How to Refer a Patient

DeKalb Medicals LTAC is committed to

utilizing innovative, evidence-based medicine
to achieve optimal recovery and results for
our patients. Care is delivered using the latest
science in a cost-effective setting that leads to
high patient satisfaction and return-to-home
rates. With specialized accommodations
such as beds that enable progressive mobility,
most patients attain treatment goals within
a two- to six-week length of stay. In addition
to intensive therapy and around-the-clock
nursing and respiratory care, we provide the
following services:

Once a physician referral has been made, well come

to the prospective patients bedside to conduct a free
pre-admission screening. Within 24-hours of the clinical
evaluation, well either proceed with the patient transfer
and admission to LTAC or make recommendations for a
healthcare setting that is most appropriate for the
patients needs.
For more information or to schedule a tour or make a

Who We Serve
LTAC is ideal for patients, with multiple
diagnosis or health challenges, who need more
time to recover under the care of a team that
will meticulously monitor their every need. Care

for intensive care. Here, we can offer them
the extra time to recover under the care of an
interdisciplinary team that will evaluate and
cultivate a multi-modality rehabilitation program
for maximal results. Working hand-in-hand with
our physician partners to develop and carry out
multifaceted programs that maximize recovery
potential, we can effectively treat the
