The Jefferson Parish Council of Jefferson Parish, Louisiana, met in regular
session on Wednesday, April 29, 2015 at ten (10:00) oclock a.m. in the Council
Chambers, Joseph S. Yenni Building, Jefferson, Louisiana.
Mr. Christopher L. Roberts, Chairman of the Jefferson Parish Council presided,
called the meeting to order. Parish President Young requested moment of silence
for Troy Bradberrys Father and Mother who died recently. Eula A. Lopez
recited a prayer. Mr. Roberts, Council Chairman lead everyone in the Pledge of
Allegiance to the Flag of the United States of America.
The Chairman then requested the Parish Clerk to call the roll. The following
members were present: Christopher L. Roberts, Chairman, and Councilmembers:
Ricky Templet, Paul D. Johnston, Mark Spears, Elton M. Lagasse, Ben Zahn,
and Ms. Cynthia Lee-Sheng.
The following members were absent: None
The following were in attendance: John F. Young, Parish President; Ms. Jennifer
Van Vrancken Dwyer, Chief Operating Officer; Ms. Deborah Foshee, Parish
Attorney; Directors of the following Departments: Ms. Terri Wilkinson, Director
Planning; Timothy Palmatier, Finance; Kazem Alikhani, Public Works; Alan
Gandolfi, Research Director; Jeremy Dwyer, Legal Analysis Director; Sonny
Burmaster, Chief of Staff; and Eula A. Lopez, Parish Clerk.
The following members were absent: None
The Chairman called upon the Honorable John F. Young, Jr., Parish President to
address the Council at this time.
The next item of business was adoption of the consent agenda.
It was on joint motion of all Councilmembers that the Consent Agenda be
adopted except Item 32.
Vote on the adoption of the foregoing motion resulted as follows:

The motion was declared to be adopted on this the 29th day of April, 2015.
On motion of Mr. Spears, seconded by Mr. Roberts, the following resolution
was offered:
RESOLUTION NO. 124792
A resolution selecting a veterinarian to provide professional veterinary services
at the West Bank Animal Shelter on a full-time or part-time basis. (Council
District 3).
WHEREAS, pursuant to Resolution No. 123879 , adopted the 5th day of
November, 2014, the Jefferson Parish Council authorized its Council Clerk to
advertise for Statements of Qualifications for a one year contract for veterinarians
interested in providing professional veterinary services at the West Bank Shelter
on a full-time or part-time basis; and
WHEREAS, the responses to Statements of Qualifications were received on the
4th day of December 4, 2014; and
WHEREAS, the Evaluation Committee met on the 2nd day of February, 2015;
and
WHEREAS, the Council Chairman was notified of the results of the Evaluation
Committee by letter dated February 6, 2015.
NOW THEREFORE, BE IT RESOLVED by the Jefferson Parish Council of
Jefferson Parish, State of Louisiana:
SECTION 1. That the Jefferson Parish Council does hereby select Kenneth M.
Allen, DVM, 906 Mazant Street, New Orleans, LA 70117 to provide professional
veterinary services at the East Bank Animal Shelter on a full time or part-time
basis.
SECTION 2. That the Administration shall negotiate a contractual agreement
with Kenneth M. Allen, DVM, and said agreement shall be submitted to the
Council in complete form, including all terms and conditions, for approval,
authorization and ratification by Council Resolution prior to execution of said
agreement.
SECTION 3. That all costs associated with this agreement shall be charged to
Account 21710 -2200-7331.

On motion of Mr. Lagasse, seconded by Mr. Roberts, the following resolution
was offered:
RESOLUTION NO. 124793
A resolution selecting from persons or firms interested and qualified to provide
Professional Architectural and Engineering Services on an as-needed basis for
projects located throughout the Parish (Parishwide)
WHEREAS, Resolution No. 124207, adopted on January 14, 2015, authorized
the Jefferson Parish Clerk to advertise for the submittal of a Statement of
Qualifications to be reviewed by the Technical Evaluation Committee from
person or firms interested and qualified to provide Professional Architectural
and Engineering Services on an as-needed basis for projects located throughout
the Parish; and
WHEREAS, the Technical Evaluation Committee met on April 1, 2015 to
review all submittals; and
WHEREAS, in accordance with Resolution No. 124207, the Council, at its
discretion, may choose one or more of the qualified firms to provide these
services.
NOW, THEREFORE, BE IT RESOLVED by the Jefferson Parish Council of
Jefferson Parish, the governing authority of said Parish:

