WELCOME

The Practical Nursing Program at Nightingale International is delighted that


you have joined us. We are in the process of making our school the most
outstanding practical nursing school in the Philippines. We believe that
providing effective quality education, commitment , determination and care
we can achieve our goal of graduating the best nurses possible.

In this handbook you will receive an introduction to our


philosophy and conceptual framework. These documents provide
the foundation of our program . The policies and procedures we
have developed help guide us in creating the integrity and
excellence in our chosen career.

The nursing administration is here to guide and assist you. We


maintain an open door policy and we are always available to
speak with you. Please feel free to call or come by our office at
any time.

Philosophy and Goals


Nightingale International – Vocational Nursing Program is committed to providing

students with high quality education and training in the area of vocational nursing as it

relates to the health care field. Our goal is to offer a highly effective degree of training

and competency needed in the successful completion of the course work and successful

entrance to the chosen field of nursing. Furthermore, the faculty of Nightingale

International – Practical Nursing program believes that:

 Education is vital in maintaining a state of constancy in Man, that would allow self-care needs
to be met

 Cultural diversity in patients and the students is recognized and valued for its unique
contribution to the education of students and that personal value system of each individual
should be respected.

 We believe that education based on the promotion and maintenance of health is vital to the
members of our community

 Individuals and society as a whole are generally concerned with health and wellness and in
providing preventive, therapeutic and restorative care for individuals and families

 Nursing is an integral part in the prevention of self-care limitations. It is vital in regaining back
the individual’s ability for self care through client teaching, therapeutic interventions and
collaboration.

 We believe that vocational nursing contributes to the education of individual patients and their
families and /or caregivers and the community in which the nurse works and lives.

 We believe that high standards of performance are a requirement for both students and
faculty and support measures that will reward competence and performance.

Nightingale International is dedicated to this philosophy. Our graduates will be


prepared to function within the scope of the healthcare profession and in accordance
with existing legal parameters, standards of care, ethical and moral principles of
Vocational Nursing.

Conceptual Framework model

The conceptual framework of Nightingale International is based on Virginia


Henderson’s views that nursing is an art and a discipline separate from medicine.
Henderson defined nursing as “assisting individuals to gain independence in
relation to the performance of activities contributing to the health or its recovery
that he would perform unaided if he had the necessary strength, will or knowledge”.
This program focuses on the nurse’s roles as substitutive (doing for the patient),
supplementary (helping the patient), or complementary (working with the patient)
with the goal of helping the patient become as independent as possible.

The condition that validates the existence of a supplementary nursing for an


individual is the absence of the ability to continuously maintain the amount and
quality of being independent that is therapeutic in sustaining life and health and in
recovering from disease and/or injury.

It is through this conceptual framework that the student nurse will attain the
necessary values, knowledge and skills necessary to determine the individual
client’s potential for substitutive and/or complementary to care for dependents.
The students nurse will learn to provide appropriate interventions to improve the
client’s capabilities when possible, or if not possible supplement the needed
assistance.

Career Training Objective/Terminal Objectives

The objective of the Practical Nursing Program is to train students to acquire satisfactory
skills as a Practical Nurse and prepare student for Vocational Nurse State Licensure
Examination. Upon successful completion of 1750 hours of required training, the graduate
receives diploma, is qualified to assume an entry-level position as a Certified Nurse
Assistant and is eligible to take the Vocational Nurse state Licensure Examination. Upon
passing the licensure exam, the graduate can enter employment as Licensed Practical Nurse

( LPN ) in medical office clinic home health, HMO , sub-acute/rehab and acute care hospital
, long-term care facility or other health care setting.

The training program is divided into four terms approximately 13 weeks in length for full-
time program and 18-20 weeks for part-time program per each term. Each term is related
to each prescribed subjects or studies that build upon each other from basic knowledge to
nursing applications. Students begin their training in term one and progress sequentially
through each term until all courses and terms have been successfully completed.

