BRING ALONG A LIST OF ALL THE MEDICATIONS THAT YOU ARE TAKING. Include medications that have been prescribed as well as
those you buy over the counter,
such as:

Vitamines;
Natural products;
Dietary supplements;
Medication sold without a
prescription at the pharmacy
We suggest that you always keep this list up-todate. Your doctor, your pharmacist or your nurse
can help you to prepare this list.

WASH YOUR HANDS REGULARLY


Hand-washing remains the best way to avoid
communicable infections. It is your right to see
that your visitors, your doctor and your
healthcare team wash their hands as well.

IDENTIFY YOURSELF PROPERLY


The personnel must frequently ask for your family name, first name and date of birth before performing any care or treatment
action. Keep a piece of identification with this information handy.
This will help avoid cases of mistaken identity.

At the CISSS des Laurentides Des Sommets Service Point, the personnel, doctors,
Users Committee, volunteers and trainees
all have a role to play in ensuring your safety.
When you, or a member of your family, receive care or services in one of our various
programs, you also have a role to play in
ensuring that everyone is safe. Here are a
few examples:

I examine all documents given to


me;

I obey the instructions and recom-

mendations given to me by the personnel, as well as the institutions


safety instructions;

If I feel that I am at risk of falling, I


ask for help in getting around;

If I am the victim of or witness to an


accident, or a situation that may
cause an accident, I let the personnel know;

If my situation deteriorates or if I
have an accident, I tell my intervener.
A team exists to analyse risky situations and
to work to put safety measures in place.

of your healthcare team along with information
about your current state of health.

Dont hesitate to ask


questions or to share
your concerns with
the personnel treating you. You must
understand the following as well as
possible:

In addition to the reasons for the consultation, tell them:

If you have consulted another doctor


or if you have received healthcare
services in another institution;

If you suffer from a chronic illness,


such as diabetes or heart disease;

If anyone in your family has specific


diseases, such as high blood pressure, asthma or cancer;

If you use, or if you have a history of


using, tobacco, drugs or alcohol;

If you have ever had an allergic or

other negative reaction to a medication or a food;

If you have recently made any changes in your medications.

Your medical problem (the diagnosis);

The nature of your treatment or of an


intervention by you or by us;

Your medications and the way to take


them;

Changes made in your medications


and your understanding of the reasons;

The instructions to follow when you


return to your home, if applicable.

To make sure that you understand properly,


repeat the information as you understand it to
your doctor or healthcare professional, in your
own words. If it isnt clear, ask more questions
to understand better. If you are not in a position to do it properly yourself, ask a friend or
family member to accompany you and do it for
you.
