NOTICE OF FAILURE TO COMPLY WITH APPEAL FILING FEE REQUIREMENT

This is to advise that you have failed to submit the $455.00 appeal filing fee.

For 42 U.S.C. § 1983 prisoner civil rights cases:

1. Be advised that you have failed to comply with the Prison Litigation Reform Act of
1995. The Act requires that prisoners must pay the full appeal filing fee of $455.00 for an
appeal in federal court.
2. If you are unable to pay the full filing fee, you must complete and return an application
to proceed without prepaying fees and costs, a copy of your trust fund account statement
for the past 6 months and a signed authorization to withdraw funds from your trust fund
account.
3. You have 30 days from the date of mailing of this notice to file the required
documents. Otherwise, your case may be dismissed by the U.S. Court of Appeals.