Upon completion of this program, the student will be able to:

1. Understand and utilize the components of the nursing process.


2. Demonstrate knowledge/ competence and safety in the performance of nursing skills.
3. Initiate and maintain effective communication with patients, visitors, and members of
the health care team.
4. Describe the anatomical structure and physiological functions of the body.
5. Discuss the normal growth and development in terms of physical, psychosocial
development, and needs of the neonate , child adolescent, adult and elderly
6. Recognize the manifestations of disease , the importance of education and nutrition.
7. Prepare a plan of care which include the basic physical , emotional , spiritual,
socioeconomic and psychological factors involved in the nursing care of the patient
and his family.
8. Demonstrate the safe administration of medication.
9. Be familiar and utilize community resources available to the patient and his family.
10. To apply critical thinking for the safety of the patient and environment using nursing
process.
11. Recognize and utilize opportunities to teach health and wellness within the scope of
practice.
12. All graduates should be prepared to take the National Council Licensure Examination (
NCLEX )

Methodology

The program will be conducted in a combination of lectures, readings, demonstrations,


research and reporting, quizzes , examinations, and clinical practice.
Theory and skills laboratory sessions will be held in the campus. Clinical experiences will be
obtained from health care facilities approved by Technical Education and Skills Development
Authority

Admissions Policy

10 Admission Policy

1.1 Employability and licensure can be affected by past experience and personal
performance records. If you have problems in your past that could be judged
as negative occurrence, need to bring it to the attention of the program.
Director and the agency that licenses, certifies, or approves you for entrance
into the profession. No employee of the school is authorized or approved to
render anything other than a personal opinion on this subject.
1.2 Minimum requirements for entrance are open to persons 17 years old turning 18 or older having
the following:

A. High school diploma or general equivalency diploma (GED).


B. Completed the Registration form
C. Two ( 2 ) passport size pictures
D. A passing score on the admission exam (s)-Vocational Nursing
Admission Test ( VNAT )- for high school graduates only
E. Physical examination or health clearance
F. Police clearance
G. College graduates & undergraduates w/ GPA of 80% are exempt from
V-NAT
H. Must have a good moral character

2.0 Admission Selection

2.1 The application and enrollment procedure begins with the completion
of a general
questionnaire and initial interview with a college admission representative. A
discussion
regarding background aptitude, career goals, motivation and placement
potential will be
conducted as well as a guided school tour.

2.2 Prior to enrollment an applicant has the opportunity to ask questions and view
the classrooms and lab. Applicants are required to pass a general aptitude
exam. Successful
Candidates will then have an interview with the director of Nursing to discuss
the nursing
Program and student expectation/s.

2.3 After compiling entrance scores, pre-vocational assessment and Head of


Education
Recommendations, students will be notified of start date.

2.4 All accepted applicants must attend a scheduled Orientation prior to entry.

3.0Non-Academic Requirements

3.1 All students attending clinical programs must be in good health.

3.2 Students must be free from transmittable diseases, including varicella,


measles, mumps, rubella, hepatitis, tuberculosis and sexually transmitted
diseases.

3.3 As a student and a potential professional in the health care field you are at a
higher risk of
Contracting Hepatitis B than the general population. We recommend you
avail yourself of information on the risks to help in your decision to be
vaccinated.

3.4 The school uses several authorized clinical services to run the appropriate
tests to render
an opinion of overall health. It is required that a copy of the completed tests
be in the student’s file no later than the 10 th day following the first day of
school. Student should be prepared to discuss any relevant medical history
with the service. Students are permitted to have their Health Screen run and
reported by their personal physician as long as they comply with the school’s
10-day schedule. Generally, Health Screens are done on the first day of
school. The schedule for your class will be disclosed at your orientation.

NOTE: It is important for the student to disclose to the


clinical service if she is pregnant. If

pregnancy occurs after starting school, it must be disclosed in


writing to the program
director and a full medical release from the student’s physician must
be submitted
prior to attending the next clinical.

10 Credit Granting Policy

1.1 Advanced standing may be granted to students who have successfully


completed
a course or courses and provided documentation to demonstrate
compliance with
Nightingale International (NI) policy and or by TESDA.

1.2 Advanced standing may be granted for the following experience:

A. Completed courses in an accredited vocation or registered nursing


school.

B. Completed courses in an accredited Psychiatric Technician school (


US student )

C. Completed courses in the Armed Services ( US student )

D. Completed C.N.A. certification.

1.1 Courses for advance standing must be completed within the last five
(5) years.

1.4 Nursing courses will be evaluated as to content and hours appropriate


to the
school’s curriculum.