WHEREAS, pursuant to Resolution No. 124208, adopted January 14, 2015, the
Jefferson Parish Council authorized the Purchasing Department to advertise
for Request for Proposals to firms interested and qualified in contracting with
Jefferson Parish to provide electronic monitoring equipment and tracking
services for Jefferson Parish under RFP No. 0329; and
WHEREAS, technical requirements of these services dictate that price should
not be a sole criterion, but rather vendor qualifications must be a factor; and
WHEREAS, community safety is of paramount concern, therefore use of the
RFP is in the best interest of the Parish in that it allows the Parish to evaluate the
vendors experience and equipment reliability as required in the specifications
provided in the RFP; and
WHEREAS, the Evaluation Committee met on the 8th day of April, 2015; and
WHEREAS, the Council Chairman was notified of the results of the Evaluation
Committee by letter dated the 9th day of April, 2015.
NOW THEREFORE, BE IT RESOLVED by the Jefferson Parish Council,
Jefferson Parish, Louisiana, acting as the governing authority of said Parish:
SECTION 1. That the Jefferson Parish Council does hereby select BI Incorporated
to provide electronic monitoring equipment and tracking services for offenders
ordered by the Jefferson Parish Courts to the Adult Home Incarceration Program
and the Department of Juvenile Services Electronic Monitoring Program under
RFP No. 0329.
SECTION 2. That the Administration shall negotiate a contractual Agreement
with selected vendor and said Agreement shall be submitted to the Council in
complete form, including all terms and conditions, for approval, authorization
and ratification by Council Resolution prior to execution of said agreement.
SECTION 3. That the costs associated with this Agreement will be charged to
Account No. 10010-0120-027-7443.
SECTION 4. That the Chairman of the Jefferson Parish Council, or in his absence
the Vice-Chairman, are hereby authorized to execute any and all documents
necessary to give full force and effect to this resolution.

RESOLUTION NO. 124801
A resolution of no objection to the location and use of a snowball stand at 200
Hector Avenue, Terrytown, LA 70056, to be operated by Mehmet Kalenden
during the period of May 1, 2015 to October 31, 2015. (Council District 1)
WHEREAS, a letter of no objection from property owners, within 300 in
all directions of the proposed location, will be submitted to the Director of
Inspection and Code Enforcement, and
WHEREAS, no additional parking shall be required for the installation of the
snowball stand at this location.
NOW, THEREFORE, BE IT RESOLVED, by the Jefferson Parish Council of
Jefferson Parish, Louisiana, acting as governing authority of said Parish:
SECTION 1. That this Council does hereby have no objection to the location
and use of a snowball stand at 200 Hector Avenue, Terrytown, LA 70056, to be
operated by Mehmet Kalenden during the period of May 1, 2015 to October 31,
2015, provided all provisions of Resolution Nos. 16122, 25536, and 39499 are
met.
SECTION 2. The Council Clerk is hereby requested to forward a copy of this
resolution to Mr. Mehmet Kalenden, 200 Hector Avenue, Terrytown, LA 70056
and the Department of Inspection and Code Enforcement.

RESOLUTION NO. 124802
A resolution accepting the lowest responsible bid of BLD Services, LLC received
on April 2, 2015, by the Jefferson Parish Purchasing Department, in the amount
of $1,185,000.00, for the Rehabilitation of Landfill Leachate Collection System,
Project No. 2014-040-ENV, Proposal No. 50-112464, in accordance with the
plans and specifications prepared by Infinity Engineering Consultants, LLC, for
the Department of Environmental Affairs. (Council District 3)
WHEREAS, Jefferson Parish received bids on April 2, 2015 for the Rehabilitation
of Landfill Leachate Collection System, Project No. 2014-040-ENV, Proposal
No. 50-112464; and
WHEREAS, one bid was received by the Department of Purchasing for this
project; and
WHEREAS, BLD Services, LLC. submitted the lowest responsible bid for
Proposal No. 50-112464 in the amount of $1,185,000.00 for the Base Bid; and
WHEREAS, Infinity Engineering Consultants, LLC, the design engineer for

this project, has reviewed the bid and has recommended that award of contract
be made to the lowest responsible bidder; and
WHEREAS, there are sufficient funds available for the Base Bid.
NOW, THEREFORE, BE IT RESOLVED by the Jefferson Parish Council, the
governing authority of said Parish:
SECTION 1. That the lowest responsible bid of BLD Services, LLC received
on April 2, 2015 for the Rehabilitation of Landfill Leachate Collection System,
Project No. 2014-040-ENV, Proposal No. 50-112464, in accordance with the
plans and specifications prepared by Infinity Engineering Consultants, LLC for
the Department of Environmental Affairs, be, and is, hereby accepted.
SECTION 2. That the Chairman of the Jefferson Parish Council or, in his
absence, the Vice-Chairman be and is hereby authorized, empowered, and
directed to sign and execute a contract with BLD Services, LLC., which contract
shall include the bid proposal, plans, and specifications for said project prepared
by Infinity Engineering Consultants, LLC.
SECTION 3. That the cost of this construction is to be funded from Account No.
45900-4061-7454 (59010.035).

