2015 Work and Travel (WAT) Participant Orientation Checklist


( After the Orientation, sign and submit to FPI)
(Your J-1 sponsor requires that you, as a Participant on the Program, receive an orientation covering the most important information
you need to know prior to your departure from your country.)
By signing this document, I am confirming that all the following topics have been addressed and discussed during the orientation, and
that I understand the Terms and Conditions of the US Work and Travel program.

General Information I understand:

The purpose of the program


The culture in the United States (Business practices, diversity, differences between home country and the US, etc.)
That I should set up an OFFICIAL E-MAIL account following FPIs FORMAT (Example: lastname.firstname.wat2015@gmail.com)
That I should bring at least $1,000 on the plane to cover my first months living expenses (The sponsor cannot lend money to participants)
That I should avoid trouble with the law
How to call the Philippines from the US (Dial 011-63 - City Code-Phone number)

Housing I understand:

That housing must be arranged / secured before leaving my home country


That I should report any change of address within 3 days to the WAT sponsor (failure to do so will result in dismissal from the program)
That I must report any change of address within 10 days to the US Government submitting an AR-11 form (available from my sponsor)

Travel I understand:

That travel plans to my final destination must be arranged before leaving my home country
That travel and housing plans must be submitted to my program sponsor two weeks before departing from my home country

Rules and Regulations I have been informed of the following:

My Passport with valid J-1 visa, DS-2019 and CFO Sticker are required documents needed to enter the U.S.
Check-In / register SEVIS with program sponsor is required within 48 hours of arrival in the US either by calling your sponsors toll free
number or online checking through sponsors database. Failure to do so will result in being dismissed from the program.
Strict Monthly Monitoring by Program Sponsor and First Place Inc
Health Insurance is activated only upon Check-In with Program Sponsor. That I may acquire extra medical and accident insurance
covering 1 day before departure from the Philippines up to 1 week after arrival in the US and during the time that I am travelling after my
work dates up to 1 week after arrival in the Philippines.

Social Security I have been informed of the:

Procedures and delays when applying for a social security number


Social Security number is needed to be included in the payroll
Documents required to apply for my Social Security Card (Passport, DS-2019, Social Security letter, additional identification)
Additional Identification may be required

Host company I understand:

I must report to the host company listed on my DS-2019 and/or Job Offer
I must arrive and be ready to begin my WAT program on the start date listed on my DS-2019
I must know the name and location of my host company
The dates of my official program and legal employment
I must follow and respect company rules. Violations of rules will result to termination and cancellation of my visa
There is no changing host companies without contacting the program sponsor first and receiving the proper approval and guidelines
My host company can end my contract at their discretion

Visa Regulations I have been informed:

Of the rules for traveling outside the U.S. during the program
That there are no extensions to the J1 visa
No assistance with changes of status will be given
Of the 30-day rule (Grace Period) for departing the US, otherwise I will be out of status
Of the 2 year home stay rule ( Work and Travel participants not subject to this)
Issuance and expiration dates of J1 visa

Insurance Procedures I have been informed of:

Insurance Emergency Center information


Claims processing information
General guidelines of insurance coverage
Emergency Procedures (Dial 911 for immediate emergencies)
Sponsors toll free number and emergency number
What constitutes an emergency: seriously ill or injured, the victim of a crime, arrest
