Abstrak: The United States (U.S.) spent the most on healthcare at 17.9% of the gross domestic product in 2010
compared to other countries. Unfortunately, the U.S. is not getting the expected quality results for this costly
outlay: the country ranks only 37 out of 200 countries in overall health system performance due to a fragmented
delivery model where insurers pay hospitals by cost per procedure and pay physicians by volume and
productivity. As providers take more responsibility for improving care delivery through coordination, they
demand timely access to relevant healthcare quality data for increased cost savings and payment. Past efforts
to reduce healthcare spend focused on cuts to providers. Instead of simply reducing payments, Congress
through the 2010 Healthcare Reform Acts, strived to create new efficient, cost effective care delivery models
that provide higher quality. However, the willingness of physicians to embrace requirements and change care
decisions is not yet clear. How will payment reform and mandatory reporting under accountable care influence
physicians' care decisions? Using qualitative phenomenology methodology, this study assessed Chief Medical
Officer, cardiologist, primary care physician, health plan leader and industry expert reactions to the care
implications under accountable care. This qualitative study provides useful insights from physician interviews on
the key components necessary for successful care coordination, alignment and accountability under emerging
payment and delivery models. Long-term, physicians see the new delivery models possible only under strong
leadership and governance aligned across the continuum of appropriate care and anchored by a value-based
shared culture of accountability.
