Thank you for requesting an assessment test for your child. You have taken the first step
towards ensuring your childs academic progress. School Supplement offers all- year round
enrichment programs in Language Arts, Math and Science.
Instructions for Parent while administering this test:
1. Please provide a quiet place for your child while taking this test. Shut off all distractions
that may affect the performance of your child while taking this test.
2. Please do NOT use a calculator.
3. Please do NOT prompt/guide your child to the correct answer.
4. Make your child go over the entire test. Some questions in this test are below/above the
grade level you chose for your child. Complete the entire test so we can assess your
childs strengths and weaknesses and identify skills gaps.
5. This test can be administered in more than one sitting.
6. Please do NOT make your child retake the same test to get a better result by explaining
the concepts in the test. The test results will be the input to the customized program for
your child, so it has to provide a true picture of your childs abilities and weaknesses.

Thunder crashed overhead as flashes of lightening streaked across the April sky. The rain was
coming down harder now. Alex watched as they hit the window pane of their living room bay
window and trailed down the glass like tiny, little rivers.
Turn on the news, Dad called from the kitchen where he was putting a roast in the oven.
Dad stopped cutting the cucumber that he was planning to use for the salad and started
scurrying through one of the cabinets for a plastic bag to put the cut up pieces in.
Hurry and help me turn off the roast, he said, I should call your mother to let her
know whats going on. I dont want her driving home if the weather is bad. Before her dad
could get to the phone, it started ringing.
Alex picked it up, Hello, she said into the receiver.

Alex, tell your dad that were under a tornado warning. You and your dad have to get to
the shelter. I wont be coming home until the storm is over, her mom said on the other end.
We know, Mom. We were just about to call you and tell you that too, Alex said. By the
time Alex was done saying bye to her mom, her dad had already grabbed the flashlights.
Lets go, he said to Alex as he tried to rush her out the door.
Outside, the rain had slowed down, but icy gusts of strong wind continued blowing against
Alex and her dad as they made their way towards the shelter that was located by the side of
the house.
Overhead, the dark clouds were moving at an incredibly fast pace. Alex quickened
Above the ground, the shelter was nothing more than a rectangular metal structure that
stuck out from the earth. However, once the huge metal door was pried open, a set of stairs
led to a cement room that was the about the size of their bathroom. After Alex and Dad were
safely inside, Dad closed the giant door and locked it. Then he turned one of two lanterns on.
There was no electricity in the room, so everything was either powered by battery or with the
generator that Dad had stored down there. However, her parents had made the rule that the
generator wouldnt be turned on unless it was necessary, and necessary meant that they were in
a situation where they needed to be down in the shelter for more than a couple hours. Alex
helped her dad turn the radio on. It was already set to one of the local stations. Music blasted
from the speakers. Dad handed her a sandwich that he had managed to put together while Alex
had been on the phone with Mom.
Its not much, but hopefully the storm wont last too long, he said. They both sat

An hour later, the radio announcer said, The National Weather Center has just reported
that the tornado warning for Logan, Johnson and Franklin counties has just been lifted.
Alex looked up from the chess game that they were playing, Did you hear that?
Hmmm? Dad asked. He was still considering his next move.
The tornado warning is over. The radio announcer just said it.
Checkmate! Dad said, moving his knight. Alex grinned. She was glad it was finally over.