On joint motion of all Councilmembers present, the following resolution was
offered:
RESOLUTION NO. 124803
A resolution accepting the lowest responsible bid from Cimsco Inc. under
Proposal No. 50-112446 for a one (1) year contract for the supply of couplings
at the tabulation price of $95,622.96 for the Department of Public Works,
Water Department (All Divisions). Maximum expenditures shall not exceed
$200,000.00 for the one (1) year period and compensation shall not exceed this
amount by a factor of more than 10% without an amendment to the contract
authorized by resolution of the Council (Parishwide).
WHEREAS, there were three (3) bids received for Proposal No. 50-112446; and
WHEREAS, Cimsco submitted the lowest responsible bid; and
WHEREAS, maximum expenditures shall not exceed $200,000.00 for the one
(1) year period; and
WHEREAS, expenditures under the previous one (1) year contract No. 55-14006
(12/12/13-12/11/14) with Cimsco Inc. were $1,795.20 (thru 12/11/14); and
WHEREAS, expenditures under the previous one (1) year contract No. 55-14007
(12/12/13-12/11/14) with LA Utilities Supply were $222,584.33 (thru 12/11/14);

WHEREAS, expenditures under the previous one (1) year contract No. 55-14008
(12/12/13-12/11/14) with HD Supply Waterworks were $274.80 (thru 12/11/14);

NOW, THEREFORE, BE IT RESOLVED by the Jefferson Parish Council, the
governing authority of said Parish:
SECTION 1. That the lowest responsible bid received from Cimsco Inc. under
Proposal No. 50-112446 for a one (1) year contract for the supply of couplings
at the tabulation price of $95,622.96 for the Department of Public Works, Water
Department (All Divisions) is hereby accepted.
SECTION 2. That the Department of Public Works, Water Department be
permitted to use the services of Cimsco, Inc. for the supply of couplings, in the
interim for a period not longer than 60 days.
SECTION 3. That expenditures are to be charged to Account Number 7454 of
the user department.
SECTION 4. That the cumulative total of expenditures under Proposal No. 50112446 shall be limited to $200,000.00 for the one (1) year period for Cimsco
Inc. Compensation shall not exceed this amount by a factor of more than ten
percent (10%) without amendment to the contract authorized by resolution of

Development Code (UDC), Subdivision Waivers, the insufficient lot depth for
Lots 106A1, 108B2, and 108B3 and the insufficient rear yard setback of 5.7 feet

transportation, and
WHEREAS, the progression of technology has allowed for innovations in
transportation that allow for safe and affordable transportation options, and
WHEREAS, the Jefferson Parish Council desires to regulate the area of
transportation network services in order to allow for the availability of
transportation solutions to the citizens and visitors of Jefferson Parish and to
provide for public protections relative to those services.

