For Immediate Release: The Michigan Occupational Safety and Health
Administration (MIOSHA) issued citations for alleged violations of worker safety
provisions totaling $357,000 in penalties. MIOSHA is part of the Department of
Licensing and Regulatory Affairs.
MIOSHAs Construction Safety and Health Division conducted a fatality
inspection of Gates Electric, Inc. and Electrical Inspection Service, LLC at a Brink
Warehouse in Holland from June 16, 2015 to August 19, 2015. During the
inspection, six alleged violations were identified.
As a result of inspection #1071586, a total of one serious and four willful serious
citations were issued. The citations are for violations of Part 1 General Rules
and Part 17 Electrical Installations.