1.5 Students who wish advanced standing must submit all documentation
including
official transcripts to the Head of Education prior to Admission.

1.6 Students will be required to take an advanced placement exam and


score a
Minimum of 75%.

2.0 Credit Granting for Specific Areas

2.1 Advanced standing may be granted for the following experience.

2.2 Completed courses in an accredited vocational or registered nursing


school.

A. Prior to admission students must submit all transcripts including


hours and

B. Upon review of documentation, the director of Nursing or Nursing


Department Designee will administer both an exam and practicum
to validate understanding of content of challenged portions of the
curriculum.

C. On successful completion of the validation, students will receive


credit
for the portion of the program.

2.1 Completed courses in an accredited Psychiatric Technician School (US


student)

A. Prior to admission student must submit all transcripts including


hours and course description showing completion of the
fundamentals of the Psychiatric Technician Program.

B. Upon review of documentation, the Director of Nursing or Nursing


Department Designee will administer both an exam and practicum
to validate the content of the (Term I) portion of the program.

C. Upon successful completion of the validation, students will receive


credit for term I and be placed in term II.

2.1 Completed courses in the Armed Services ( US student )


A. Prior to admission the student will submit all certificates,
documentation of
hours served and the letter from the commanding Officer, attesting
to rank
and status.

B. Upon review of documentation, the Head of Education or Nursing


Department Designee will administer an exam and practicum to
validate the clinical content in term I.

2.5 Completed Certified Nursing Assistant (CNA) Certification.

A. Prior to admission the student will submit proof of current


certificate issued

by the Department of Health Services. ( US only )

B. Upon review of documentation, the Head of Education or Nursing


Department Designee will administer an exam and practicum to
validate the
Clinical content in term I

B. Student will then receive credit for clinical portion only of term I
and be granted those days off.

2.6 Credit Granting for Challenge

2.6.1Applicants with relevant knowledge and or skills acquired in


a non-traditional setting including non-certified nursing
assistants.

A. Application must be completed 30 days prior to admission


date.

B. Applicants will successfully pass a written and


performance examination.

C. Applicant must pass a competency based objective


examination for course being challenged .

2.6.1Credit will be granted if the applicants meets all minimum


requirements equivalent to those required of a student
completing the course.

Requirements for Graduation

10 Requirements

1.1 Must complete all 52 weeks or 80 weeks of the program or its equivalency.

1.2 Must achieve a C or 75% in each term.

1.3 Must have a Satisfactory in each clinical segment.

1.4 Must have completed minimum theory and clinical hours as approved by the
Technical Education and Skills Development Authority ( TESDA ).

1.5 Must not be on probationary status for absenteeism or behavioral deficiencies.

1.6 Must have completed the program within 1 ½ times the planned program
length.

1.7 Must be in good financial standing with the school.

1.8 Must satisfactorily complete NCLEX-PN practice exams for each system.

20 Licensure for L.P.N./L.V.N.

2.1 Must complete application to BVNPT and pay appropriate fees.

2.2 Must complete application for National Council Licensure Exam and pay
appropriate fees.

2.3 Must schedule a Computer Adapted Testing Exam time with the appropriate
agency.

General Conduct

10 Policy

1.1 A professional environment is maintained by the school. Students are


required
to be courteous and considerate towards instructors, staff and fellow
students.

1.2 School policies must be observed regarding curriculum, testing,


absences, tardiness and in the clinical area.

1.3 Students are expected to maintain professional conduct at all times in


the school
and clinical lab. Use of foul language, threats against another person,
fighting or
sexual harassment will result in immediate termination from the
school.

1.4 Failure to abide by these general rules of conduct may result in


probation or

Termination from the program.

Student Grievance Procedures

10 If a situation arises in which a student has a complaint or grievance


regarding
grades, instruction, or other topics related to the curriculum, the
following
procedure is in effect. A Referral Form will be issued, and the
student will
proceed as follows:

Attendance/Absence Policy

10 Policy

1.1 The school encourages excellent attendance so that students can

get the most out of their education. Dedication to lectures, lab work,
assignments,
and clinical experiences are required.

1.2 Students are required to attend all scheduled classes.

1.3 If absences cannot be avoided, students are required to call the school or
clinical
area 30 minutes prior to start times.