(A) Such general appearance inspection of the vehicle shall include the exterior
of each vehicle, ensuring the vehicle is free of gross damage (dents larger than
six (6) inches in any direction) and that the vehicles has factory-finish grade
paint.
(B) The general appearance inspection shall further include an interior
inspection of each vehicle to ensure the vehicle interior is maintained in a
reasonable condition, including door panels, arm rest, floor covering and seats
that are free of holes, snags and soiled spots.
(7) Establish a uniform logo, insignia, decal or trade dress for use on a motor
vehicle at any time a motor vehicle is providing or arranging to provide
transportation network services;
(8) Maintain a commercial automobile liability insurance policy that:
(A) Provides primary coverage of at least one million dollars ($1,000,000.00) per
incident for accidents involving a transportation network operators operation of
the vehicle regardless of whether the transportation network operator maintains
personal insurance adequate to cover any portion of a claim. Coverage required
by this section applies from the moment a transportation network operator
accepts a ride request and continues during an accepted trip until the operator
completes the trip or the passenger that requested the trip exits the vehicle at the
passengers intended destination, whichever is later;
(B) Provides uninsured/underinsured motorist bodily injury coverage of at least
one million dollars ($1,000,000.00) per incident;
(C) During the time that a transportation network operator is available for
service, but not providing service, provides additional bodily injury coverage of
at least $50,000 per person and at least $100,000 per accident, and coverage of at
least $25,000 for property damage per accident, in the event that the operators
personal insurance policy does not pay.
(D) Provides that written notice shall be given to PMZ upon any cancellation or
termination of the policy.
(E) Is placed with an admitted insurer licensed under the Louisiana Insurance
Code or with a surplus lines insurer eligible under the Louisiana Insurance Code.
(9) Establish a program of random drug testing for transportation network
operators in accordance with the standards set forth in Section 38-95(c)(6) above
and a zero tolerance policy on the use of drugs or alcohol while transportation
network operator is providing transportation network services;
(10) Immediately suspend a transportation network operator upon receiving a
passenger complaint alleging that the operator violated the zero tolerance policy.
Such suspension shall last the duration of the investigation;
(11) Conduct an investigation when a passenger alleges that an operator violated
the zero tolerance policy required by paragraph (9);
(12) Maintain records relevant to the requirements of this section, for a period
of 3 years; and
(13) Before doing business in Jefferson Parish, submit an application for a permit
to PMZ that must include:
(A) Proof that the company is licensed to do business in the State of Louisiana;
(B) Proof that the company maintains a registered agent and a principal place of
business in Jefferson Parish;
(C) Proof that the company maintains a website that includes the information
required by paragraph (4) of this section;
(D) Proof that the company has established a uniform logo, insignia, decal, or
trade dress required by paragraph (7) of this section; and
(E) A certification under penalty of perjury that the company has complied with
the requirements of this ordinance, including, but not limited to the registration
requirements for transportation network operators.
(F) Proof of the required insurance under Section 38-96 (a)(8), naming PMZ as
a certificate holder.
(G) A permit fee of $100,000.00 payable to PMZ.
(14) Establish a policy prohibiting a transportation network operator from
operation for more than twelve (12) consecutive hours within a twenty-four (24)
hour period.
(15) Make available vehicles that comply with the minimum guidelines
and requirements for accessibility standards established for vehicles by the
Americans With Disability Act (36 C.F.R. 1192.1 et seq.; 49 C.F.R. parts 37 and
38 and 42 U.S.C. 12101 et seq.) to individuals that can select such vehicles directly
on the digital network or software application provided by the transportation
network application company.
(16) Immediately suspend the transportation network operators registration with
the transportation network application company concurrent with the suspension
or revocation of the transportation network operators drivers license.
(b) A transportation network application company shall not disclose a
passengers personally identifiable information to a third party unless: the
passenger consents, disclosure is required by a legal obligation, or disclosure
is required to protect or defend the terms of use of the service or to investigate
violations of those terms. In addition to the foregoing, a transportation
network application company shall be permitted to share a passengers name
and/or telephone number with the transportation network operator providing
transportation network services to such passenger in order to facilitate correct
identification of the passenger by the operator, or to facilitate communication
between the passenger and the operator.
(c) A transportation network application company shall transmit a $100,000.00
annual permit fee to PMZ, due on or before January 15th of each year.
(d) A transportation network application company shall prohibit any
transportation network operator licensed in a jurisdiction or political subdivision
other than Jefferson Parish from providing services in the Parish of Jefferson,
except that such transportation network operator shall be permitted to transport
passengers from such other jurisdiction or political subdivision to or through the
Parish of Jefferson.
Sec. 38.97 Requirements for transportation network operators.
(a) A transportation network operator shall:
(1) Exclusively accept rides booked through a transportation network application
companys digital platform and shall not solicit or accept street-hails;
(2) Use the required logo, insignia, or trade dress in accordance with the
standards established pursuant to Section 38-96(a)(7) above, at any time that
the operator uses his or her motor vehicle to provide transportation network
services;
(3) Possess a valid Louisiana drivers license;
(4) Possess proof of Louisiana registration for the motor vehicle used for
transportation network services;
(5) Possess proof of motor vehicle insurance with an admitted insurer licensed
under the Louisiana Insurance Code or with a surplus lines carrier eligible under
Louisiana Insurance Code for the motor vehicle used for transportation network
services; and
(6) Be at least 21 years of age.
(b) If an accident occurs involving a motor vehicle that is being used for
transportation network services, including any time the transportation network
operator is logged into or otherwise using the software application or network,
the transportation network operator shall provide proof of:
(1) The operators personal auto liability insurance; and
(2) The transportation network application companys automobile liability
coverage; provided, that a transportation network operator shall have twentyfour (24) hours to provide proof of excess liability coverage.
(c) A transportation network operator shall not communicate via electronic mail,
text message, instant message or any other electronic message format while
carrying a passenger in a vehicle providing transportation network services.
(d) A transportation network operator shall not provide transportation network
services more than twelve (12) consecutive hours within a twenty-four (24)
period.
Sec. 38.98. Charges.
(a) A transportation network application company may offer service at nocharge, suggest a donation, or charge a fare; provided, that if a fare is charged,
a transportation network application company shall disclose the fare calculation
method, the applicable rates being charged, and the option for an estimated
fare to a passenger before the passenger arranges a trip with the transportation
network application company.
(b) Upon completion of a trip, a transportation network company shall transmit
an electronic receipt to the passengers electronic mail address or mobile
application that lists:
(A) The origin and destination of the trip;
(B) The total time and distance of the trip; and
(C) A breakdown of the total fare paid, if any.
(c) A transportation network application company may impose surge pricing
subject to the following conditions: (i) surge pricing shall be disclosed in
accordance with the terms and conditions set forth in subsection (a) above; and
(ii) the provisions set forth in La. R.S. 29:732 (Louisiana Price Gouging Law)
shall be applicable to any surge pricing.
Sec. 38-99 Requirements for Operation at Louis Armstrong International
Airport
Transportation network services and/or transportation network operators
shall not operate at Louis Armstrong International Airport unless specifically
authorized to do so by the New Orleans Aviation Board.
Sec. 38.100. Enforcement.
(a) Upon submission of the required permit application in compliance with
Section 38-96 PMZ shall issue a permit to a transportation network services
company.
(b) PMZ shall have the authority to enforce the requirements of this Article IV,

the Business Incubator and Economic Services Department that will provide
economic and planning assistance to business enterprises located in or to be
located in Jefferson Parish; and
WHEREAS, the PARISH has a reasonable expectation of receiving the value of
increased job creation, business retention, business growth, and ultimately an
increased tax base which is at least equivalent to the $300,000 provided for in
this agreement; and
WHEREAS, the transfer or expenditure of public funds or property is not a
gratuitous donation; and
WHEREAS, the citizens of Jefferson Parish will benefit from the efforts of these
parties working together.

