Raymond Jeffrey Extension Mahayahay, Iligan City, do hereby declare and state the following:
That I am a General Manger representing LANAO BUILDERS & ENTERPRISES, with
the principal address at Lanao Builder Building, Macapagal Avenue Iligan City.
That the Republic of the Philippines, through the Department of Public Works and
Highways, will be implementing Widening of National Road at Tubod-Ganassi K1589+(936)-K1589-035, Poblacion Tubod Lanao del Norte, KO K1589+(-930)-KO 1589-(120)
with the contract amount of Php 20,754,714.37 under contract with Lanao Builders &
Enterprises.
That, pursuant to the above-mentioned undertaking, I do hereby mutually, irrevocably,
freely and voluntarily release and forever discharge the Republic of the Philippines By
authority of the Secretary of the Department of Public Works and Highways represented
by the District Engineer of Lanao del Norte 2 nd Engineering District, including its officers,
directors, employees, stockholders, and successors-in-interest of the FIRST PARTY,
from any and all manner of action, causes of action, sum of money, damages, liability,
responsibility, obligation, claims and demands whatsoever in law or equity, now and the
future, all of which claims or causes of action by these presents the parties hereby
abandon and waive.

Enterprises.
That, pursuant to the above-mentioned undertaking, I do hereby mutually, irrevocably,
freely and voluntarily release and forever discharge the Republic of the Philippines By
authority of the Secretary of the Department of Public Works and Highways represented
by the District Engineer of Lanao del Norte 2 nd Engineering District, including its officers,
directors, employees, stockholders, and successors-in-interest of the FIRST PARTY,
from any and all manner of action, causes of action, sum of money, damages, liability,

Highways, will be implementing Widening of Lanao del Norte Iligan Circumferential Road
K1527+ (-567)-K1527+(-460) R/S Only with Drainage and Road Right of Way K1527+(460)-K1527+(-223) Both sides with drainage with the contract amount of Php 11,
852,933.43 under contract with Grand Builders.
That, pursuant to the above-mentioned undertaking, I do hereby mutually, irrevocably,
freely and voluntarily release and forever discharge the Republic of the Philippines By
authority of the Secretary of the Department of Public Works and Highways represented
by the District Engineer of Lanao del Norte 2 nd Engineering District, including its officers,
directors, employees, stockholders, and successors-in-interest of the FIRST PARTY,
from any and all manner of action, causes of action, sum of money, damages, liability,
responsibility, obligation, claims and demands whatsoever in law or equity, now and the
future, all of which claims or causes of action by these presents the parties hereby
abandon and waive.
