Provide me copies of following documents:


1] The map signed by a certified architect showing the location of the
said telecom towers in the abovementioned premise submitted in
local ward office of municipality.
2] Permission letter / No Objection Certificate (NOC) for
construction of telecom towers on the terrace of said building by the
Town Planning Dept., KMC.
3] Type test certificate issued by Automotive Research
Association of India (ARAI) to the manufacturers of the Diesel
Generator (DG) Sets on rooftop of the said premise submitted to the
municipal authorities showing decibel emissions to be within sound
pollution norms as laid down by TRAI/EPA.
4] Standing Advisory Committee on Radio Frequency Allocation
(SACFA) application for the said location submitted to Wireless
Planning & Coordination (WPC) wing of DoT with registration
number and WPC acknowledgement along with undertaking that
in case of any objection/ rejection, TSPs/ IPs will take corrective
actions / remove the tower (Reference TRAI Regulation 13,
Subsection 2.23).
5] Copy of SACFA clearance certificate (NOC) for frequency
allocation to every telecom service providers antenna erected on the
towers of the abovementioned premise.
6] Acknowledgement receipt issued by TERM Cells (Dept. of
Telecommunications, DoT) of the self-certificate submitted by
Telecom Service Provider/ Infrastructure Provider to the said
premise/tower in respect of mobile tower/ BTS (roof top/ Pole/ wall
mounted) in the format as prescribed by Telecommunications
Engineering Center (TEC), DoT, establishing / certifying all General
Public areas around the tower will be within safe EMR exposure
limit as per peak traffic measurement after the antennae starts
radiating.
7] LBT dept., KMC, NOC to the said premise.
8] Every mobile service providers undertaking I and II of the said
tower submitted to local ward office, KMC.

9] Provide me with the inspection report copy of KMC Health


Dept.s survey on the impact on the health condition changes of the
citizens in 300 meters radius from the center of the said telecom
tower from the time of installation of the antennae.
10] If the health department has no expertise in conducting such a
survey, then did the KMC take external help from other government
agencies/departments such as Bhabha Atomic Research Center
(BARC) Radiation Dept.? Provide copy of the KMC departmental
meetings notations and/or reports regarding the same.
11] If no such survey was conducted, provide copy of the minutes /
notations of the discussion / meeting that decided not to conduct such
survey by KMC.
12] Provide me with the information as to how many telecom
antennae are installed on the said premise, along with all the
important information of the tower/s which shall include
a. Height from the terrace floor
b. Weight of antennae and tower
c. No. of Antennae
13] Provide me with the information as to the number of telecom
antennae that can be installed on any single premise as per
TRAI/KMC guidelines/rules.