that public funds may be used for programs of social welfare for the aid and
support of the citizens of Jefferson Parish; and
WHEREAS, the PARISH, pursuant to Jefferson Parish, Louisiana, Code
of Ordinances Section 1.01(15), has the authority to provide recreational and
cultural programs and related services to all citizens of Jefferson Parish; and
WHEREAS, the public purpose of the Project is to provide cultural enrichment
and entertainment activities for the public, while promoting economic
development and tourism in the City of Gretna and Jefferson Parish; and
WHEREAS, Jefferson Parish has a reasonable expectation of receiving the value
of increased tourism and economic development which is at least equivalent
to the Two Hundred Thousand Dollars ($200,000.00) provided for in this
Agreement; and
WHEREAS, the Parish wishes to enter into an Intergovernmental Agreement
with the City of Gretna to provide Two Hundred Thousand Dollars ($200,000.00)
for the City of Gretna to use towards recreational and cultural programs to
increase enjoyment for the citizens of Jefferson Parish and increase tourism and
provide economic development for the City of Gretna and the Parish.
NOW THEREFORE, BE IT RESOLVED by the Jefferson Parish Council acting
in the governing authority of said Parish:
SECTION 1. That the Intergovernmental Agreement between Jefferson Parish
and the City of Gretna for promoting and staging tourism-related events and/or

suspended or revoked pursuant to Jefferson Parish Code of Ordinances, Section

WHEREAS, Peggy Sues, owned by Peggy Sues LLC, located at 2416 4th
Street, Harvey, holds a 2015 Alcoholic Beverage Permit; and
WHEREAS, on January 24, 2015, the Jefferson Parish Sheriffs Office
investigated an incident involving a Simple Battery that occurred on the
premises of Peggy Sues; and
WHEREAS, on March 8, 2015, the Jefferson Parish Sheriffs Office investigated
an incident involving an Aggravated Battery that occurred on the premises of
Peggy Sues; and
WHEREAS, pursuant to Jefferson Parish Code of Ordinances, Section 4-34,
this is a motion to have a hearing set and heard by the Jefferson Parish Alcoholic
Beverage Permit Review Committee to determine whether the 2015 Alcoholic
Beverage Permit of Peggy Sues, Peggy Sues LLC should be suspended or
revoked for reasons to include, but not limited to, allowance of acts enumerated
in Louisiana Revised Statutes 26:90 and the commission of unlawful acts in

WHEREAS, Thomas C. Laughlin dedicated his life to the aging residents of
Jefferson Parish; and
WHEREAS, Thomas C. Laughlin led the Jefferson Council on Aging for 35
years; and
WHEREAS, Thomas C. Laughlin dedicated his life to providing a better quality
of life for the aging residents of Jefferson Parish; and
WHEREAS, Thomas C. Laughlin passed away in 2010 at the age of 80; and
NOW, THEREFORE, BE IT RESOLVED, by the Jefferson Parish Council of
Jefferson Parish, Louisiana, acting as governing authority of said Parish:

