PRODUCTION TITLE: FABLES
DIRECTOR/ PRODUCER: HANNAH DARBY/SEI-KAI LEUNG
1. ShootingPeriod
TheshootingperiodforthemotionpicturetentativelyentitledFABLES,willtake
place between _____________________ 2016 until _______________ 2016. The
producer may also require the actor for retakes and actors will be required to make
themselves available for these if needed.
2. Breaks/ Food
The Production Company will provide meal breaks and/or food service at approximately
four (4) hour intervals. The production company will cover meal arrangements and costs.

3. Expenses
TheProductionCompanywillnotcovertraveltoandfromlocations.TheProduction
Companywillnotcoverarrangementsandcostsforaccommodation.
4. Remuneration
Thepaymentofthesumof________willbemadeuponthefinaldayoffilming.Artist
shall be responsible for the payment of all taxes and other sums payable as a result of
receiving such remuneration.
Artist's Warranties
Artist warrants that:
(a)
Artist is free to enter into this Agreement and to grant to Company all rights and
consents granted hereunder;
(b)
Artist has not entered and shall not enter into any arrangement which may conflict
with this Agreement;
(c)
Artist will not incur any liabilities on behalf of Company nor represent that Artist
is entitled to do so nor pledge Company's credit;
(d)
Artist is, to the best of Artist's knowledge and belief, in such state of health that
Company will be able to effect insurance cover under normal conditions without
the premium being subject either to loadings or special exclusions and that Artist
shall be able to perform the services required by this engagement; and
(e)
Artist shall perform all services as are customarily performed by actors in first
class feature films.

I, __________________________________ the undersigned person, for good and valuable

consideration[_____________]thereceiptandsufficiencyofwhichisherebyacknowledged,
hereby grant the perpetual and worldwide permission to SEIKAI LEUNG / REVERIE
PRODUCTIONS (the Filmmaker) to use my name, image, likeness, and all recordings,
photographs,orotherrenderingsofmyname,image,orlikeness(collectively,myDepiction)
as such Depiction appears in, or in connection with, the motion picture tentatively entitled
FABLES(Film),alladvertisingandpublicityrelatingtotheFilm,andinallexhibitions,
performances,orexploitationoftheFilm,inwholeorinpart,byanyandallmeans,media,
devices,processesandtechnologynoworhereafterknownordevised.Iherebyacknowledge
thatyouhavenoobligationtoutilizetheDepictionintheFilmorinanyothermotionpicture.
IfmyDepictionisusedintheFilmIshallbeaccordedcreditintheFilminsubstantiallytheform
of:

In no event shall I seek, or be entitled to, damages of any kind by reason of any credit mistake, nor shall I be
entitled to seek to enjoin or restrain the exhibition, distribution, advertising, or other exploitation of the Film,
all such equitable remedies being hereby expressly waived by me.

I understand that you have been induced to proceed with the production, distribution and
exploitationofthePictureinrelianceuponthisagreementandthatforthisandothersreasons,
thisagreementshallbeirrevocable.
I hereby grant to______Sei-Kai Leung__________ (the Filmmaker), their successors, assigns
and licensees the perpetual right to use, as you may desire, all still and motion pictures and sound
track recordings and records which you may make of me or of my voice, and the right to use my
name or likeness in or in connection with the exhibition, advertising, exploiting and/or
publicizing of the picture. I further grant the right to reproduce in any manner whatsoever any
recordings including all instrumental, musical, or other sound effects produced by me, in
connection with the production and/or postproduction of the Picture.
I agree that I will not assert or maintain against_________ Sei-Kai Leung __________ (the
Filmmaker), your successors, assigns and licensees, any claim, action, suit or demand of any
kind or nature whatsoever, including but not limited to those grounded upon invasion of privacy,
rights of publicity or other civil rights, or for any reason in connection with your authorized use
of my physical likeness and sound in the Picture as herein provided.
TheFilmmakershallbetheexclusiveowneroftheFootageandtheResultsandProceeds
givingthemtherightto,withoutlimitation,inanyandalllanguages,inanyandallmediaknow
knownorhereafterinvented:(a)exhibit,broadcast,use,reproduceandlicenseotherstouseas
theyseefitalloranypartoftheFootageand/ortheResultsandProceeds;(b)edit,dub,subtract
from,addtoormodifytheResultsandProceedsinanymanner,combineitwithanyother
materialand/orincorporateitintootherfilms,projectsorprograms;and(c)useandlicense
others to use my name, voice, likeness, image, photograph, performance, participation,
expressions,personalexperiencesandbiographicalmaterial(collectivelyNameandLikeness),
in and in connection with the production, distribution, advertising, publicity, promotion,
exhibitionandotherexploitationoftheFootage,theResultsandProceedsand/oranyportion
thereof,anunlimitednumberoftimes,withoutanyobligationofanykindtomewhatsoever.
Iherebyacknowledgethatalloftheresultsandproceedsofmyservicesinandinconnection

