SCOPE

This purpose of this project is to create a database system that focuses on the services offered
by Tan Dental Clinic.
The scope of the project is:

To plan, design and build a database on which services offered by Tan Dental Clinic will

be managed.
The system will record all necessary information with regards to patients, treatments,

services, job orders, and supplies.

The system will determine the payment summary of the patients regarding their current

balance.
The system keeps track the records of the patients.

LIMITATIONS
The project doesnt cover the business side of Tan Dental Clinic.
The limitation of the project is:

Only authorized personnel are allowed to insert records in the system.

Only authorized personnel are allowed to delete records in the system.
Only authorized personnel are allowed to update records in the system.
Only authorized personnel are allowed to modify records in the system.
The system will not cover the calculations of the salaries of the employees.
The system will only be used to record patients payments, treatments, services, job
orders, and supplies.

BUSINESS RULES

Associate Dentists can handle other patients.

Associate Dentists are on-call; Associate Dentists dont have definite schedules
Clinic receives 50% share of associate dentists income from a patient.
Secretary can assist associate dentists and the primary dentist.
No exclusivity on lab works; Clinic orders job works depending on the specifications of

job order.
Dentist can request job order to laboratory of choice basing on specialization of the

laboratory.
Can buy clinic supplies from different suppliers.
Patient s either makes appointment or walk-in.
o Appointment patients make schedule when to be treated.
o Walk-in patients go to the clinic without prior appointments.
Clinic keeps a copy of OR.
Associate Dentist handles a patient depending on the case/treatment to be done and

availability of the Associate Dentist.

Patient fills up patient information and medical history form.
Dentist first does check-up and then gives advice / diagnosis before performing

treatment.
After treatment patient can pay in full or instalment.
Payments and instalments are recorded in the Patients Payment Summary.
All surgical cases require medical clearance.
Referral patients are required to submit a Referral Letter from another dentist / physician

before accepting the patient.

Waiver is needed for surgical procedures such as root canal, impacted tooth extraction,

etc.
Comprehensive case history is required before any treatment is done.
Patient must fill up form and answer interview before treatment.
