JUDGMENT AND DECREE

Respondent.

This matter came before the court for trial pursuant to written notice. Appearing on behalf of the
plaintiff was attorney Andy Smith. Defendants appeared by Mike Smith. Testifying on behalf of the
plaintiff was Deb Germann who identified plaintiff's exhibits one - four all of which were offered and
received into evidence without objection.
Based on the evidence presented the court finds that of the services listed in Exhibit 1 and rendered
on May 22, 2013, a balance of $1167.01 was the patient's responsibility; of the services listed in
Exhibit 2 and rendered on August 22, 2012, a balance of $371.86 was the patient's responsibility;
of the services listed in Exhibit 3 and rendered on August 27, 2012 a balance of $196.02 was the
patient's responsibility; of the services listed in Exhibit 4 and rendered on May 13, 2013 a total of
$181.17 was the patient's responsibility.
Based on the foregoing the court finds that the plaintiff is entitled to judgment against the defendants,
jointly and severally, in the amount of $1916.06 and judgment shall be entered in the plaintiff's favor
in that amount plus interest at the rate of 2.21% per annum from October 18, 2014 plus with the costs
of this action.
Defendant requests a $100 per month payment plan and the court grants that request. Plaintiff shall
pay $100 per month by the first beginning with a first payment on February 1, 2015.
It is so ORDERED.
