CHAPTER I: PLANNING PHASE


ABSTRACT
Based on the proponents observation and through an interview with
the head nurse of the clinic, the proponents were able to identify some
things that need improvements. The said department is having some
problems with the existing process such as having only a manual listed
information on a logbook, difficulty in searching of records because all the
files were only stored in one cabinet, and having a low security which can
cause loss of patients(students and employees)records.
The proponents came up with the idea to develop a system for the
clinic of Lyceum of Subic Bay. The proposed Clinic Information System for
the Lyceum of Subic Bay helps to improve the manual monitoring,
updating, and adding medical and dental records inside the school clinic of
the Lyceum of Subic Bay. Specifically, it will improve the manual listing of
transactions, inventories, and reports. The use of this system is to help the
head nurse to easily manage the patients medical and dental history. As
the result of improving the manual process that the clinic is using, storing
and searching of patient information will be even faster. All of the patients
(students and employees) records will be secured without a need of only

one cabinet. The transactions that are done inside the clinic will be much
easier because it is a computerized program.
The use of the system will be of benefit of the clinic administrator
and the patients (students and employees).

INTRODUCTION
Today knowledge has power. It controls access to opportunity and
advancement. These are the words from a writer, professor, and a
management consultant, PETER F. DRUCKER. As of now, people live in the
information age, our society is driven by knowledge and this knowledge
has power, an ultimate one. But it is what you do with it that really counts.
So what should the people do with the knowledge that they currently have?
It is a big world out there, big enough for everyone to derive benefit from
each and everyones knowledge. How you use it in a productive way is very
important. People should have to take the initiative and share their
knowledge with others. They need to help each other improve, and they
can do that by sharing strategies, mindsets, and other types of information.
Empowered by the idea of the man who invented management, the
proponents are willing to develop a system that will help the locale of the
study. Through effective coordination, the proponents will use all the
possible knowledge and resources efficiently to accomplish our desired
goal.
Schools and Universities nowadays have their own school clinic
where students and faculty members who have health issues can ask for

Most

of

their

clinics

use

computerized program that will input and store all the medical records and
transactions in a database. The locale of the study, the Medical Healthcare
Department of Lyceum of Subic Bay, has some aspects that need
improvement. The proponents were given the chance to analyze and fully
comprehend how the clinic handles and records the profiles of the patients.
Processes and transactions are done manually ever since the school clinic
was established in 2004. It is very tiring for the part of the head nurse to
check for the record every time a patient comes back for another
transaction. It also takes a lot of time finding the existing records for a
particular patient (students and employees). Their records are being filed
in folders, maintained and updated by the head nurse without any data
backups. And all the records are locked up in the filing cabinet so there is a
possibility that unauthorized personnel can access the files by simply
destroying the lock.
The proponents decided to develop a computer-based information
system that will help the clinics current system become more efficient and
less time consuming. This will be customized according to what the clinics
requirements. It will keep track the medical and dental records of each
students or school personnel and it will organize the reports of inventories
and transactions in a timely manner. Storing of information on the physical

and dental examination of each patient will already have a computer based
program.
In case of emergency, this system will be an advantage to the head
nurse and to the doctor. The system will retrieve or update the records
immediately. Upgrading of the current system will be a big help for the
school especially for the school clinic of Lyceum of Subic Bay to make all
the transactions inside the clinic faster and easier.
The best way to predict the future is to create it.

STATEMENT OF THE PROBLEM


General Problems:
The process of monitoring and searching of clinical records are time
consuming because it is done manually.
There is no other secured copy of clinic files aside from the manual
records.

Specific Problems:
Patients manually fill-up clinic transaction in a logbook that has the
possibility of mistakes as a result of misreading a patients
handwriting.
Updating of records for returning patient can cause delays because
the nurse will by hand look for the patients previous records on the
filing cabinet first to check the medical history.
All the files are only locked up inside a cabinet therefore there is a big
risk of the files being flushed away by floods or being burnt down by
school fire.
The clinic inventories are being monitored manually.
Due to manual process the clinic is using, reports are also affected in
a way that they are created based on the manual documents.

OBJECTIVES:
General Objectives:
To be able to develop a computer based system that will improve the
performance inside the school clinic.
To develop a system that will backup all the clinic files in a database.

Specific Objectives:
To have an accurate and faster medical transaction by not writing on
a logbook instead the patient will give the information and the user
will encode it to the system.
To help the user for fast modification and retrieval of information by
searching information in a short period of time through the use of the
proposed clinic system.
To improve security and avoid a complete damage of all the files in
case of unfortunate events by using the proposed system.
To categorize the clinic inventories by sorting them into medicines,
equipments, and consumable supplies including some details about
each inventory.

To generate reports of daily transactions inside the clinic quicker.

SIGNIFICANCE OF THE STUDY


The proposal is a computer-based system that will upgrade the
clinics manual storage of records which is stored in a logbook and locked
up in a storage cabinet. The current system will improve in a way that all
the records of transactions, inventories, and medical/dental examinations
will be monitor, update, retrieve and report faster and easier. Through this
new process, the problems regarding the possibility of losing all the files
through the manual process of storing will eliminate. Just like the other
schools and universities, Lyceum of Subic Bay Clinic will be also now using
a computerized program of encoding and storing important records.
The target users of this proposal are:
a.) The Head Nurse
-The school head nurse has the full access to the system. She will
easily

update,

print

reports

and

keep

track

of

the

medical/dental, inventories, and transactions records.


b.) Nurses
-For emergency purposes, when the head nurse is not around. The
next person that can access to the system is the head nurses co
nurse that has an account on the system which has been approved by

the head nurse. The co nurse can only view all the transactions and
needed records but cannot edit, add, delete, or update anything.

