3.1.
Induction
Prior to start work all personnel must attend the specified HSE induction course, conducted by the
COMPANYAreaAuthority.Allemployeesalsoshallsignthesheetsofacknowledgementwithcorporate
HealthandSafetyPolicy.Inductionlogbookshallbeproperlyimplementedandmaintained.Personnel
whohavenotpassedtheProjectInductionCourseshallundertakenowork.
3.2.
AllpersonnelinvolvedinworkactivitiesshallcomplywiththeprojectPPEstandards.TheminimumPPE
requirementsonsiteduringworkactivitiesshallbe:
SafetyBootswithmetaltoeprotectors
SafetyHardHat
AppropriateEyeProtection
Coveralls
Gloves
HearingProtection(whereapplicable)
Additional PPE identified by the risk assessment shall also be implemented. Personnel shall take
reasonable care about personal protective equipment issued by contractor. PPE logbook shall be
implemented and maintained to ensure all respective personnel is issued with standard PPE set and
defectiveordamagedPPEisreplacedwithappropriateone.AnyviolationofPPEstandards,misuseof
corporateissuePPEorequipmentwillbethesubjectfordisciplinaryaction.HandmadeorpersonalPPE
willnotbeacceptablebyCONTRACTOR.
3.3.
Thecrewsupervisorshallinspectallequipmentfordamageand/ordeficienciespriortothestartofwork
ondailybasisandappropriatesurveillanceshallbeimplementedandmaintainedonequipmenttocheck
ifithassustainedfromanydamageduringthecourseofexploitation.
Defectiveitems,thataffectthesafeoperationofthepieceofequipment,shallbeimmediatelyremoved
fromserviceuntilrepairedorreplaced.OtherdeficienciesshallberectifiedonASAPbasis.
CrewsupervisorsshallcompletedailyaPlant&EquipmentChecklistfortheitemofplanttheyoperate
and shall report all deficiencies to the site supervisors who are in charge of rectification from any
deficiencies.TheTechnicaldepartmentshalldocumentanyrectificationworkperformed.Allpersonnel
shall exercise care when performing their duties with the contrast movement of vehicle plant and
personnel within the work area. The Crew supervisors and workmen shall take special care when
workingandmovingnearorunderoverheadservicesandburiedexistingservices.

3.4.
Risk Assessment
Suitable and sufficient risk assessment shall be implemented by CONTRACTOR, which shall cover
activitiesofcontractorspersonnel as wellasvisitors,clientspersonnel.Refertothespecifictaskrisk
assessmentfordetailsoftheassociatedhazardsandmitigationmeasurestobeimplemented.
Prior to the commencement of works the risk assessment shall be reviewed by HSE Department and
Operations(AreaAuthority)inconjunctionwithConstructiondepartmenttoensurethatalltherisksare
addressed and the required control measures are in place, and followed during operations. Lessons
learnedpracticemustbeimplemented.
3.5.
Prior to the commencement of works the Crew supervisor, with support from the HSE Officer, shall
conduct a prestart toolbox talk with all of the crew present. The crew supervisor shall explain the
method of works focusing on the hazards, risks and the mitigation measures that shall be taken as
detailedintheRiskAssessment.AllTBTsshallbedocumentedandfiledaccordingly.Registrationshall
takeplaceinworkplaceinstructionandtraininglogbook.ThecustodianofallTBTdocumentationshall
bethecrewsupervisor.
3.6.
Permit to Work
APTWshallbeobtainedinaccordancewiththerequirementsofPermittoworkProceduredescribedin
corporate occupational health and safety manual, prior to performing the following operations or
activities.Thesiteandcrewsupervisorshallexplainthemethodsinvolved,focusingonthehazardsand
risks involved the activity, and the mitigation measures that shall applied as detailed in the Risk
Assessmentincluding.
3.7.
Environmental Impact
Prior to commencing work all personnel shall have been inducted by the CONTRACTORS HSE
DepartmentinaccordancewiththerequirementsoftheProject.Personnelwhohavenotattendedthe
ProjectInductionCoursesshallundertakenowork.
TheHSEDepartmentshallensurethatalltherisksareaddressedandthecontrolmeasuresareinplace.
3.8.
Allequipmentshallbemaintainedinefficientstatetominimizenoiseandvibrationemissions.Potential
sourceofnoiseshallbeidentifiedandcontrolmeasuresimplementedasfarasitreasonablypracticable.
Regularnoiseemissionsmonitoringshallbecarriedoutbytrainedpersonnel.
IncasenoisemonitoringiscarriedoutbyCOPMANYpersonnelonregularbasisandCONTRACTORnot
likelytoexceedregularsitenoiseemissions,thensubjectsectionisnotapplicable.
3.9.
Significant environmental impacts resulted from scope of work performed by CONTRACTOR shall
adequately addressed with consistent control and monitoring measures implemented. Where
necessary, an Environmental impact assessment shall be carried out by competent environmental
engineer.

3.10.
Prior to commencement of any work general environmental awareness programs will be run by
