LETTER OF UNDERTAKING BY AGENT/CONSULTANT

In consideration of my/our employment/engagement and/or contractual relationship with

SIRIM QAS International Sdn.Bhd (Company No.410334-X), a company incorporated under
the laws of Malaysia and having its business address at Block 8 ,SIRIM COMPLEX , No.1 ,
Persiaran Dato` Menteri, Section 2, P.O Box 7035, 40911 Shah Alam, Selangor, as an
Agent/Consultant who act on behalf of client/applicant, I /We hereby irrevocably agree,
confirm and undertake that:1) I/We shall abide by every terms and conditions stated herein;
2) I/We shall conformed and adhere to SIRIM QAS International`s rules and regulation;
3) I/We am/are not for the past 2 years a permanent and/or contract employees of
SIRIM Berhad or its subsidiaries (including its associated and related companies);
4) I/We am/are not a family member of any permanent and/or contract employees of
SIRIM Berhad or its subsidiaries who had been employed for the last 2 years;
5) I/We had understand and confirmed that SIRIM QAS International will not
responsible on any client/applicant`s choice for agent/consultant listed in SIRIM QAS
International`s system;
6) I/We had understand and confirmed that my/our position as the Agents/Consultant
who act on behalf of client/applicant as listed in SIRIM QAS International`s system
shall only be as a reference and shall not be regards as being certified or registered
by SIRIM QAS International;
7) I/We had understand and confirmed that my/our position as Agents/Consultants who
act on behalf of client/applicant as listed shall have no connection with SIRIM QAS
International;
8) SIRIM QAS International shall not be responsible on my/our act, conduct or omission
at all times;
9) I/We shall act in a professional manner at all time;
10) I/We shall not enter into any close business or personal relationship which is related
to SIRIM QAS International business/activities with any employees or staff of SIRIM
QAS International to maintain impartiality;
11) I/We shall be fully responsible for any application by me/us on behalf of the
client/applicant;
12) I/We had agreed to provide appointment letter and scope of appointment from
client/applicant for every applications made by me/us;
13) I/We shall undertake to supply all information and provide a complete set of product
including Interfaces, Connector, Test Jig, Test Adaptors, computer or accessories,
which are deemed necessary as required by SIRIM QAS International for the
purpose of evaluation of the product(s) to be certified;

14) I/We shall only provide ACCURATE information to SIRIM QAS International and
my/our respective clients;
15) I/We shall undertake to pay SIRIM QAS International all costs of processing the
application;
16) I/We agree that SIRIM QAS International may release information obtained in the
course of processing the application to the public or government authorities so far as
is prudent to warn in the opinion of SIRIM QAS International;
17) I/ We not to disclose any clients information to any interested party;
18) I/We shall not put any influence on decision made by SIRIM QAS International in
whatever form;
19) I/We shall never interrupt on SIRIM QAS Internationals office operation and work
flow;
20) I/We have great depth of understanding in technical knowledge and procedures for
the application of type approval, import permit or label;
21) I/We recommended to attend any related courses regarding to the process of type
approval, import permit or label.
I irrevocably believe the obligations herein are fair and reasonable and are essential for the
protection and orderly management of SIRIM QAS International. It is agreed that these
paragraphs are binding upon my heirs, executors and administrators
