relevant Government Authorities to carry on the business of car rental in Malaysia.
We apply for admission as ORDINARY MEMBER of the Association.
We state that we are a Malaysian Registered Company, which is licenced by the
relevant Government Authorities to carry on the business of limousine services in
Malaysia. We apply for admission as ASSOCIATE MEMBER of the Association.
We hereby undertake to abide by the Constitution, Code of Ethics, By-laws and Rules and
Regulations of Persatuan Sewaan Kereta Malaysia (Car Rental Association of Malaysia) [CRAM] as
shall be in force from time to time and we will accept as final and binding the decisions of the
Executive Committee on all matters dealt with them in accordance with the provisions of the
Constitution, By-laws and Regulations. We further undertake that we will promote the objectives
of the Association as far as may be in my power.
We hereby certify and confirm that our appointed Representative or Alternate Representative is
authorized to exercise all our privileges of membership and to attend, to act and to vote on our
Companys behalf at all meetings and to sign all circulars and resolutions of the Association. Until
revoked by us in writing all actions or representations or omissions of the Representative or
Alternate Representative shall be deemed to be actions, representations and omissions of our
Company.

We enclose the following documents of our Company duly Certified True Copy by the Company
Secretary and relevant parties:(i)

Copy/copies of Ministry of Tourism Licence.

A copy of Companys Board Resolution resolving to join the Association and to nominate our
Representative to represent our Company at the Association Meetings.
(iii) A copy of the Certificate of Incorporation and Certificate of Change of Company name.
(iv) A copy of the Companys Memorandum and Articles of Association.
(v) A copy of the Latest Annual Return of the Company.
(vi) A copy of the Latest Form 49 } please furnish if there are any change in directors & share
(vii) A copy of the Latest Form 24 } capital information after the date of the latest Annual Return