1.4 When a student is absent, it is his/her responsibility to inform the Instructor of


the
reason for the absence, and to arrange to make up missed clinical
assignments and
class work.

1.5.1 Two (2) absences will result in being placed on probation.

20 Tardiness and Leaving Early

2.1 Classroom Policies

2.1.1 Students arriving late for an exam or a scheduled test will be admitted
to
class and allowed the remaining time allotted to complete the test.
Pop
quizzes may not be made up.

2.1.2 Tardiness and leaving early disrupts the learning environment and is
discouraged. Three (3) occurrences within one (1) month will be
tallied
as one (1) absence.

2.2.1 Failure to meet clinical objectives due to excess absences will result in
probation or dismissal from the Program.

2.2.2 Students arriving more than 15 minutes after the scheduled arrival
time
will be sent to the campus to schedule a make-up day.

2.2.3 Students may not leave the clinical site before the designated time.

Dress Policy
After admission to the program, the student must comply with the following dress code,
which is
derived from concepts of asepsis, protection of the patient and professional appearance.
The basic
school dress policy will be followed on the first day of school.

1.0 General

1.1 Only the approved school uniform is to be worm on the clinical units, except
when
a specific hospital requires a specific dress policy. Students may not wear lab
coats.

1.2 The uniform should be laundered and free of wrinkles each time it is worn in
the
clinical area.

1.3 Only nursing shoes or white nursing sneakers will be worn with white
uniform.
No clogs, platform shoes, open toed shoes or any other shoes are allowed.
White sneakers or solid white tennis shoes without design may be worn on
campus
with designated school scrubs.

1.4 The name tag is to be worn on all hospital units at all times. It must be
attached to the
lab on your uniform. No emblems or tags may be affixed to the name tag.
School
patches must be worn on the left sleeve.

1.5 You are to be dressed in full uniform before pre-conference and not to change
clothes
until after post-conference.

1.6 Nothing is to be worn around the neck including jewelry, key and pencil
holders.
1.7 Students may wear school scrubs or school uniform on campus only.

1.7.1 If the student does not wear scrubs, he/she must be in full school

uniform.

1.7.2 A white or school maroon cardigan may be worn on campus or at the


clinical site. If a cardigan is worn, the name tag must be
attached at the collar. No jackets or sweatshirts are allowed at any
time
while in uniform.

1.7.3 Hats may not be worn on campus or in clinical.

1.8 No cellular phones or pagers will be carried by the student in the clinical
area
or classroom. If an emergency arises, the school should be called and the
student
will be immediately notified.

2.0 Personal Hygiene and Grooming

2.1 Good personal hygiene is expected.

2.2 No gum chewing or smoking is allowed on the hospital units. All hospital “No
Smoking” policies must be observed.

2.3 Hair must be worn off the collar and face. Hair must be secured to avoid the
possibility
of falling into food or onto a sterile field. Hair must not be allowed to fall onto
a patient.
Ponytails are permitted if secured at both ends. If hair is dyed it must be
natural hair color.

2.4 Make-up should be subdued.

2.5 No perfume or cologne is allowed. Scents in hairsprays, deodorants, hand


lotions, etc…
all this may be offensive to patients.

2.6 Fingernails should be short and clean. Acrylic nails are not allowed. Only
clear nail polish may be worn.

2.7 A. No facial or oral jewelry is allowed

1. No earrings will be allowed.

B. Only one plain band may be worn on one finger.

C. Bracelets or necklaces may not be worn at anytime.

D. One watch with a second hand will be worn at all times.

2.8 A current CPR card must be in the student’s possession when at a clinical
assignment.

4.0 Full Uniform-Female

4.1 White pantsuit with white short-sleeve top with


school patch sewn on left sleeves . Uniform pants only. No
jeans, knit pants or tapered pants are allowed. School uniform scrub will be
worn on campus

4.2 Knee length white or natural hose or white socks may be worn with pants.
Stockings with designs may not be worn.

4.3 White or beige bra.

5.0 Full Uniform-Male

5.1 Uniform shirt with school emblem sewn on left sleeves. Plain white T-
shirt may be worn underneath. School uniform scrub will be worn on campus.

