JUDICIAL AFFIDAVIT
OF IRISH AYAO
The Judicial Affidavit of Irish Ayao is executed to serve as her direct testimony in the
instant case. The Judicial Affidavit is offered to prove: (1) all the representation in the
Comlaint including all annexes appended thereto, which he respectfully requests to be
correspondingly marked as EXHIBITS in this case; and (2) all other related matters, facts
and circumstances relevant and material to this case, with reservation to present additional
exhibits in the course of the proceedings of the instant case.
Questions were propounded by petitioners counsel, Atty. Theresa Ventura, at his
office in the 7th Floor, Ayala Life Bldg., Cebu Business Park, Cebu City, in English, which
the affiant fully understands. The questions are numbered consecutively and each question
is followed by the answers.
Questions and Answers
Q1. Do you swear to tell the truth and nothing but the truth?
A1. I do.
Q2. Are you aware that you may face criminal liability for the false testimony or
perjury if you will not tell the truth?
A2. I am.
Q3. Please state your name, age, address and occupation?
A3. I am Irish Ayao, 23 years old, single, and residing at 123 Hitandrun Street,
Lahug, Cebu, Philippines, and an an employee of Lovebirds Company, Lahug, Cebu City.
Q4. How did you know and meet the accused in this case, Hans Fei Coy?

A4. Hans and I were officemates at Loverbirds Company in Lahug, Cebu City. Our
office mate, Mary Jane, introduces me to Hans. Hans then courted me sometime in January
2013.
Q5. Did you accept his proposal to be his girlfriend? Why?
A5. Yes, because I fall in love with him the moment I saw him at that time, it was
love at first sight.
Q6. Describe the courtship?
A6. The courtship didnt took long because I said yes immediately when he
proposed to me, that is how much I love him.
Q7. How about the relationship after the two of you became sweethearts?
A7. Our relationship was great and just right. We are both in love and crazy with
each other. The best feeling ever, but great things has always an end. He broke up with me
on November 2013.
Q8: Why did he break up with you?
A8: I dont really know until now I keep on asking myself WHY? He broke my
heart into pieces, and I was terribly hurt. He was my everything. But I think, he left me for
another woman, who then became his wife. On December 2013, I found out that he is
already married.
Q9: How did you know that he is married?
A9: On December of the same year, it as Hans who told me. He got in touch with me
again and continued making phone calls. He tried to convince me to elope with him.
Q10: What did you say then?
A10: I rejected his proposal and stopped answering his calls. I move away/distanced
from him and told him to take his responsibility and be a good husband to his wife.
Q11: Did he stop calling you after that?
A11: No, Hans still continued making phone calls, he even went to my house several
times on different occasions.
Q12: Why is he in your house?
A12: He is trying to convince me to elope with him.
Q13: What did he say or do when you refused?
A13: He still keeps on convincing and insisting me to elope with him, but I
consistently refused every time he asked and begged me. However, he cannot understand
and accept it. On several occasions he beat me. In one instance, he grabbed me on both
arms and shook me with such force that caused bruises. At another instance, he beat me on
the chest and slapped me many times, and hit me forcefully on my lips that caused some
bleeding.
Q14: Can you remember the exact dates that he beat you?
A14: No, I cant remember all the exact dates, but I can remember that one instance
was on December 20, 2013. I went to the Cebu City Medical Center after the incident for
medical assistance on the injuries that I sustained. I have with me the medical certificate
issued by the said hospital (handling over the documents).
Q15. I am marking this document as Exhibit A, and Ill attach this to your judicial
affidavit to form part of it. Do you confirm my action?
A15. Yes, I confirm.
Q16. Why are you filing a case against Hans Fei Coy?
A16. I wanted to have a peaceful and undisturbed life for myself and my baby. I am
asking for support for my baby while the case is still pending, and Temporary Restraining
Order. I also asked for Barangay Protection Order/Injunction against Hans Fei Coy for our
safety.
Q17: What else do you want?
A17: Only those, nothing else.

1. I personally conducted the interview of IRISH AYAO of 123 Hitandrun Street,
Lahug, Cebu, Philippines, complainant in Criminal Case No. 01234 of the
Regional Trial Court of Cebu City;
2. I faithfully recorded the questions I asked to Ms. Ayao and the corresponding
answers she gave me; and
3. Neither I nor any other person then present coached Ms. Ayao regarding her
answers.