of a Statement of Qualifications (Jefferson Parish Professional Services
Questionnaire) from firms interested and qualified to provide professional
engineering services related to the Design and Construction of Subsurface
Drainage Improvements on Mounes Street between Dickory Avenue and
Elmwood Park Boulevard. (Council District 2)
WHEREAS, the Jefferson Parish Drainage Department has determined the need
for a new drainage structure under Mounes Street between Dickory Avenue and
Elmwood Park Boulevard; and,
WHEREAS, it is in the best interest of the Parish to advertise for the submittal of
a Statement of Qualifications from persons or firms interested in and qualified to
provide professional engineering services related to the design and construction
of the improvements; and,
WHEREAS, the Department of Drainage will be responsible for the preparation
of a Public Notice that is to be submitted to the Parish Clerk with instructions
to commence the advertisement process as required by law.
NOW, THEREFORE, BE IT RESOLVED by the Jefferson Parish Council, the
governing authority of said Parish:
SECTION 1. That the Parish Clerk is hereby authorized to advertise for the
submittal of a Statement of Qualifications from persons or firms interested in
and qualified to provide professional engineering services for the Design and
Construction of Subsurface Drainage Improvements on Mounes Street between
Dickory Avenue and Elmwood Park Boulevard. (Council District 2)
SECTION 2. That the following criteria will be used to evaluate the statement of
the persons or firms submitting:
(1) Professional training and experience in relation to the type of work required
for the engineering services - 35 points
(2) Size of firm considering the number of professional and support personnel
required to perform the type of engineering tasks, including project evaluation,
project design, drafting of technical plans, development of technical
specifications and construction administration - 20 points
(3) Capacity for timely completion of newly assigned work, considering the
factors of type of engineering task, current unfinished workload, and person or
firms available professional and support personnel 20 points
(4) Past Performance by person or firm on projects of or similar comparable size,
scope, and scale. Assertions of fault by a person or firm, which shall include
time delays, cost over-runs, and or design inadequacies in prior work completed
for the Parish shall be evidenced by substantiating documentation provided
by the Director of Public Works for the requesting department or the Director
of Engineering and received by the Chairman of the Evaluation Committee
a minimum of two (2) weeks prior to the scheduled date of the Technical
Evaluation Committee meeting 10 points
(5) Location of the principal office where work will be performed. Preference
shall be given to persons or firms with a principal business office at which the
work will be performed as follows: (1) Jefferson Parish, including municipalities
located within Jefferson Parish (15 points); (2) Neighboring Parishes of
the Greater New Orleans Metropolitan Region, which includes Orleans,
Plaquemines, St. Bernard, St. Charles and St. Tammany Parishes (12 Points); (3)
Parishes other than the foregoing (10 Points); (4) Outside the State of Louisiana
(6 Points) 15 points
(6) Adversarial legal proceedings between the Parish and the person or firm
performing professional services, in which the Parish prevailed, and excluding
those instances or cases where the person or firm was added as an indispensable
party, or where the person or firm participated in or assisted the public entity
in prosecution of its claim. In the event that the person or firm fails to provide
accurate and detailed information regarding legal proceedings with the Parish,
including the absence of legal proceedings, the person or firm shall be deemed
unresponsive with regard to this category, and zero (0) points shall be awarded
15 points
(7) Prior successful completion of projects of the type and nature of the
engineering services, as defined, for which firm has provided verifiable

SECTION 3. That the person or firm submitting a Statement of Qualifications
shall have the following minimum qualifications:
1. The person or firm under consideration shall have at least one principal who
has at least five (5) years experience in the field or fields of expertise required
for the project.
2. The person or firm under consideration shall have at least one principal who
is a registered professional engineer who shall be registered as such in Louisiana
with a minimum of five (5) years experience in the discipline involved.
SECTION 4. That only those persons or firms receiving an overall cumulative
score of at least seventy percent (70%) or greater, of the total possible points
for all categories to be assigned by the participating Technical Evaluation
Committee members shall be deemed qualified to perform engineering tasks.
SECTION 5. That the cost of these services will be negotiated with the Jefferson
Parish Department of Capital Projects.
SECTION 6. That no work associated with this project shall be authorized by
Jefferson Parish until a funding source is established.
SECTION 7. That when a funding source is established for financing necessary
services associated with this project, proper account lines will be established.

years at an amount not to exceed Fifty Thousand Dollars ($50,000.00) per year.

September, 2014, the Jefferson Parish Council authorized the Purchasing
Department to re-advertise a Request for Proposals to provide liaison services
including but not limited to interpretation to the Hispanic community and other
minority groups on an as-needed basis for various Jefferson Parish Departments
under RFP No. 0324; and
WHEREAS, it is in the best interest of Jefferson Parish to obtain competitive
proposals as allowed by Section 2-985 et.seq. of the Jefferson Parish Code of
Ordinances from bona fide, qualified proposers who are interested in providing
liaison services including but not limited to interpretation to the Hispanic
Community and other Minority Groups on an As-Needed Basis for various
departments; and
WHEREAS, the critical nature of the scope of services deserves a more in-depth
analysis of proposers experience, financial stability and capacity, therefore the
RFP process is warranted over and above a public bid; and
WHEREAS, pursuant to Resolution No. 124415, adopted on the 4th day of March,
2015, the Jefferson Parish Council selected All World Language Consultants,
Inc. to provide liaison services including but not limited to interpretation to
the Hispanic community and other minority groups on an as-needed basis for
various Jefferson Parish Departments.

Jefferson Parish Council ratified a contract with Cigna Life Insurance Company
of North America to provide employee paid supplemental Long Term Disability
insurance benefits beginning July 1, 2012, through December 31, 2013; and
WHEREAS, pursuant to Resolution No. 120901, adopted May 1, 2013, the
Jefferson Parish Council ratified Amendment No. 1 with Cigna Life Insurance
Company of North America to provide employee paid supplemental Long Term
Disability insurance benefits beginning January 1, 2014 through December 31,
2015.
WHEREAS, Cigna Life Insurance Company of North America has agreed
to provide Long Term Disability coverage for the same coverage with no rate
increase beginning January 1, 2016 through December 31, 2017
WHEREAS, the Insurance Advisory and Coordinating Committee met on
March 18, 2015, and recommends extending the Long Term Disability contract
with Cigna Life Insurance Company of North America beginning January 1,
2016 through December 31, 2017 with the current plan at the current rates; and
WHEREAS, it is in the best interest of the Parish to extend the contract for two
years with the current plan and no rate increase.
NOW THEREFORE, BE IT RESOLVED by the Jefferson Parish Council of