The beneficiaries of this proposal are:
a.) Patients
-Such as the students, professors and other school employees are the
beneficiaries of this proposal. Through an organized system of storing
information and retrieving the records, the said beneficiaries would be
given a proper medication they needed. Instead of writing the
transaction on a logbook, they will just now give the needed
information to the head nurse and it will be encoded to the system.

SCOPE AND LIMITATIONS


This proposal aimed on how the proponents can improve the
efficiency of the services. It is design for the school clinic of Lyceum of
Subic Bay to upgrade the manual process they are using inside the clinic.
This will help the user to easily track and update the medical and dental
history of every student and faculty members during the school year.
Retrieving of records for emergency purposes will be much easier. The
clinic reports will be organized and provided on time. In proposed system most
of the work is done by this software man power is reduced and data is stored in database. With
these features total performance of the medical clinic inventory system is increased.

Almost all the things developed here in this world have limitations.
The system is only design inside the clinic of Lyceum of Subic Bay. There
would still a need of manual documentation for new records and updating

purposes such as changes on the patients address, contact number, age
and more. This proposal is a stand-alone information system which cannot
be access through a web since the only user is the clinic administrator.

LOCALE OF THE STUDY


The locale of the study is in the Lyceum of Subic Bay Lot73 Central
Business District Subic Bay Freeport Zone SBMA Olongapo City,
Philippines. Inside the school, there is place where the students and faculty
members ask for medical attention during the class hour and that is the
School Clinic of Lyceum of Subic Bay. The proponents study will be of
benefit of school clinic.

Observation

The proponents have observed how the clinic works through their
own experience of having transaction inside the school clinic every
time they ask medication, medical certificate, or other purposes.

Interview
-

The proponents had gathered information through an interview with


the head nurse of the school clinic. Through this method, the
proponents identified the current process they have that needs
improvement and understand how the clinic works.

The proponents have used online resources for them to gathered


more ideas about how the proposal will turn out to be more
interesting and effective for the user.

Summary
The main reason why the proposal was made is to improve the
routine inside the clinic of Lyceum of Subic Bay. The proponents have fully
understood all the processes that are done inside the clinic and identified
what are the things that needed improvements.
Through conducting an interview to the clinics head nurse, the
proponents have gathered lots of ideas about how the clinic works, what
are the usual complaints of the patient, what files do the clinic have and
how did they handle or keep those important files.
Compared to other College Schools and Universities clinic, the
school clinic of Lyceum of Subic Bay doesnt have any other file backups
like computer-based storage aside from manual storage. All the clinic files
were placed and locked up inside a cabinet. The manual filled up clinic
documents which is stored in a concrete storage have a low level of
security. There is a possibility that unauthorized personnel can have the
access to the cabinet by destroying the lock. It is also possible that all the
clinic documents will wipe out in case of unfortunate events like flood or
school fire. The clinic used manual process of storing and checking clinical
records since it was established. The head nurse or her assistants were the
one who will search a clinical record of a patient manually in case of
urgent situation. They manually managed the number of total, available

and used inventories for inventory reports. And also it is very tiring to
manually search the transaction and medical history of a returning patient.
All those manual method caused delays and is exhausting on the side of the
clinic administrator. That is why the proponents came up with the idea to
develop a system that will improve the clinics current system.

Conclusion
In todays generation, people cannot imagine a world without
computers. It is very much accurate, fast and can accomplish many tasks
easily. That is the reason why the proponents decided to build a system for
the Lyceum of Subic Bay clinic to upgrade its manual system.
The proponents have gone several revisions to make a better system.
There are a lot of trial and errors that have been done. And by that, the
proponents became more determined to develop a better system and openminded to possibilities.
The Proposed Information System for Lyceum of Subic Bay was
developed to improve the current system that the clinic is using. To be able
to secure all the important files and to easily track all the clinic
transactions that was done inside the school clinic. The proponents made a

system not to replace the manual system but to improve the process of
documenting and for it to have a secured backup.
Though the manual system has its advantage, having a computerized
system is more reliable and more efficient nowadays. Computer has
become an important tool for keeping databases and tracking records. It
has made record keeping and account keeping much easier than it ever
was. Data analysis became easy and it can be done at a single click of the
mouse.

Recommendation
The proponents have used different software to develop a functional
clinic system for the Lyceum of Subic Bay. Some of this software is
Microsoft Visual Basic .Net, Microsoft Access 2003, and Microsoft 2007. It
doesnt necessarily need to be the same version as the other, just like what
the proponents have done. The proponents have used different versions for
their software but dont have encountered any glitch. The proponents
recommend the use of earlier or later versions of the same programs,
depending on its system requirements.
There would still be an advantage for using the medical records that
is manually filled up by the patient. It will be time consuming for the part
of the head nurse if she will encode all the medical information of each
patient in the system especially when theres a lot of patient waiting for
medical attention. So the proponents is suggesting the best way to save
time is to let the patient fill up the medical examination sheet first and give
the appropriate medical attention for the patient. Since the head nurse
doesnt want others to have access to the system, the head nurse should
encode all the written information in the system to have a secured backup
and to easily retrieve records after all the medical consultation. Through

using the proposed system, the patients would be able to have a proper
medical attention that they need during the class hours.
For the future researchers, the proponents advise that they upgrade
the proposed system in a way that the patient can encode their own
personal information on the system. That is if the head nurse will already
allow others to access to the clinics desktop or the clinic already have two
desktops. The decision is still on the future developers hands on how they
can help the school clinic. The important thing is the locale of the study
will have the benefit they could ever have.