5.2 Knee length white socks only.

5.3 White uniform pants only, no jeans, knits, tapered pants or scrubs are allowed.

Infraction of Dress Code


1.0 Infraction of Dress Code

Those students not complying with the clinical policies will be sent home from clinical, be
given an
Absence, and school policies regarding clinical absences will be in effect.

1.1 If the fraction is correctable i,e., hair, jewelry, makeup, the student must
correct
it an remain in the clinical area. A deficiency notice will be written explaining
the infraction. With the second infraction the student will be sent immediately
to school to see the Nursing Administration. This will then be an absence.

1.2 If the infraction is not correctable i.e., improper uniform, the student will be
immediately
Sent to school to see the Nursing Administration. A deficiency notice will be
written and the student will receive an absence.

1.3 Students are responsible for upkeep of uniforms and must replace all lost or
damaged uniforms and equipment.
1.4 Second infraction, student will be removed from the clinical site, sent directly
to the
Nursing Administration, receive an absence for the day, and be placed on
performance
probation until term completion.

Grading clinical Grading Policies

10 Clinical Grading Policy:

2.1 Grades for the clinical segment of a course are assigned as follows:
P - PASS Student has met all clinical objectives.
F – FAIL Student has not met all clinical objectives

2.2 Any student who fails to achieve a grade of “P” (Pass) in the clinical segment
of
a course may not progress to the next course in the program and must
petitionfor re-admission to take both theory and clinical portions of the course
failed.

2.3 Clinical Instructors will provide each student documentation for clinical
evaluation.
Each student will be evaluated according to stated clinical objectives in each
term.

2.4 Instructors will provide students with on-going written or verbal program
reports
as well as a skills proficiency check list to keep students apprised of their
progress.
The lead instructor will provide the clinical instructor with satisfactory
academic progress reports as needed.

2.5 A PASS grade is determined by compliance with basic hospital rules of


conduct
and satisfactory performance of clinical objectives and skills.

2.6 A FAIL grade is determined by noncompliance with basic hospital rules of


conduct
or unsatisfactory performance of clinical objective and skills.

Testing Policy
1.0 Policy
1.1 If possible, the student must inform the Instructor in advance if the student
will be
unable to attend class on a scheduled examination day
1.2 If the absence is due to illness or emergency, written verification must be
submitted
to the Instructor in order to be eligible for a make-up examination.
1.3 Make-up testing schedule is determined by each individual instructor and
presented
at the class start.
1.4 POP QUIZZES ARE NOT ELIGIBLE FOR MAKE-UP EXAMINATIONS.

10 Term 1 Policy
1.1 Any student receiving a failing grade for either Intro to Supplemental Nursing
Care IA or IB, will be dropped from the Vocational Nursing Program.

A. The student may apply for re-admission to the vocational Nursing


Program
upon a space available basis, recommendation of the Instructor and
approval of the Director of Nursing only.

term in the program, without attending remediation and passing the
failed unit with a 75 %
C. The third failure within the same term and the accumulating grade is
below 75% , the student may not progress to the next term.

2.0 Terms II, III, and IV Policies


2.1 Unsatisfactory progress, such as a system failure, constitutes an academic
warning status
Notification for the remainder of that term and immediate referral to the
Education Assistance office.
2.2 A student will be permitted to remediate only one failure per term providing
that:
A. The student successfully completes and passes the remediation
requirements
upon completion of that term. This will allow the student to receive
the
maximum allowable grade of 75% for that system.
2.1 A student who fails to meet the remediation requirements as above, or
receives a
Second system failure within the same term, may be dropped from the

C. The new grade received for all subjects in that term, will then be
entered.

Probation and Dismissal


1.0 Policy

1.1 The School reserves the right to place students on a probationary


or dismissal status. Students may be subject to probation or dismissal for any
of
the following:

A. Completed academic performance at scheduled evaluation period


results
in cumulative Grade Average of less than 75%.

B. Failure in attaining clinical objectives and/or satisfactory clinical


Performances for scheduled evaluation period.

C. Habitual violation of Attendance Policy.

D. Failure to adhere to the Nursing Program’s General Conduct

1.1 Students placed on probation will receive written notification, a counseling


appointment
With the Nursing Administration, and a scheduled time allotted for
remediation. Failure to correct and/or improve the deficiency will result in
dismissal from the Nursing Program.

