I. INTRODUCCIN
El parto pretrmino sigue siendo la causa principal de muerte neonatal temprana. Los
neonatos prematuros (menos de 37 semanas de gestacin) a menudo padecen
morbilidad inmediata significativa y permanecen hospitalizados en unidades de cuidados
intensivos neonatales durante perodos prolongados (Donoghue 2000). Adems, en una
proporcin de los sobrevivientes el riesgo de morbilidad neurolgica a largo plazo es
significativo (Johnson 1993). Cuanto ms prematuro es el neonato, mayores son los
riesgos, especialmente cuando el nacimiento ocurre antes de las 32 semanas de
gestacin. Los padres, los profesionales de la salud y la sociedad comparten la carga de
responsabilidad y los costos, tanto personales como econmicos, en cuanto al
nacimiento prematuro y la secuelas. Por lo tanto, la prevencin del nacimiento prematuro
sigue siendo una prioridad.
El desarrollo de la ciencia y la tecnologa mdica ha permitido mejorar la sobrevida de
los neonatos prematuros, pero los esfuerzos por disminuir de manera sustancial la
frecuencia del parto pretrmino han sido poco fructferos.
II. OBJETIVOS
Prevenir el parto pretrmino.
Uniformizar criterios de deteccin de factores de riesgo, diagnstico y manejo en el
parto pretrmino.
Incrementar el porcentaje de mujeres con amenaza de parto pretrmino que reciben
corticoterapia apropiadamente.
III. DEFINICION DEL PROBLEMA DE SALUD
Parto pretrmino: Es la interrupcin de la gestacin despus de las 22 semanas y
antes de las 37 semanas de embarazo. (WHO 1977)
Amenaza de parto pretrmino: Embarazo mayor de 22 y menor de 37 semanas, con
una frecuencia de contracciones uterinas mayor de 4 en 20 minutos acompaado o no
de la presencia de cambios cervicales iniciales consistentes en dilatacin menor o igual
de dos centmetros, y/o longitud mayor de 2 cm. o borramiento de hasta 50%.
Trabajo de Parto Pretrmino: Embarazo mayor de 22 y menor de 37 semanas, con
actividad uterina regular que no cede con el reposo en cama o con la hidratacin; con
cambios cervicales, progresivos, durante un periodo de observacin borramiento de 80%
o ms y crvix dilatado de ms de 2 cm (GUIAS DE MANEJO DEL TRABAJO DE PARTO

La incidencia est entre el 5 al 20% de los embarazos y se asocia al 60% u 80% de las
muertes en neonatos sin anomalas congnitas.
En EsSalud la incidencia es de 4%, constituyendo el 69% de la morbilidad neonatal y el
62% de neonatos muertos sin anomalas congnitas. (SVP 2004)
El parto pretrmino puede causar en el recin nacido:
Sndrome de dificultad respiratoria (estado en el que los pulmones de los recin
nacidos no estn lo suficientemente desarrollados para tomar el aire que necesitan),
Los mecanismos o la patogenia del trabajo de parto prematuro no son bien entendidas.
Sin embargo, las causas del trabajo de parto pretrmino espontneo pueden incluir
factores genticos, estrs, respuesta inflamatoria, factores mecnicos y hemorragia
(Lockwood 2001; Savitz 1999).
IV. POBLACION OBJETIVO
Mujeres en edad frtil y gestantes aseguradas que acuden a los centros asistenciales de
EsSalud.
V. MANEJO
1. ACTIVIDADES DE PROMOCION, PREVENCION Y DETECCION
A. FACTORES DE RIESGO ASOCIADOS CON PARTO PRETRMINO
MATERNOS
Infeccin:
- Vaginosis bacteriana.
- Infecciones urinarias, bacteriuria asintomtica, pielonefritis.
- Enfermedades de transmisin sexual.
- Intra amnitica ( con membranas ovulares intactas)
- Otras infecciones.
Bajo nivel socioeconmico
Edad menor de 15 aos
Tabaquismo ( ms de 15 cigarrillos por da)
Peso materno menor de 40 Kg.
Menos de 4 controles prenatales
Analfabetismo
Drogadiccin
Anomalas tero cervicales
- Incompetencia stmico - cervical
- Malformaciones uterinas

B. ACTIVIDADES
Detectar factores individuales de riesgo.
Identificar factores modificables de riesgo e intervenir
en consecuencia.
Considerar mtodos de educacin relacionados con
factores de riesgo.

