REGISTRATION TERMS AND CONDITIONS

By registering for the [Rise and Shine Parenting Summit] (the Event) through this registration
form, you agree to the following terms and conditions:
Participant means (a) any person who completes this registration form, and (b) any person
who has been signed up by the person who completes the registration form.
1. Giraffe Consulting Asia Pte. Ltd. (the Organiser) may, at its sole discretion, modify the
Events terms and conditions or cancel this Event, without prior notice to any person.
The Organisers decisions on all matters relating to this Event will be final, binding and
conclusive on all Participants, and no correspondence will be entertained.
2. All views expressed by the speakers are their own. The Organiser and Wyeth Nutrition
(Singapore) Pte. Limited (Wyeth Nutrition) are not responsible or liable for advice given,
or views expressed, by any speaker at the Event or any material provided to Participants.
3. Participation in the Event is at the sole risk of the Participant. The Organiser and Wyeth
Nutrition will not be responsible or liable for any incident or accident that happens to the
Participant during the Event or while traveling to/from the Event. The Organiser and
Wyeth Nutrition will also not be responsible or liable for damage to or loss of the
Participants belongings during the Event. To the fullest extent permitted by law, each
Participant waives, releases and discharges the Organiser, Wyeth Nutrition and
commercial partners (and their respective agents, employees and officers) from any and
all liability, including but not limited to, death, disability, personal injury, property damage,
property theft, or actions of any kind which may be suffered or incurred by the Participant.
4. In registering for the Event, Participants grant permission to Wyeth Nutrition and the
Organiser, its agents and commercial partners, to take and to have full and free use of
video/photographs containing their image/likeness, in any media or form (Materials).
These Materials may be used for marketing, publicity and promotional purposes by and
for Wyeth Nutrition in connection with the Event. Participants are not entitled to
remuneration, residuals, royalties or any other payment from Wyeth Nutrition in respect
of the creation and use of their image/likeness and/or the Materials. Participants release,
discharge, and hold harmless Wyeth Nutrition and its agents, employees and officers
from any and all claims, demands or causes of actions that they may hereafter have in
connection with the Materials.
5. The Organiser may expel a Participant from further participation in the Event should the
Organiser, at its sole discretion, deem the Participants behavior inappropriate or
offensive to other Participants, or to the Organisers or Wyeth Nutritions employees. No
compensation will be given to a Participant who has been expelled from the Event.
6. Wyeth Nutrition and its respective affiliates, service providers and commercial partners
may use all contact information and personal data provided by each Participant to
conduct verification, administration, and other actions in connection with the Event. In
addition, the above-mentioned parties may also use the personal data for any
promotional, advertising (included targeted advertising) or marketing activities which
Wyeth Nutrition believes may be of interest to the Participant. This includes sending out
marketing, advertising (including targeted advertising), communication and promotional

materials to the Participants relating to any products manufactured, marketed or sold by

Wyeth Nutrition, and their respective affiliates.
7. Any information, personal data and material (including videos and photographs) about or
obtained from the Participant may be accessed, stored or otherwise processed in any
medium or format determined by Wyeth Nutrition, and may be transmitted across
national borders for storage and/or processing.
8. If a Participant requires access to his/her personal data (including videos and
photographs) or requires any amendment or correction to be made, or wishes to
withdraw consent to any specific use of his/her personal data (including videos and
photographs), he/she should email the Wyeth Nutrition Customer Service Officer at
privacy@SG.nestle.com. To find out more about how Wyeth Nutrition uses a
Participant's personal data (including videos and photographs), the Participant can refer

9. The Event and these terms and conditions are governed by and construed in
accordance with Singapore law, and each Participant shall submit to exclusive
jurisdiction of the Singapore courts.
10. Any trademarks, graphic symbols, logos or intellectual property contained in any
materials used in connection with this Event are the property of their respective owners.
This Event, the Organiser, Wyeth Nutrition, and its affiliates and contractors, are not
affiliated with, or endorsed or sponsored by, those owners and the owners relevant
affiliates where those owners or the owners affiliates are not part of the Wyeth Nutrition
group of companies.
