In accordance with the Washington Federation of State Employees, Council 28, AFSCME, AFL-CIO (WFSE) collective bargaining agreement with my employer, I authorize and direct my employer to deduct from my pay each pay period, regardless of whether I am or remain a member of the Union, the amount of dues as set in accordance with the WFSE Constitution and By-Laws (currently 1.5% of my salary each pay period, not to exceed the set maximum). This authorization shall be irrevocable, regardless of whether I am or remain a member of the
Union until terminated by me by written notication to my employer with a copy to the WFSE in accordance with the collective bargaining agreement.

Dues or fees paid to WFSE are not deductible as charitable contributions for federal income tax purposes. Dues or fees paid to the WFSE, however, may qualify as business expenses and may be deductible in limited circumstances subject to various restrictions imposed by the Internal Revenue Service.
 
 All employees covered by a collective bargaining agreement must, as a condition of employment pay either union dues or a union representation fee to the WFSE no later than 30 days following the effective date of the bargaining agreement or the beginning of their employment in a covered bargaining unit position. If you choose to NOT become a union member please provide the information requested on the reverse and sign and date below indicating your choice to NOT become a union member. I understand that by choosing NOT to become a WFSE union member I still must pay a union representation fee but I am giving up the right to participate in all union activities. I will
be unable to vote for union ofcers. I will be unable to participate in union member trainings and events. I will not be eligible to vote to accept (or reject) my union contract or proposed changes in wages or benets as a result of contract negotiations. I will not be eligible for member-only union benets. I will not be eligible to participate in union-management meetings regarding workplace concerns. I will not have the right to vote on the ratication of the collective bargaining agreement or in any union elections. As a non-member I authorize the deduc
-tion from my earnings each payroll period of the union representation fee in accordance with the collective bargaining agreement and as allowed in RCW 41.80.100.
