The supervisor will ensure that pre-service and annual
physical examinations are completed. A copy of the
physical exam Form 564 must be maintained in the
personnel file of the FSW.
Good Practice. Supervisors should inform the family
services workers that all employees who use their
personal vehicles at work should let their personal
vehicle insurance carrier know about such use of the
vehicle.
