1. INTRODUCTION
1-1 Features of product
(1) You can measure the refracting power, corneal shape, intraocular pressure and corneal thickness of the
patients eyes by easy operation.
(2) When measuring the intraocular pressure, air blows more gently than before.
(3) You can perform measurement quickly in the optimal condition because of the auto-alignment function
and auto-start function.

2-5 Principles
2-5-1 Kerato-refractometer unit
1. Target image projection to eyeground (ref function)
The fundamental principle is the same as for previous models: the measurement target is projected from the
iris image of the pupil section and the light reflected from the eyeground is detected.
The following figure is a general diagram of this method.

Ring iris

Eyeground

Eye being measured

Measurement
target

The eyeground is observed from the center by

shooting from the pupil section ring iris image
into the eyeground.

As the figure shows, the ring pattern is used as the measurement target, the image is projected onto the eyeground from the pupil ring iris and the image reflected from the eyeground is detected with the CCD CAMERA. The detected image is image processed and the way in which the circle is converted shows the
refraction of the eye.
2. Measuring principle (kerato function)

Kerato ring image projected on the cornea.

Size: 3mm (in the case of R7.7)
Kerato ring

As shown in the figure, the kerato ring is projected on the cornea and the reflected image is detected with a
CCD CAMERA. The reflected image detected here undergoes image processing to compute the radius of
curvature and the refracting power of the cornea.

4. Image processing
(1) Objective
To find the centers of the inner diameter and outer diameter of the ring image reflected back from the eyeground.
This circle or ellipse
is measured.

(2) Method
1 As shown in the figure below, the image is divided up like a checker board. The right and left lines are at
regular intervals and parallel to the Y axis and the up and down lines are at regular intervals and parallel to
the X axis.
The axes are set up so that the intersection of the X-axis and Y-axis is the center of the ring.
2 Suppose that the difference of the coordinates for the inner diameter and the outer diameter of the ring is
only 7 units, as in the figure below. In this case, if the smallest unit that can be resolved is 1 unit, the center
between the outer diameter and the inner diameter is either Circle A or B. In other words, if the difference
between the inner diameter and the outer diameter is an even multiple of the minimum resolution, then the
center can be measured accurately, but if the difference is an odd multiple, then the center cannot be measured accurately.
3 If the reference units are made smaller than the measurement precision, an error of one reference unit
could be ignored.
Y axis

X axis

A
B

16

R-TRK1P-0-0809-02

The CCD CAMERA is a collection of extremely small such checker board squares. Normally, a CCD CAMERA comprises hundreds of thousands of squares.
In summary:
1 The reflected image detected with the CCD CAMERA is stored into memory with one data bit for each
checker board square.
2 The coordinates are calculated from the stored data and the center of the inner diameter and the outer
diameter of the ring image is found.
3 The pattern for the figure found in 2 is identified to obtain the change in the image.

17

R-TRK1P-0-0809-02

5. Rotary prism method

There is fluctuation between the measured values because the eyeground condition is not always even due to a
disease, blood vessles, etc. Place a prism in front of the ring iris and rotate it during measurement. In a wider
area than the usual eyeground, the eyeground information is averaged to reduce the above fluctuation.

6. Principles of auto-alignment
For vertical or horizontal directions (X and Y axes), check the alignment by using the positional relation
between the alignment spot and alignment mark of the reflected image as the conventional RM/KR-8800
series. For the focus direction (Z axis), check the alignment by using the relation between the parallel projected spot and placido ring.

2-5-2 Non-contact meter unit

The principle of this instrument is to blow an instantaneous jet of compressed air on the cornea, then to calculate the intraocular pressure from the numerical value of the cylinder pressure required to make the cornea to
a certain plane surface area.
Measurement method is as follows.
Measurements are made by blowing an air jet on the cornea of the patient's eye, determining the amount of the
change in shape from the change in the amount of the light reflected and detecting the condition at the instant
of applanation.
The jet of air blown on the patient's eye causes the cornea to change its shape in the sequence in the following
figure.
Cornea

Convex

Plane

Concave

Plane

Convex

The change in shape is measured from the difference between the received light from the infrared LED and
the reflected light detected by the intensitometer.
The reflected light to the applanation sensor is weak when the cornea is spherical (in the following figure at
the left). When the cornea is made to a certain plane surface area (in the following figure at the right), the
maximum reflected light is detected.

R-TRK1P-0-0809-02

Amount of reflection light

The pressure of the air jet increases together with the chamber inner pressure (CP), which is continuously
measured by the pressure sensor. The intensity of the light reflected on the cornea (L) is detected by the intensitometer as the function of the chamber inner pressure (CP). (Refer to the following figure.) Using the function in the figure, L=f (CP), the chamber inner pressure (Pc) is determined for the maximum reflected light
intensity (Lmax). The intraocular pressure (IOP) is determined according to the obtained chamber inner pressure (Pc).

Chamber inner pressure

Corneal thickness measurement principles

Slit light is slantingly emitted for the patient's cornea and the light reflected by the front and backside of the
cornea is brought in by the line sensor.
The corneal thickness is calculated by using the interval (L) between the front reflection image and backside
reflection image on the line sensor.

(1) Fix the test eyes for tools on the chinrest and measure each of them in order, confirming that the values are
within the limits listed below.
(However, when the measured values exceed the accuracy limits, re-correct the setting.)
(2) Also check the diopter value of the accessory test eyes at this time.
Method of appraisal:
a. The typical values should be obtained after measuring the power and axial angle five times.
b. The fluctuation is the fluctuation width after measuring five times.

b. The fluctuation is the fluctuation width after measuring five times.
(However, there should be a separate judgment for H and V.)

3-1-4 Checking accuracy of tonometer-use measurement

(1) Fix the test eyes for tonometer tool on the chinrest and measure each of them in order, confirming that the
values are within the limits listed below.
(However, when the measured values exceed the accuracy limits, re-correct the setting.)
(2) Checking the intraocular pressure measurement
Method of appraisal:
Align the center of the test eye, measure it ten times and make sure that the measured values are within the
following standard values.
|Reference value - Average of the ten measured values| 2.5
Less than the reference value 30 : 0 - 30mmHg range
Reference value 30 or more
: 0 - 60mmHg range
(3) Checking the accuracy of the corneal thickness
Method of appraisal:
Measure each of the test eyes having five types of thickness five times and make sure that the measured
values are within the following standard values.
Within the reference value 0.01mm (The test eye thickness is less than 0.6mm.)
Within the reference value 0.025mm (The test eye thickness is 0.6mm or more.)