SECTION 1. That Amendment No. 2 to extend the contract with Cigna Life
Insurance Company of North America (contract #55-12728) to provide employee

113125 ratified a contract with Cigna to provide a Parish paid term life insurance

and an optional dependent life insurance plan for 2010, 2011 and 2012; and
WHEREAS, Resolution No. 118656, adopted on March 28, 2012, ratified
Amendment No. 1 to the contract with wherein Cigna Life Insurance Company
of North America proposed a renewal for 2013 and 2014, that reduced the Parish
paid term life insurance and AD&D rate from $.115 to $.08 per thousand; and
WHEREAS, Cigna Life Insurance Company of North America began said rate
reduction upon execution of Amendment No. 1; and;
WHEREAS, Resolution No. 122670, adopted on April 9, 2014, the Jefferson
Parish Council ratified Amendment No. 2 to the contract with Cigna Life
Insurance Company of North America to provide a Parish paid term life
insurance and accidental death and dismemberment (AD&D) plan for active
employees and retirees, and an optional employee paid term life insurance
and AD&D plan, and an optional dependent life plan, with no rate increase,
beginning January 1, 2015 and ending December 31, 2016; and
WHEREAS, the Insurance Advisory and Coordinating Committee met on

March 18, 2015 to discuss the Cigna Life Insurance Company of North America
contract and it was decided to renew the contract through December 31, 2017 as
Cigna Life Insurance Company of North America agreed to a contract extension

was ratified by Resolution No. 115359, adopted on September 22, 2010, to

WHEREAS, Resolution No. 117816, adopted on October 26, 2011, ratified
Amendment No. 1 to the contract, extending the contract to June 30, 2012; and
WHEREAS, Resolution No. 118660, adopted on March 28, 2012, ratified
Amendment No. 2 to the contract, extending the contract to December 31, 2014;
and
WHEREAS, Resolution No. 122671, adopted on April 9, 2014, ratified
Amendment No. 3 to the contract, extending the contract to December 31, 2016;
and
WHEREAS, Cigna Life Insurance Company of North America has agreed to
provide an employee paid voluntary accidental death and dismemberment plan
for employees of Jefferson Parish for an additional year (2017) with the same
plan at no rate increase; and
WHEREAS, the Insurance Advisory and Coordinating Committee met on
March 18, 2015 to discuss the Cigna Life Insurance Company of North America
contract and it was decided to renew the contract for an additional year (2017);
and
WHEREAS, it is in the best interest of Jefferson Parish and its employees to
extend the contract with Cigna Life Insurance Company of North America, with
current rates and plan to stay the same, to provide a voluntary accidental death
and dismemberment plan for the period of January 1, 2017 to December 31, 2017.

Permitting Process associated with Jefferson Parish under RFP 0310, Contract
No. RP-00000310, to extend the consulting services to the Departments of
Planning and Public Works for an additional cost of $19,750.00 per department
and to increase the term of the agreement for an additional six (6) months.

WHEREAS, pursuant to Resolution No. 122598, adopted March 19, 2014, the
Jefferson Parish Council authorized the Purchasing Department to advertise
for Request for Proposals to provide for Consulting Services to assist in the
Improved Process Management and Delivery of Services for the Permitting
Process associated with the Jefferson Parish Department of Inspection and Code
under RFP No. 0310; and
WHEREAS, pursuant to Resolution No. 122902, adopted June 11, 2014,
the Jefferson Parish Council selected SSA Consultants, LLC, to provide
for Consulting Services to assist in the Improved Process Management and
Delivery of Services for the Permitting Process associated with the Jefferson
Parish Department of Inspection and Code; and
WHEREAS, It has been discovered that to fully improve the Process
Management, and Delivery of Services for the Permitting Process the
improvements and services need to extend into the Departments of Planning
and Public Works; and
WHEREAS, SSA Consultants, LLC, is prepared to extend its consulting
services to the departments of Planning and Public Works for an additional cost
of $19,750.00 for each department; and
WHEREAS, and an extension of Six (6) months time is necessary to complete
this additional work; and
WHEREAS, a sealed bid procurement is not mandated by law, use of the RFP
is in the best interest of the Parish in that it allows the Parish to evaluate vendor
experience, accomplishments and expertise against the technical mandates
required to improve the operation and efficiency of the Plan Review and
Permitting section of the Departments of Planning and Public Works; and
WHEREAS, The Parish of Jefferson and SSA Consultants, LLC wish to amend
the Agreement to contribute additional funding to this endeavor by increasing
the contract cap in the amount of $39,500.00 for a new total cost not to exceed
$74,000.00 and to extend the term of this Agreement.
NOW, THEREFORE, BE IT RESOLVED by the Jefferson Parish Council, the
governing authority of said Parish:
SECTION 1. That the First Amendment which amends the Agreement with SSA
Consultants LLC, dated September 26, 2014, to provide for Consulting Services
to assist in the Improved Process Management and Delivery of Services for the
Permitting Process associated with Jefferson Parish under RFP 0310, Contract
No. RP-00000310, to extend the consulting services to the Departments of
Planning and Public Works for an additional cost of $19,750.00 per department
for a cost not to exceed $74,000.00 and to increase the term of the agreement for
an additional six (6) months is hereby ratified.
SECTION 2. That the costs associated with this agreement in an amount not to
exceed $19,750.00 for the Department of Planning will be charged to Account
No. 10010-0068-7331.
SECTION 3. That the costs associated with this agreement in an amount not to
exceed $19,750.00 for the Department of Public Works shall be charged to the

