I, CARMELITA B. JUANZON, of legal age, Filipino, married and

presently residing at 3434 West Vigan Street, Sta. Mesa, Manila, after
having been duly sworn to in accordance with law, depose and state:
1.

That I am the biological mother of Bryan Austine B. Juanzon

who was born on August 25, 1998 in Pasig City;

2.

That said child will travel to Singapore for a tour/vacation;

3.

That said child will be leaving on December, 2013 and will

stay in the said country for a period of at least tem (10) days
and will be taken cared of by my mother-in-law, Nelia S.
Juanzon and sister-in-law, Jenny S. Juanzon;

4.

That I cannot personally travel with my child due to my work;

5.

That I am giving my full consent for the minor child to travel

abroad to be accompanied by my mother-in-law, Nelia S.
Juanzon;

6.

That I personally guarantee the financial support of the child

in the course of his travel and stay outside the Philippines
and further guarantee that he will not in any manner become
a burden to the Philippine and Singapore governments;

7.

That I am executing this Affidavit in connection with my

childs application for a passport at the Department of Foreign
Affairs and/or travel clearance from the Department of Social
Welfare and Development for his travel abroad.

This is in compliance with the Order of this Honorable Court dated
February 9, 2014 in connection with the above-entitled case wherein the
undersigned is directed to submit a copy of the Transcript of Stenographic
Notes dated July 10, 2007 within five (5) days from receipt thereof.
With due respect to the Honorable Court, when the undersigned left
Branch 87 in 2007 and was recalled at the Office of the Clerk of Court,
she was not allowed to bring with her any court documents, folders, even
file copies of her Transcript of Stenographic Notes because of the ongoing
inventory of cases at that time. The undersigned turned over all the
original copies and file copies of her TSNs to the then Civil In-Charge, Ms.
Myra Millora. But when the undersigned spoke to Ms. Millora regarding
the subject TSN, the latter told the undersigned that she could not locate
the same and will just ask the counsels of both parties for a duplicate
copy. Unfortunately, the undersigned does not have any means to produce
a copy of the subject TSN considering that it has been a long time since
she left Branch 87 and has been transferred from one court to another.

The undersigned does not wish to cause delay in the proper disposal of the
instant case.

Dear Sir/Madam:
The undersigned requested for the revocation of her flexi-time schedule
which was subsequently approved by the Honorable Deputy Court Administrator
Raul Bautista Villanueva in his Letter dated June 16, 2014. The Letter stated
among others that the official time of the undersigned shall revert back to the
regular working hours of 8:00 A.M. to 12:00 noon and 12:30 P.M. to 4:30 P.M.
effective 1 July 2014.
However, the undersigned received the said letter only on July 24, 2014,
hence she was not able to adjust her schedule according to the regular working
hours as requested as of the date of its effectivity.
