COMES NOW, Petitioner,
respectfully alleges that:

Petitioner is of legal age, Natural-born Filipino Citizen, Single,

born on September 4, 1987 at City of Navotas, presently residing at 53
F. Cruz St. Tangos Navotas City with telephone number 282-36-06; and
with office address at 1611 Laon-Laan Cor. Navarra Streets, Sampaloc,
Manila, Philippines with office numbers 743-21-16;

Petitioner is a member of good and regular standing of the

Philippine Bar under Attorneys Roll No. 50794 , having been admitted
as such on May 18, 2006 and duly authorized to practice law under the
laws of the Republic of the Philippines as supported by a photocopy of
the Certificate of Membership in the Philippine Bar issued by the Office
of the Bar Confidant, Supreme Court of the Philippines, hereto attached
as ANNEX A and forms as an integral part hereof;

Petitioner is a registered member of the Integrated Bar of the

Philippines under good standing with IBP No. 773124 issued on December
4, 2008 at Las Pinas City as supported by a photocopy of the said
official receipt issued by the Integrated Bar of the Philippines and
hereto attached as ANNEX B and forms a integral part hereof;

Petitioners professional fee license corresponding to the current

year had been paid, as shown in PTR Official Receipt No. 7123948
issued on January 26, 2009
at Manila City and a
photocopy of the same is hereto attached as ANNEX C and forms
an integral part hereof;

Petitioner possesses all the qualifications and none of the

disqualifications for the said Notarial Commission as Notary Public and
was read, understood and is willing to abide with the 2004 Rules on
Notarial Practice which took effect on August 1, 2004;

Petitioner is of good moral character, has never been accused or

convicted of any crime involving moral turpitude and has likewise not

been a respondent in any administrative case in connection with his

present position;

Attached and made part of this petition are specimens of his

notarial seal prepared in accordance with Sec.2, Rule VII of the 2004
Rules on Notarial Practice to be approved by the Honorable Court as
ANNEX D and form an integral part hereof;

Petitioner is likewise attaching herewith his three (3) passport

size colored photograph with light background , taken thirty (30) days
before the filing of this application with signature at the bottom part
thereof as likewise required for the herein petition, ANNEX E;

9.

Also attached herewith and forms integral parts hereof are the
following documents;
a) Specimen signatures of the herein petitioner, ANNEX F;
b) Certification from the Integrated Bar of the Philippines on Bar
Compliance, ANNEX G;
c) Certification from the Bar Confidant of the Supreme Court, ANNEX
H;
d) Certificate of Good Standing issued by the Local Chapter of Manila,
Integrated Bar of the Philippines, ANNEX I;
e) Sketch of Office Address, ANNEX J;
f) Picture of Law Office, ANNEX K;
g) Photocopy of latest Community Tax Certificate, ANNEX L;

Finally, attached is the official receipt showing Proof of
Payment for the filing of the petition as required by the said 2004 Rules
on Notarial Practice, ANNEX M.
WHEREFORE, it is most respectfully prayed THAT;
(a) Petitioners appointment as NOTARY PUBLIC for and in the National
Capital Judicial Region covering the City of Manila be granted from
2014-2015;
(b)Petitioners notarial seal impression be approved, and a Certificate
of Authorization to Purchase Notarial Seal as provided for under
Section 9, Rule III of the 2004 Rules of Notarial Practice, and
Certificate of Authority for a Notarial Act as provided under Sec. 1,
Rule IX of said rule be issued;
(c) Petitioner further prays for such other relief and remedies that are
just and equitable within the premises .
