Purpose
The purpose of this document is to set out guidelines for appraising the
performance of the GUC Interns
General Information
The evaluation form shall provide a supportable basis for making decisions such
as continuation of the internship, Training needs and/or future employments.
The Supervisor should provide advice support and guidance to enable the intern
to achieve objectives.
Procedure
The Intern will complete Part I and Part II and forward it to the supervisor.
The supervisor/mentor is responsible for filling in Part III.
An evaluation discussion shall take place between the Supervisor and the Intern
After the discussion both the Supervisor and the Intern will date and sign.