withthePictureshallconstituteaworkmadeforhire,speciallycommissionedbyyou,and
thatyoushallownalloftheresultsandproceedsofsuchservices.Youmaymakesuchuseofthe
PictureandmyDepictionasyou,inyoursolediscretion,deemappropriate.
Ifmyservicesarenotrecognizedasaworkmadeforhire,Iherebyirrevocablygrant,sell,and
assigntoyouallofmyright,title,andinterest,ofanykindandnature,inandtotheDepiction
and the Film, including, without limitation, all copyrights in connection therewith and all
tangible and intangible properties with respect to the Depiction and the Film, in perpetuity,
whetherinexistencenoworasmaycomeintoexistenceinthefuture.
Ivoluntarilyassumeanyandallrisks,knownorunknown,associatedwithmyparticipationin
theFilmandactionsandundertakingsinconnectionwiththeFilmandtheFootage(collectively
myParticipation).IacknowledgethatmyParticipationmaypresentcertainriskstome,andI
herebyassumeanyandallrisksassociatedtherewith,including,withoutlimitation,theriskof
physicalormentaloremotionalinjury,minorand/orseverebodilyharm,and/orillness,which
arisebyanymeans,including,withoutlimitation:acts,omissions,recommendationsoradvice
givenbyProducerortheiragents,employees,orotherpersonsorentitiesaffiliatedwiththe
Film;participationininherentlydangerousactivities,latentorapparentdefectsorconditionsin
anyequipmentusedintheFilm;weatherorothernaturalconditions;humanerror;myphysical
andmentalcondition;myownactsoromissions;firstaid,emergencytreatmentorotherservices
renderedtomeorothers.Notwithstandingtheforegoing,Iherebyagreetovoluntarilyacceptand
assumeanyandallsuchrisksaswellasanyrisksnotmentionedhereinthatareinanyway
associated with my Participation and the subsequent or simultaneous exhibition or other
exploitationordisseminationoftheFootage,theResultsandProceedsand/oranyportionthereof
inperpetuitythroughouttheuniverseinanyandallmedia,whethernowknownofhereafter
invented(Exhibition).
I hereby waive the exercise of any moral rights, droit moral, and any analogous rights
howeverdenominatednoworhereafterrecognized.Ifurtherwaivetherighttoinjunctiveor
otherequitablereliefaswellastherighttorescindorimpairinanymanneranyoftherights
grantedtoyouhereunder. Allrightsgrantedandagreedtobegrantedtoyouhereunderare
irrevocableandshallvestandremainperpetuallyvestedinyou,andyoursuccessorsandassigns,
whetherthisagreementexpiresinnormalcourseorissoonerterminated,andshallnotbesubject
torescissionbymeforanycausewhatsoever.
It is an essential condition of this engagement that Artist agrees to maintain the totally
confidential nature of all matters relating to the Film, Artist's services hereunder and the terms on
which such services are to be rendered and Company's affairs and the affairs of its associated
companies generally including in particular any character in the Film, the script, the cast, the
financing or economic arrangements and technical processes involved in the Film.
Artist agrees not to disclose or permit the disclosure of any of the foregoing to any third party
save Artist's professional advisors save that Artist shall have the right to make non-derogatory
reference to the Film incidentally in interviews or personal publicity provided that Artist shall not
any time hereafter do anything detrimental or say anything derogatory about the Film and any
person involved in or associated with the Film.
By my signature here I understand that I will, to the best of my ability, adhere to the schedule
agreed to prior to the beginning of my engagement. Additionally, I agree, to the best of my

ability, to make myself available should it be necessary, to rerecord my voice and/or record
voice-overs and otherwise perform any necessary sound work required after the end of filming.
I further acknowledge and agree that any commitments beyond the scope and intent of this
release are the sole responsibility of the above named production, or its duly appointed
representative(s) and NOT 'I' the individual signing this agreement.
This is an integrated agreement. I acknowledge and agree that I am not entering into this
agreementonthebasisofanyrepresentationsorpromisesnotexpresslycontainedherein.This
Releaseshallbindandinuretothebenefitofthepartiesheretoandtheirrespectivesuccessors,
assigns,heirs,legalrepresentatives,administrators,executors,andguardians.IrepresentthatI
amatleast18yearsofage,andthatifIunder18myparentorlegalguardianhassignedbelow.

I represent and warrant that I am the parent or guardian of the minor whose name appears above, that I have read,
understood, and approve of the foregoing Release, and I consent to its execution by my child/ward. For good and
valuable consideration, the sufficiency and receipt of which is hereby acknowledged, I hereby release the Producer
as set forth and in accordance with the foregoing Release from any claims and causes of action I may have against
them of any nature whatsoever, and I hereby fully and unconditionally guarantee my child's/wards releases, waivers
and grant of rights as set forth above.
