QUESTIONNAIRE

FOR MATH JUNIORS - 2015



Please, fill in the questionnaire below and email it to your assigned faculty advisor
at least 24 hours prior to your scheduled advising session. Also bring a hard copy
of the questionnaire with you to your session. The questionnaire has five
sections:

Please list your questions for the advisor in Section 0.

Section 1 is aimed at demonstrating that you have familiarized yourself with
the material posted on the mathematics department website; this is essential
to avoid wasting time on obvious details during the advising session.

Section 2 is aimed at familiarizing the advisor with your specific passions and
interests for mathematics, so that he/she can best recommend a suitable
course of studies. There is no right or wrong answer: answer as honestly as
possible to maximize the usefulness of your advising session.

Section 3 informs the advisor of your academic history, your study habits
etc., so that she/he can best tailor their advice to your specific needs. There is
no right or wrong answer to questions in this section: answer as honestly as

Section 4 includes your tentative plan of studies. Please refer to the course
offerings and check the schedule for overlapping classes.
