stage 2 Access to dance floor without utilizing safety harness (Dance floor
completed)
At this stage of the construction the dance floor shall be completed and all fall
protection requirements are installed and in place, in order for alumina civil
crews / emergency crews to safety gain access the dance floor without utilizing
a safety harness. The following control measures shall be required to allow
personnel to work at height without the use of safety harnesses.
An approved staircase system on alternate sides of the alumina shall be
installed with proper fall protection railing; these stairs shall be built by
approved third party scaffolders and inspected by approved third party
scaffold inspectors.
A complete railing system, comprising of hand rail, mid rail and toe
boards with netting shall be installed at all leading edges
A watchman shall be placed at the base of the staircases, 1 to act as an
access point (Log In) and the other as an egress point (Log Out). The
watchman duties shall be to monitor the amount of personnel on the
dance floor for emergency purposes and to ensure that personnel do not
access the wrong staircase.
A safety officer shall be required to be monitoring activities on the dance
floor at all times and advice and assist construction on safety related
matters.
General access / egress requirements
Means of access to elevated areas shall be of an approved type, of sound
construction, suitable for the purpose and shall be used in a safe manner.
The choice of the right means of access has to consider:

Where the means of access is of a mobile or a portable type, it shall not be
moved either mechanically or manually, whilst personnel are upon it, unless it is
specifically designed and approved for the carriage or persons.
All of accesses shall be kept clear of any loose articles e.g. tools, equipment,
scaffold boards, etc. shall be removed or securely lashed down, prior to the
means of access being moved.
Where personnel must transfer from/to the means of access or from/to the high
structure, the arrangements at the point of transfer shall e such that personnel
are not a risk whilst transferring.
The means of access shall adequately provide for safe means of egress, for all
personnel, from the high structure
4.4

The High Structure


The integrity of the high structure shall be such that the required operations can
be performed safely.
This includes, but is not limited to: the loading of the structure e.g. men,
equipment and materials; the environmental conditions; the forces imposed
upon it by any means of access which is connected to it; and the forces
imposed upon it by any lifting or winching operations in which the high structure
serves as an anchor point, counterweight or tie point.

Pre-Work Checks
Before work commences, the ASTALDI and subcontractor Supervisor in charge
of the work shall ensure:

That all necessary permits have been obtained;


That all necessary electrical, mechanical and process isolations have been
carried out.
That all static, moving, lifting and anchoring equipment, integral to the
means of access and high structure, inclusive of those associated with the
work task, are correctly fitted, conditionally sound and fit for the intended
purpose.

4.6

That all personnel have been provided with the required personal
protection;
That all personnel have been briefed in, and understand, the safety aspects
of the job;
That communications drills between members of the work team have been
adequately rehearsed and all communications equipment is functioning;
That suitable emergency transport is permanently available for use on site.

Working practice
Personnel shall not be permitted to climb whilst carrying tools or equipment, the
only exception being tools carried in a waist belt designed specifically for the
purpose.
Where necessary, tools shall be hauled up and lowered in a suitable container,
using a handling of suitable size and condition.
Electrical, pneumatic and hydraulic tools shall not be hauled up or lowered by
their power lines.
All lifting operations shall be controlled by a rigger who shall ensure that safe
lifting practices are used at all times.
During lifting operations, no person shall be positioned beneath the load, hoist
or jib.
Similarly, loads shall not be slewed over the heads of personnel.
Where practicable, personnel shall not be assigned to work directly below the
overhead work positions or other persons.
Where this is unavoidable, suitable precautions shall be taken.
All personnel shall be clipped on also when moving about the structure, its
obligatory to use double lifelines.
There must be a safe and strong anchorage for lifelines that should resist at
least 2 tons.

ALUMINA SILOS
Where a safe and strong anchorage is not available, a steel cable with supports
of 50 cm must be provided.
This system must be tested and certified.
The anchorage should be preferably placed in a place higher than the worker to
reduce the fall.
Tools, equipment shall not be left lying around the structure.
They shall be kept in such a location that they do not represent a tripping
hazard and cannot fall or be knocked from the structure.
All overhead work shall cease when wind speed reaches 40 km/h (10 m/sec).
The only exception being such work which is intended to render the height
structure or its means of access is safe.
Any overhead work which, it is claimed, must be carried out during the hours of
darkness, shall be subject to written authorization from the OWNER site
representative.
Such authorization shall be inclusive of the required standard of illumination.
When overhead work is required on wet structures or their means of access,
those facilities shall be inspected by the job supervisor and OWNER site
representative prior to job start.
Where such facilities become wet during the operation, work may continue only
in order to make the structures safe.
Work shall then cease until inspection by the above named confirms that it is
safe to continue.
Upon completion of the days work, all ropes and tackles shall be securely tied
back on to a permanent structure.
The means of access shall be blocked off and any crane jibs shall be tied down
to their resting brackets.

Where there is a risk to third parties, high visibility warning tape, notice boards
and warning lights shall be situated.
4.7

Ladders
Its forbidden to work on ladders without safety harnesses with lanyards clipped
to a strong and safe anchorage.
Ladders will be 4.5 meters in length:

Used only by one worker;


In good condition in all their parts;
In metal, except the ladders used for electrical work;
With stabilizer feet;
Linked with a safe and strong anchorage;
Not sloped more than 75 degrees;
1 m higher than the work-plan;
Stored with appropriate hooks.

Ladders will be frequently checked and kept clean, particularly from oil and
grease. Its forbidden to use ladders as work-plan and horizontally.
5. SLIP FORM PREPAREDENESS
Installation of slip form structure & decking
During the installation phase of the slip-form structure the following shall apply

All personnel designated to install the slip form components shall be required to
be trained at work at height and utilize a safety harness as well as maintaining
100% tie-off to approved anchor points.
Only approved scaffolders and carpenters may be authorized to install the slip
form under guidance and supervision from the slip form technician
All ladder access / egress points shall be boxed in by scaffolding and green
netting shall be attached to boxed area for dropped object protection.

Moveable scaffold arm shall be placed at all scaffold ladder access points to
prevent personnel from falling down ladder when moving past or working in the
vicinity of the ladder.
All access ladder points shall be inspected by a scaffold inspector and green
tagged for use.
All access / egress points shall be required to be clearly marked with
appropriate signage informing personnel of access points / egress points and
emergency routes.
A designated watch man shall be provided to monitor personnel accessing and
exiting the slip form. A log book will be maintained and personnel shall be log in
and out during the slip form activity.
