AFFIDAVIT OF TWO DISINTERESTED PERSON
We, FLORENTINA LIWANAN and TONI BURAS, both of legal age,
Filipinos citizens and residents of Camp 4, Tuba, Benguet, Philippines, after
having been duly sworn to in accordance with law hereby depose and state:
1 That we know personally know AGO COTSE BUMANGHAT, such
facts of birth were duly registered, and a member of the Social
Security System (SSS);
2 That, however, in his Marriage Contract his name appears as
SANTIAGO
BUMANGHAT
and
in
the
Entry
Father:
BUMANGHAT MABASLING appears as the name of his father
and in the Entry Mother: JOSEFA LIWANAN appears as the name
of his mother;
3 That the real name of the aforementioned person is AGO COTSE
BUMANGHAT, as appearing his Certificate of Live Birth, and the
real name of his parents are BUMANGHAT MACASLING and
JOSEFA COTSE, respectively, which names is known to us, to his
relatives, friends and in the community and as evidenced by his
NSO Certificate of Live Birth and Certifications issued by Asia
Pacific Theological Seminary and Keys of Kingdom Ministries Life
In the Spirit Fellowship, Inc;
4 The aforementioned errors/discrepancies regarding his name in the
Marriage Contract was inadvertently written and not due to any
reason such as to evade in liability or whatsoever, hence, the name
SANTIAGO BUMANGHAT, as per Marriage Contract and AGO C.
BUMANGHAT, as per NSO Birth Certificate, pertains to one and
the same person;
5 That furthermore, the name BUMANGHAT MABASLING and
JOSEFA LIWANAN, as per Marriage Contract of Ago and
BUMANGHAT MACASLING and JOSEFA COTSE, as per NSO Birth
Certificate of Ago, pertains to one and the same persons, which are
the parents of Ago Cotse Bumanghat;
6 That we know the above facts and circumstances because we are
long time family
friends and neighbors of the above
named person and her family;
7 That we are not in any way related to the above-mentioned person
neither by consanguinity nor affinity;
8 That we are executing this Affidavit to attest to the truth of all the
foregoing statement and to state the aforementioned facts and for
all legal intents and purposes that it may serve.
