B. Chief Complaint:
She kept on complaining why shes in Full House Custodial Center.
Sabi ng brother ko, addiction daw of smoking. 3-5 packs a day daw nauubos
ko. Di ko nga alam kung panu niya nabilang un eh. Diba Sister
Malakas din ako Sister sa kape. When I asked her how many cups a day,
she stated, Di ko matandaan Sister, basta marami akon naiinom, orasoras ata.
Nakikipag-away din ako sa telepono, pero sa telepono lang naman yung mga friends
ko.
Inaway ko din Sister yung asawa ng brother ko, yung sister-in-law ko. Kasi
pinapaalis ako sa bahay, eh bahay ko yun.
And she keep on insisting that her brother is going to get her back to their house.
Gusto ko nang umuwi Sister.
Nakita mo ba brother ko? Yung matangkad, maputi at parang foreigner? Brother ko
yun.
Sabi niya kasi susunduin niya na ako. Nasaan kaya siya. Sister, nakita mo ba?
Tawagan mo, sabihin mo, gusto ko nang umuwi.
C. Medical History
She had said that she had an asthma when she was a little.
She was diagnosed to have a Hypertension and UTI before.
She is Diabetic in which according to her, it was inherited from her mothers.
D. Psychiatric History:
She is diagnosed as Bipolar, has behavioural changes by Dra. Encarnacion on
April 2014.
There is no particular person in her family who has been diagnosed to have
mental dysfunction.
E. Personal History:
There is no particular memory that she can remember during her early childhood.
As what she had said during her Childhood, she was happy because her Mommy
was alive.
During her Adolescence, the only thing that she can remember is when both her
parents went to States to work and she and her brother, Augustus were the only
one here in the Philippines.

When she was 30 years old, her mother died in States. According to what she had
said, Di ko siya nakita nung namatay siya. Sinabi na lang ng brother ko, patay
na ang mommy ko. At nang mamatay siya sa States, nasa hospital ako dahil di
ako makahinga. She also said that she didnt went to her wake. Dun din
nagsimula ang sakit ko at nung pumunta ako Sister sa States at nang pinabalik ako
ditto.
