Health and Well-Being Assessment
provides important information that your
doctor can use to help better manage your
health. It can help your doctor understand
how you are managing chronic conditions
you may have, or may be at risk for. While
its not a substitute for an annual physical
exam, your Health and Well-Being
Assessment is an important part of your
care.
3. We make it convenient for you.
Theres no trip to the doctors office, or
time spent in a waiting room. The Health
and Well-Being Assessment is conducted
by a licensed nurse practitioner with
Matrix Medical Network in the privacy of
your home.

The Health and Well-Being

Assessment is a Four-Step Process
to Better Health.
Step One: You will be visited in
your home by a licensed nurse
practitioner. This health care
professional is with Matrix Medical
Network and is specially trained in
providing Health and Well-Being
Assessments.
Step Two: The nurse will begin by
reviewing your medical history and
taking your blood pressure and pulse.
Step Three: The nurse will then ask
you detailed questions about your
health. The Health and Well-Being
Assessment will likely take between
60 and 90 minutes. You can take as
much time as you like to talk about
your health concerns.
Step Four: The results of your
assessment will be shared with your
primary care doctor. This enables you
and your doctor(s) to work together
in managing your medical concerns
and supporting your overall health
and wellness. If you do not have a
primary care physician, the nurse
practitioner will refer you to your
health plan to assist you in finding
one.

How much does the Health and WellBeing Assessment cost?

There is no cost to you for the Health and
Well-Being Assessment. It is a part of your
health care plan with Humana.
What happens if a problem is
discovered during the assessment?
Matrix Medical Network will communicate the
information to your primary care physician to
ensure appropriate care is provided.
Can the results from the assessment lead
to higher premium costs to me?
No. The assessments are not used to recalculate
benefit coverage, change rates or charge
additional fees.