Leave of Absence (LOA)

1.0 Policy

1.1 A Leave of Absence may be granted under special circumstances. A student


may
receive one (1) Leave of Absence during the course of the educational
program

1.2 Leaves of Absences are for emergency purposes, up to 90 days.

1.2 The time taken for a Leave of Absence will not be counted against the
maximum allowable time for program completion. Hours of the program will
resume when the Leave of Absence is officially completed.
1.4 Leaves of Absences cannot be taken within the first 90 school days.

Senior Practicum
10 Term IV Policy for Nursing Specialties

1.1 As part of Supervision and Leadership, each Term IV student will be required
to select nursing specialties/unit for 24 hours or 3 days and or mentor/guide
from one to three new Term I students. The goal is to provide direction and
assistance in meeting their career goals and specialization

A. Each Term I student will be given the name of an incoming student or


students by
the Nursing Department.

B. The initial introductions will take place on Orientation Day for Term I
students.

C. It is the responsibility of the Term IV student to initiate further contact


and to

maintain a line of communication and inform the Nursing Administration


the selected unit of specialization for clinical arrangement on the
respective affiliated facility.

Education Assistance/Remediation
Policy and Procedure

Purpose

The purpose of Education Assistance is to provide supervised time for the students to
increase their understanding of theory objectives and proficiency in clinical skills, such as
the review of mathematics, aspects of medication administration, assessment, and clinical
documentation.

Clinical Instructor Referral


The instructor may refer a student to Education Assistance if the instructor feels that the
student needs to increase their knowledge base of clinical or theory subjects.

Theory Instructor Referral


Students with a grade below 75% in a subject, must attend remediation for that subject.

Student Referral
The students are encouraged to come to the lab independently to increase technical
proficiency and expand their knowledge base.

Student Education Assistance/Remediation

10 Policy
1.1 Theory Students

1.1.1 All Term I students receiving less than 75% on any test or quizzes will
be required to meet with the Educational Assistance Coordinator
during office hours. The Coordinator will create a remediation plan for
each student. The remediation plan includes a review of the material,
homework assignment and a retest on content. Upon successful
completion of the homework assignment the student is then retested.
The student achieve 75% on the retest. Only three (3) long test
remediation are allowed for the whole term.

1.1.2 All Term II – IV students receiving less than 75% in a single subject
failure must successfully complete remediation to continue the term.
Remediation consists of a material review, a homework assignment
and a retest on content. Upon completion of the homework
assignment the student must achieve a 75% on the retest. The
student may only remediate two system per term. A 3 rd system fail in a
term will require a repeat of the term.

1.2 Clinical Students

1.2.1 Students deficient in any skill or clinical objective will be required to


have remediation. The Clinical Instructor will provide a remediation
plan that may include homework assignments, lab time, individual
counseling and instruction. The clinical instructor may also refer any
student to the education assistance coordinator.

1.2.2 Students who demonstrate unsatisfactory skills after remediation will


be removed from the clinical area and receive a clinical failing grade.

Remediation Procedure

Remediation is available to students to assist them in strengthening their skills. The


procedure is as follows:
➢ After each test in Term I and each system in Term II – IV
the theory instructor will

75% on retest, non-completion or deficient return
demonstration, homework/project/study guide, did not
retest) will receive counseling on failing grade by
Education Assistance Coordinator and Head of Education
and /or Lead Instructor

Definition of Clock Hour


School measures student progress in clock hours. A clock hour in theory and clinical
coursework is defined as follows:

1 Theory Hour = 50 minutes of instruction

1 Theory Hour = 50 minutes of clinical instruction

Standard of Satisfactory Progress

At Nightingale International, satisfactory progress for All students is defined by the


following criteria:

1. Students are permitted to attempt up to 1.5 times the number of hours in


their program.
2. Students must successfully meet or exceed the minimum GPA or course
completion standards set forth below. The evaluation points are based on
their percentage of clock hours attempted.

and successful course completion while enrolled at the program. The student
progress will be reviewed at the end of each module to determine satisfactory
academic progress. The school requires student to participate in remediation when
failing a test.

Academic Policies and Information


Program Measurement

The school measures its program in a quarter credit hours/units to allow for
comparison with other post-secondary institutions and clock hours allow
measurement of the program on this basis where required.

