Sports Education Assistance Submission Guidelines


**Effective May 5, 2015**

All Sport Education Assistance forms are to be submitted to the Recreation


Department each Tuesday.

Any Sport Education Assistance forms submitted to the Recreation


Department after Tuesday will then be submitted to the Finance Department the
next following Tuesday.

Sport Education Assistance Travel and Meal submissions are required to be


submitted two (2) weeks prior to your childs scheduled tournament/competition
date. Failure to comply with our Sports Education Assistance Travel and Meal
submission process will affect your childs cheque from being released in a timely
fashion.

Sport Education Assistance forms can be obtained online at


sipeknekatik.ca/sport-education-assistance-submission-guidelines-form/ ,at the
Sipeknekatik Band Administration Office receptionist desk or at the Recreation
Department (second floor of the Sipeknekatik Multipurpose Centre).

Sport Education Assistance can be submitted by email to


joanpaul@sipeknekatik.ca, by fax to (902) 236-3021 or dropped off in person to
the Recreation Department or at the receptionist desk at the Sipeknekatik Band
Administration Office.

Parents/Guardians are responsible for obtaining your childs Sport Education


Assistance cheque (s).

Sport Education Assistance cheque (s) will not be mailed or personally


delivered to your residence.

Please be advised there are six (6) pages to the Sports Education Assistance
Program:
Pages 1 and 2: Submission Guidelines, information on submitting Sport
Education Assistance Forms.
Page 3 and 4: Sport Education Assistance Guidelines, basic information
on the Sport Education Assistance Program.
Page 5: Sport Education Assistance Form. This form needs to be
completed and returned to the Recreation Manager every time youre
seeking Sport Assistance.
Page 6: Child bio. This form needs to be filled out once (1) per fiscal year
(April 1 March 31).

Sipeknekatik Youth Sport Education Assistance


Program Guidelines
The Sipeknekatik Youth Sport Education Program provides assistance to:

All children and youth registered with the Sipeknekatik Band

The following assistance may be provided, so long as a copy of the tournament


schedule, acceptance letter or registration letter is submitted to the Sipeknekatik
Community and Youth Recreation Coordinator via email at joanpaul@sipeknekatik.ca or
by fax to (902) 236-3021.

Travel Assistance = totaling $100 once per fiscal year - April 1st to March 31.

Meal Assistance = totaling $100 once per fiscal year - April 1st to March 31.

An organized sport not exceeding the amount of $300 once per fiscal year - April
1st to March 31. If your childs sport should be a lesser value of $300, he/she is
then eligible to claim up until the value of $300 is reached.

To be eligible to claim travel and/or meal assistance, a Sipeknekatik child/youth must


already be registered for the sport they are seeking assistance for, throughout the sport
season/year.
For all Aboriginal hockey tournaments, a letter from your childs division coach must be
attached with the Sport Education Assistance Form along with a tournament schedule.
The Sipeknekatik Youth Sport Education Program does not provide assistance for any of
the following:

Equipment of any kind, this includes attire/uniforms


Late fees/cost
Materials/Supplies
Team fees/dues
Team and/or individual photos
Insurance fees/cost

Parents, guardians please note: if your child(ren) has not used all their Sport Education
Assistance eligible to them, the remainder does not get carried over into the next fiscal
year. (New year, new budget)
