EQUIPMENT LOAN CONTRACT and

HOLD HARMLESS AGREEMENT

By signing the Equipment Loan Contract Form below, the Borrower/Assignee agrees to
the following:

Surveyors who borrow (Borrower) or are assigned (Assignee) portable equipments

(Equipments) for performing inspection for cargo quantity and quality must complete this
agreement when portable equipments are entrusted to your care.

The Borrower/Assignee must be an employee of OCEANUS INSPECTION SERVICES PTE LTD

and/or sub-contactor(s) authorized to represent OCEANUS INSPECTION SERVICES PTE LTD to
perform any assignment(s).
The equipment is for job assigned representing OCEANUS INSPECTION SERVICES PTE LTD
The Borrower/Assignee agrees to accept full responsibility for the equipment and will reimbursed
for any damages incurred by misuse, negligence, carelessness, or failure to follow required
procedures as described below.

The Technical Manager will investigate and determine if reimbursement is required:

The Borrower/Assignee agrees to be the sole user of the equipment (including software).
The Borrower/Assignee agrees not to make unauthorized copies of software to third party.
The Borrower/Assignee agrees to return the equipments on or before the date specified on
last day of work. Failure to return the property on the date specified may result in a demand
for reimbursement of the full value.
The Borrower/Assignee agrees to return the property at any time when asked to do so by
the management.

Responsibility:
Surveyors using the equipments must use all reasonable precautions to protect company
property in their sole care, custody and control.

Surveyors shall ensure that all equipment, including equipment not under the charge of the
company, used during inspection work is calibrated and traceable to the SI unit.
Particular attention must be given to safeguarding company property be protected from
damage and deterioration during handling, maintenance and storage. Surveyors using the

equipments must report immediately when a defect is detected. (EQUIPMENT FAULT

REPORT) form.

Special attention must be paid to equipments that required calibration, surveyors shall ensure
the accuracy and cleanleness of the equipment. Unplanned check will be conducted by
Technical Manager or Representative, any non conformance found might results disciplinary
actions taken.

The police must be notified immediately upon becoming aware that property has been
stolen or damaged. Lost property must be reported to the Principal/Site Administrator
within 24 hours of discovery.

IN WITNESS WHEREOF, the parties hereto have duly entered and executed this Agreement as of the day and year first
above written and represent and warrant that the party executing this Agreement on their behalf is duly authorized.