• for lecture classes, one-quarter credit hour/unit is equal to 10 clock hours


• for laboratory classes, one-quarter credit hour/unit is equal to 20 clock hours
• for externship work experience, one- quarter credit hour/unit is equal to 30
clock hours

*** a clock hour is a minimum of 50 minutes in which lectures, demonstrations and


similar class activities are conducted

*** attendance is vital to academic progress. Students are expected to attend all
classes and clinical learning experiences. The student must adhere to the
attendance policy, specially as it relates to “ Make-up of theory and clinical
coursework. In order to earn a satisfactory grade course, the student must complete
95% of the hours instruction offered in all theory and all clinical coursework. Refer
to the attendance policy for additional information.

Clinical Make-up Policy

1.1 In order to be eligible for Licensure, Vocational Nursing students are required to
complete 982 clinical hours

Practical Nursing Program Commitment Contract


I understand that the following items are important for completion of the Practical Nursing
Program. Each item has been fully discussed with me in detail.

 I must have transportation to and from the school and the training hospitals (clinical).
 Hospital rotations are based on skills and experience needed for the completion of this
program.
 I must be willing to travel to whichever training facility (clinical) I am assigned within
Philippines.
 I must accept the mandatory hospital rotation assigned to me without exception.
 I must be available for hospital (clinical) assignments for 8 or 12 hour shifts. A from
6:45am – 2:45pm or 2:45pm- 10:30pm or 7am-7pm.
 I must have childcare and personal matters arranged so that they do not interfere with my
school hours. This includes the 2 month time period (Mon- Fri) after the program is
completed, to prepare for the NCLEX- PN.
 I must be willing to commit myself to completing the program successfully. I will
accomplish this by being in attendance, being on time, maintaining good study habits and
not obtaining a job with no more than 20 hours per week for the day program and 32-40
hours per week for week-end program.

Clinical Orientation Policy

According to the Nightingale International policies as well as the policies


that regulate the clinical facilities all students must attend the first day of clinical.
Attendance of this day is for assurance that all requirements are met for orientation
to the hospital, including: infection control, fire and safety, floor assignments,
charting, absence policy, medication policy, care plans and all other required
hospital policy.
 All students must attend orientation day, no exceptions.
 Failure to attend orientation will result in loss of your clinical rotation
assignment and dismissal from the current term.
 You must attend the entire orientation day.

Clinical Makeup Policy


Arrangement for clinical makeup must be done within one week of the clinical absence or the
following week of the return date from the date of absence.

All clinical and theory absences must be made up before the end of the current term, or the
student cannot progress to the next term.

When a student is absent from clinical, a makeup or educational assistance form must be
obtained from the clinical instructor. Bring this form to the clinical coordinator and have it filled
in with the clinical makeup date and time. Give this form to the instructor at the clinical makeup
site.

Physical assessment and progress note for each patient cared for must be attached to the makeup
form. If these are not satisfactory, the student must schedule another clinical makeup date.

Theory and clinical makeup hours go on the same form and need to be turned in prior to the end
of the term.

I understand the above clinical makeup policy. Any absences exceeding 16 hours per quarter
require making an appointment with the clinical coordinator.

Clinical Makeup Guidelines

1. You must go on your scheduled day; there are many students needing makeup hours; if
you miss your day you must see the clinical coordinator of the PN program.

2. Clinical dress code and attendance policies for makeup days are the same as any clinical
day – if you are more than five minutes late, you will be sent home. You cannot leave
early or you will not receive any credit for the day. If you are noncompliant with the
dress code and it cannot be corrected, you will be sent home.

3. You must be an active participant in the clinical setting for 8 hours. Any student stating
they are just “making up time” and not involved in patient care will be sent home.

4. You must have a Make-up or Educational Assistance form to give to the clinical
instructor at the clinical makeup site. If you do not have that form, you will be sent home.

5. The signed theory/ clinical makeup form needs to be returned to the clinical coordinator.
Without this form, you will not received credit for your makeup hours. Your Board of
Practical Nursing Papers will not be sent until all of your hours, both theory and clinical,
are made up.

6. You must attach to the signed makeup form, the physical assessment and progress note
for each patient that you cared for. These must be satisfactory or you will not be given
credit for you makeup hours.

7. The instructor at your makeup clinical site is aware of these guidelines. Get the most out
of your makeup hours!