services related to the Design and Construction of Subsurface Drainage

General
The scope of work associated with this project consists of installing a new
drainage structure under Mounes Street between Dickory Avenue and Elmwood
Park Blvd and associated roadway reconstruction and utilities relocation.
The project is expected to require the following supplemental services:
surveying, geotechnical, preparation of right-of-way plans (if required), and
traffic engineering related services.
The firms submitting a Statement of Qualifications (Jefferson Parish Professional
Services Questionnaire) must identify all sub-consultant firms which they expect
to use to provide professional services, and submit a Statement of Qualifications
(Jefferson Parish Professional Services Questionnaire) for each sub-consultant
firm. (Refer to Jefferson Parish Code Ordinance, Section 2-928).
Compensation
Compensation for the required design services will be based on a lump sum
fixed engineering fee. The fixed fee will be based on the percentage shown on the
1981 Consulting engineering guide for the engagement of engineering services
(ASCE manual No.45) Curve A when applied to an estimated construction
cost (including a 15% contingency) prepared by the consultant and approved by

Minimum Requirements for Selection
1. The persons or firms under consideration shall have at least one principal who
has at least five (5) years experience in the field or fields of expertise required
for the project.
2. The persons or firms under consideration shall have at least one principal who
is a registered professional engineer who shall be registered as such in Louisiana
with a minimum of five (5) years experience in the discipline involved.
Evaluation Criteria
The following criteria shall be used to evaluate each firm submitting a Statement
of Qualifications:
(1) Professional training and experience in relation to the type of work required
for the engineering services. (Maximum points awarded shall be 35).
(2) Size of firm considering the number of professional and support personnel
required to perform the type of engineering tasks, including project evaluation,
project design, drafting of technical plans, development of technical
specifications and construction administration. (Maximum points awarded shall
be 20).
(3) Capacity for timely completion of newly assigned work, considering the
factors of type of engineering task, current unfinished workload, and person or
firms available professional and support personnel. (Maximum points awarded
shall be 20).
(4) Past Performance by person or firm on projects of or similar comparable size,
scope, and scale. Assertions of fault by a person or firm, which shall include
time delays, cost over-runs, and or design inadequacies in prior work completed
for the Parish shall be evidenced by substantiating documentation provided
by the Director of Public Works for the requesting department or the Director
of Engineering and received by the Chairman of the Evaluation Committee
a minimum of two (2) weeks prior to the scheduled date of the Technical
Evaluation Committee meeting. (Maximum points awarded shall be 10).
(5) Location of the principal office where work will be performed. Preference
shall be given to persons or firms with a principal business office at which the
work will be performed as follows: (1) Jefferson Parish, including municipalities
located within Jefferson Parish (15 points); (2) Neighboring Parishes of
the Greater New Orleans Metropolitan Region, which includes Orleans,
Plaquemines, St. Bernard, St. Charles and St. Tammany Parishes (12 Points); (3)
Parishes other than the foregoing (10 Points); (4) Outside the State of Louisiana
(6 Points). (Maximum points awarded shall be 15).
(6) Adversarial legal proceedings between the Parish and the person or firm
performing professional services, in which the Parish prevailed, and excluding
those instances or cases where the person or firm was added as an indispensable
party, or where the person or firm participated in or assisted the public entity
in prosecution of its claim. In the event that the person or firm fails to provide
accurate and detailed information regarding legal proceedings with the Parish,
including the absence of legal proceedings, the person or firm shall be deemed
unresponsive with regard to this category, and zero (0) points shall be awarded.
(Maximum points awarded shall be 15 for the lack of any such adversarial
proceedings as defined).
(7) Prior successful completion of projects of the type and nature of the
engineering services, as defined, for which firm has provided verifiable
references. (Maximum points awarded shall be 15).
Only those persons or firms receiving an overall cumulative score of at least
seventy percent (70%) or greater, of the total possible points for all categories to
be assigned by the participating Technical Evaluation Committee members shall
be deemed qualified to perform engineering tasks.
