FEZ BUOK
This Judicial Affidavit of FEZ BUOK, a witness, is executed to serve as his direct
testimony in the instant case.

This Judicial Affidavit is being offered to prove:

A)
The annexes appended thereto and which were already marked as
exhibits during the Pre-Trial of this case;
B)
All other related matters, facts and circumstances relevant and
material to this case.
This Judicial Affidavit was taken at the office of Atty. Twit Nger, Zamaboanga City.

Questions were propounded by Atty. Twit Nger and these questions are
numbered consecutively and each question is followed by the answer of
the witness.
1. Do you swear to tell the truth and nothing but the truth?
I do.

2. Are you aware that you may face criminal liability for false testimony or perjury if
you will not tell the truth?
I am.

3. Please state your name, age address and occupation.

I am Fez Buok, 30 years old, married, and residing at Zamboanga City.

4. Are you the same Fez Buok, a witness in this case?

Yes.

5. Do you know the Defendant in this case, Mr. Gal Axy?

Yes.

6. How did you come to know the Defendant?

I met him in Cagayan de Oro City on December 04, 2014

7. Do you know the Plaintiff in this case, Mr. Sam Sung?

Yes.

8. How did you come to know the Plaintiff?

He has been engaging my services from time to time, even prior to this case and
prior to the execution of Exhibit____.

9. Can you attest to the fact that both Plaintiff and Defendant personally know each
other?
Yes. I was the one who facilitated the execution of the contract and notarized it.

10. Showing you a document, marked as Exhibit____, can you identify what this
document is?
Yes. That is the contract of sale.

11. What is your involvement in this contract?

I was a witness then and there when it was executed and I was the one who
notarized it.

12. How did you come to be involved in Exhibit____?

On December 03, 2014 I was called by Sam Sung and was asked to facilitate the
execution of one and to notarize it.

13. How was Exhibit____ executed?

As agreed, I met Piolo Pascua, Gal Axy and Sam Maybe in Lim Kit Kai inside Bos
coffee shop where it was executed and signed by Gal Axy at around 9 oclock in the
morning. Then I subsequently left to attend to my personal business in Cagayan de
Oro City and went back to Zamboanga City on the same day where Sam Sung
signed the contract and where I notarized it in my office.
