Occurrence : Fire of Distribution Pipe caused by Illegal Tapping

The Event Scenario

Pipe sized 16 that installed on the side road is in running condition to deliver Crude Oil from production field to the Oil &
Gas Refinery, suddenly it is leaking followed by oil bursts and lead to fire and explosion due to the flame from smoking
that not too far from the scene.
This emergency resulting 3 casualties, 2 persons get serious burns, 4 persons get medium and minor burns and 3 houses
get fire. The big open fire from oil bursts and 3 fired houses making the road traffic at the scene is totally stop.
The fire reported by community to the Local Fire Department and forwarded to the Call Center of Pertamina and Fire
station to ask for emergency response.
HSE Shift Supervisor at Fire station reports the emergency to the on scene commander and forwarded to the incident
commander as well as Emergency Response Commander
Incident Commander declares the situation as emergency after getting the approval from Emergency Response
Commander and commanding to sound the emergency siren therefore the team involves in Emergency Response
Organization conducting the response action at the scene.
Incident Commander commanding the On Scene Commander along with Fire Brigade, Rescue Team and completed with
Foam/Water Crash Tender as well as Medical Team and ambulance straight to the scene.
Emergency Response Accomplished and Incident Commander by the approval from Emergency Response Commander
declares that condition is safe at the scene.

Emergency Situation and Condition

The leakage of Crude Oil Distribution Pipe sized 6 causes many victims, properties
and the traffic is totally stop.
Incident Commander declares the situation as emergency after getting the approval
from Emergency Response Commander and commanding to sound the emergency
siren
HSE Supervisor sounds the emergency siren as the notification that emergency
occurs and all officers involve in Emergency Response Organization conducting the
task and responsibility as meet procedure.
Then the On Scene Commander coordinates the emergency response with
Emergency Response Team , Fire Brigade, Rescuer Team and Medical Team with
Fire Water Crash Tender and Ambulance at the scene.

Security Team arrives at the scene and evacuate the community at the vicinity area.
Security officer assists the Police Officer controlling the traffic by prioritizing fire crash
tender and ambulance.
Security controls regarding the possibility of any personals entering within the hazard
range about 48 m (4 times of the height of fire-U.S. Fire Fighting Practices).
Security officers keeps securing around the scene until the condition is stated safe.

1. When just arrived at the scene and there are many victims thus, Rescue Team
1 coordinates with Medical Team regarding the victims evacuation and reporting to
Incident Commander.

2. Rescue Team evacuates those victims and medical team takes first aid as meets
priority:
- Helping 2 persons with serious burns at the scene of explosion.
- Helping 4 persons from the community who get medium and minor burns at the
scene.
- Evacuating 3 casualties at the scene as the perpetrators of Illegal Tapping.
3. The victims with serious and medium burns brought by ambulance straight to the
nearest hospital to have further medical treatment.

After receiving the emergency information, HSE Shift Supervisor at Fire Station
reports to the On Scene Commander forwarded to Incident Commander and
Emergency response Commander.

On Scene Commander dispatches Foam/Water Crash Tender along with Fire

Brigade to the scene followed by Rescue team and Medical Team as well as
ambulance.

On Scene Commander orders the Auxiliary Fire Team to conduct fire fighting towards
the 3 houses of community using Water Crash Tender completed with 3 units water
Sprayer Nozzle 2,5.
For the fired oil bursts then Fire Brigade doing fire fighting using Water Crash Tender Fire Brigade
completed with Water Sprayer Nozzle 2,5 but it is not successfully extinguished due
to the oil is so wide spread.
On Scene Commander coordinates with Pump House Supervisor to stop pumping On Scene Commander
activity and shut off the discharge valve for stopping the oil bursts.

Based on concerning the size of open fire which is 12 m height and situation as well
as the condition of the hectic road therefore to fasten the fire fighting operation, On
Scene Commander commanding fire brigade to do fire fighting by using Foam Crash
Tender by utilizing Foam Monitor/Branch Pipe thus the fire can be extinguished
rapidly.

Pumping activity and shutting off the valve is accomplished and then On Scene
Commander ask for Technical Maintenance to clamp temporary in order to stop the
oil bursts.
To prevent from back fire, the HSE Officer and Security conducting:
- Testing and monitoring the Hydrocarbon gas condition regularly.
- Controlling the ignition source that can emerge sparking at the vicinity area.
- Controlling the traffic to stay away from oil spill.