C. Acciones preventivas:
1. Generales: Corregir la anemia cuando exista, mejorar el estado nutricional,
buena hidratacin, reubicacin laboral, reducir la ansiedad, abandono del
cigarrillo y otros hbitos nocivos.
2. Especficas:
a. Reposo relativo y continuo en pacientes de riesgo.
b. Evitar las relaciones sexuales con coito.
c. Antibiticos: si se demuestra bacteriuria o vaginosis empricamente en
baja situacin econmica o promiscuidad sexual.
d. Procedimientos quirrgicos: cerclaje en caso de incompetencia
stimicocervical.
El examen vaginal rutinario para evaluar el cervix no es
un mtodo efectivo para predecir el parto pretrmino y no
debera ser efectuado. NR (A)
2. CRITERIOS DE DIAGNOSTICO
DETECCIN TEMPRANA (PREVENCIN SECUNDARIA)
Toda paciente que consulte por sntomas sugerentes de amenaza de parto pretrmino
debe ser evaluada de manera exhaustiva para poder enfocar y determinar claramente su
diagnstico y manejo.
Sintomas:
Dolor abdominal inferior, intermitente, irradiado a la espalda.
Percepcin subjetiva de contracciones uterinas en aumento.
Descarga vaginal mucoide o sangunea.
Sensacin de pesadez en hipogastrio.
Sndrome miccional.
Infeccin vaginal asociada.
Establecer:
1.- Edad gestacional de 22 a 37 semanas de amenorrea confiable a partir de la fecha de
la ltima menstruacin. Si hay dudas, debe establecerse la edad gestacional por
mtodos clnicos, ultrasonogrficos o anlisis de lquido amnitico (pruebas de
madurez pulmonar).
2.- Contracciones uterinas que ocurran con frecuencia mayor de una en 10 minutos,
duracin mayor de 30 segundos, durante un periodo de 20 minutos (monitorizacin
clnico- tocogrfico). (4 en 20 minutos)
Exmenes Auxiliares:
Hemograma, eritrosedimentacin, sedimento urinario, urocultivo, Protena C Reactiva.
Ecografa transvaginal: medir longitud del cervix (v.n. > 25mm) dilatacin de orificio
cervical interno (normal hasta 5 mm) y signo del embudo .
3. ACTIVIDADES DE TRATAMIENTO
Medidas Generales
Hospitalizacin
Reposo absoluto en decubito lateral izquierdo
Hidratacin rpida: Cloruro de sodio al 9 o Dextrosa al 5% 1 litro ms 2 ampollas
de Cloruro de sodio al 20%; pasar 300 cm3 la primera hora, luego 100 a150 por hora
hasta detener las contracciones.
Mas de la mitad de las amenazas de parto prematuro se
controlan con esta medida.
Sedacin:
Fenobarbital, 100 mg cada 12 horas x 4 dosis, IM o V.O
Corticoides
Tocolisis
Descartar Infeccin de Tracto Urinario
Profilaxis Antibitica (infeccin estreptococo
grupo B)
5. Descartar infeccin intra amnitica.
6. Atencin del parto
1. CORTICOIDES: (ENTRE 24 Y 34 SEMANAS)
La terapia antenatal con corticoides es la intervencin
obsttrica ms importante en la prevencin de
morbilidad y mortalidad neonatal precoz. (NE IA)

Medidas habituales del control prenatal (AU, PESO, TA, FCF, etc.)
Control de infecciones urinarias recidivantes (urocultivos cada 30 das) y tratamiento
segn antibiograma.
Con cultivos crvico vaginales positivos: tratamiento segn germen hallado y posterior
repeticin de la prueba.
Indicar a la paciente que debe regresar inmediatamente, a emergencia, si vuelve a
tener las contracciones antes de las 37 semanas o si tiene sangrado o pierde lquido
amnitico.
PARTO PREMATURO:
Cuando la evolucin puerperal haya sido adecuada:
- Que los loquios sean de caractersticas normales, volumen normal y sin mal olor.
- Que la involucin uterina sea adecuada.
- Funciones Vitales normales.
Debe hacer su control puerperal normal segn se indica en el protocolo de atencin
del parto.
DESCANSO MDICO
Segn el caso entre 5 y quince das
2. REFERENCIA Y CONTRAREFERENCIA.
En trminos generales se atendern en hospitales de tercer nivel (hospitales que
cuentan con cuidados intensivos neonatales) todos los partos prematuros de menos de
34 semanas o con fetos con pesos menores a 1500gr.
Debern ser referidas adems las madres con enfermedades concomitantes y los fetos
con patologa.
La referencia del nivel I al nivel II se especifica en el item VII.
La contrareferencia se realizar, en caso de amenaza de parto prematuro una vez
superado el peligro o luego de cumplida la semana 34 segn criterio mdico y luego
del parto en caso de producirse parto prematuro.
El mejor medio de transporte para el nio/a es el
intrauterino, por lo que hay que procurar referir a
la madre antes que al recin nacido.
