Center, LDA Colony, Sector-G Kanpur Road, Lucknow-226012
(i) Two passport size photos (one should be pasted on Application form)
(ii) Photocopy of Identity Card issued by college/Institute/University.
(iv) Fees may be paid in Cash OR through DD favor of AO (Cash) BSNL RTTC
