1 You have to identify minimum 23 uncontrolled hazards and 2 good practices. 2 While writing uncontrolled hazards in observation sheet, you have to mention , What type of hazard it is, - e.g. Oil spillage on the floor (Physical Hazard). What type of risk it has, - e.g. Slip What is the consequence of hazard (Minimum 2) e.g. specific Injury, fracture etc. Place, where you found this hazard. 3 While writing actions to be taken. you have to mention , Short term action Any control measures which can be implemented within 2 weeks Long term action Any control measures which can be implemented in more than 1 month According to the ERICPD concept you have to prioritize your control measures. 4 While writing timescales, Dont give all time scale as immediate. You can give time scales as short term & Long term like one day, one week, and one month respect to practical implication. Management Report :( 70 Marks) 1. Order for writing management Report: Introduction: (5 Marks) Clear and appropriate description of the company where you did inspection like, 1. Company Name, 2. Date and time of inspection, 3. Location, 4. Scope of inspection 5. Equipments and the activities occurring in the area. 6. Workers strength.
Executive Summary: (5 Marks)
Concise overview of important points and main conclusion /recommendations. Main Findings: (30 Marks) In the observation sheet you have to pick 5 major uncontrolled hazards and explain it in detail, 1. Activities, 2. Hazards, Risk Consequence, 3. Breaches of International and Regional HSE regulation. 4. Immediate and Long term Action 5. Cost of implication. Conclusion: (15 Marks) Based on your main findings, Consequence of non compliance respect to Legal, social and moral. Recommendations: (15 Marks) Recommendations mean actions to be taken to reduce the risk level of your main findings. Based on your conclusion and main findings. The recommendation table you have to make in the report separately based on the template in the management report. While giving your recommendations please consider the below points. Likely Resource Implication for implementing your action to be taken (control measures) which requires resources like cost, equipment, manpower and material that has to be added. You can add value of the resources also. Priority based on risk level, you can priorities the recommended actions like High, Medium and Low. Target Date- The action to be taken (control measure) has to be completed / implemented on or before the mentioned date. 1 Recommendations should be -based upon conclusion and main findings.
2 Recommendations should be realistic and target dates to be
mentioned. 3 It should be prioritized appropriately. 4 Include appropriate resource implication for all recommendations.
