health system there by realizing the geographic expansion of community based
nutrition program(CBN), strengthening multi-sectoral nutrition coordination, doing
supportive supervision and building the capacity of health centre staff, Health
Extension workers(HEWs) and Health development Army (HAD)
.
Specific Responsibilities
Ensure the expansion of CBN in new woredas in Amhara Region as per the
regional AWP

and ensure they stared implementing monthly GMP and submit

Conduct capacity gap assessment and capacity building for PHCU staff in CBN
Woredas on preventive nutrition programs using the opportunity of the planed
health center staff training and through on the job support.

coordination.

Attend nutrition multisectroal stakeholder planning and review meetings at


Region, zone and woreda level in order to make sure nutrition is well reflected
and followed up.

Conduct field supportive supervision to Zones/woredas on monthly basis

Organization of periodic ( at least once every two months) review meetings at


woreda and regional levels, using the community-generated data, and compile
issues discussed in the review meetings and provide feedback

Collect information about related programs, explore effective linkages, and


facilitate the establishment of linkages at the regional/woreda levels

Closely work and communicate with the Nutrition monitors and NGOs in the
region to facilitate the information sharing and follow up of activities.

Ensure that the CBN supplies are distributed to all CBN implementing woredas
and collect report on utilization and management

Consolidate on quarterly basis CBN report on progresses in CBN implementation,


bottlenecks, constraints and actions taken to address bottlenecks.

Documentation of CBN activities including complementary food as well as linkages


with other sectors

Support and follow the Complementary food production in 5 woredas per region

SHN programming;

Generally: Being the Public Health Emergency Management Core Process
Coordinator and Woreda EPI focal person I was responsible for Planning,
organizing, implementing, monitoring and evaluation of diseases surveillance
activities in the district. Moreover, doing vaccine forecast, planning, organizing and
implementation, monitoring and evaluation of EPI programmes were my duties.
Specific Responsibilities

Doing active case search in outpatient and inpatient registry of health facilities
and other priority sites and report if unreported cases of reportable
diseases/deaths found , like AFP, Measles and NNT

Represent the district in IDSR (integrated Diseases Surveillance and Response)


and EPI meetings and trainings

Compile and report weekly IDSR report to Zonal Health Department

Sensitize health workers, health extension workers and community members in


the prioritized areas about AFP, Measles, NNT, AWD and other diseases
surveillance and IDSR

Analyse and present IDSR data for local use

Make available surveillance and EPI guidelines, report formats and posters in
office and health facilities

Preparing case file for each reported AFP and measles cases

Maintain line listing of reported AFP, Measles and other important diseases in
surveillance

Prepare annual, semi-annual, quarter and monthly plan of activities and submit
to immediate supervisor

Ensure proper documentation of surveillance reports.

Follow health facilities cold chain system and immunization service status and

Achievements

AFP suspected case detected and reported for the first time since the past eight
years in the district

Measles epidemic investigated early and epidemic management started as soon


as possible

Health extension workers were given full course of IDSR training and also
refresher training including on AWD.

All health facilities, with special emphasis to high priority sits, and other priority
areas for surveillance ( Eg. Hostel) were visited for register review and staffs
were updated on surveillance.

Line list was prepared and documented for reported AFP case, for AWD case and
for the cases of measles throughout the epidemic period.

Enough amount of recent manuals and weekly reporting were distributed to


health facilities

Assisted AWD epidemic investigation and management outside my work place,


Dasenech district

Provide IDSR training out of my working place, Dasenech district

Undertake nutrition screening in the district in selected kebeles
