The undersigned Petitioner before the honorable Integrated Bar of the Philippines
respectfully states:

the requisite membership dues, professional tax and penalties, all done merely for the
sake of compliance.

described and defined above. Petitioner has not accepted any cases for litigation or
engagements requiring the giving of legal advice or execution of legal instruments.

that effect with the Secretary of the Integrated Bar, who shall immediately
bring the matter to the attention of the Supreme Court. Forthwith, he shall
cease to be a member and his name shall be stricken from the Roll of

PRAYER
WHEREFORE, in view of the foregoing, it is respectfully prayed that a Decision
or Resolution be rendered:
1) Granting this Petition;
2) Endorsing this Petition/Notice favorably to the Supreme Court for:
a) The cessation of the Petitioners membership in the Integrated Bar of the
Philippines;
b) Removal of the Petitioners name from the Roll of Attorneys.
Other reliefs just and equitable under the law and rules are likewise prayed for.
