3. Improper AB cable installtion practices found, cable
was folded at two places. No proper arrangement
available to lift the cable incase if it stuckup with
existing LT line network. Suggested bamboo "T"
structure to be used to support the cable while
stringing & trained them on site.(Supervision
negligence & Unskilled labour)

3. Extra AB Cable laying on ground seems tobe laying

away from pole though space was available.

Minimum level need to topup the same.
3. Need to correct the Oil level indicator
(MOG) on Conservator tank from Kryfs
(vendor) as its not working (Need to fill
Nitrogen gas inside a float. Above both
activity should be at one GO to avoid
multiple shutdown problem.
4. At Chapra store found some transformer
oil leaking need to take corrective action.

Definition
Any nonconformity which may result in hazardous or unsafe conditions for individuals using,
maintaining or depending upon the product or prevent performance of a vital agency mission
Any nonconformity other than critical, which may result in failure or materially reduce the
usability of the product for the intended purpose (i.e. effective use or operation, weight or
appearance (where a factor), health or safety) and which can not be completely eliminated by
rework or reduced to a minor nonconformity by an approved repair.
Any nonconformity which does not adversely affect the performance, durability,
interchangeability, reliability, maintainability, effective use or operation, weight or appearance
(where a factor), health or safety of a product. Multiple minor nonconformities when
considered collectively may raise the category to a major or critical nonconformity.
