RISK (AGREEMENT) CAREFULLY BEFORE YOU SIGN IT. THIS IS A LEGAL
DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS. IF YOU HAVE ANY

AssumptionofRisk

participantin the 3v3 ChristmasCup. Iamaware thatI may be subjectto anumberofrisksanddangers.I
understand and acknowledge that the competitionmay involvehazardous or dangerous activities and that
my participation in such competition may subject me to serious risk.I understand and acknowledge thatI
may be subject to a number of additional risks and dangers Iunderstandthat adequatemedical facilities
may notbe available shouldI requiremedical attention. Ivoluntarilyagreetoassumealloftheaboverisks,
andallotherrisksassociatedwithmyparticipationintheChristmasCupwhetherknownorunknown.

GeneralReleases
As consideration for being permitted by Lars Nordang, Matt Kemper, and Drew Meinke, tocompeteinits
tournament, I hereby release and forever discharge Lars Nordang, MattKemper, andDrew Meinke,their
directors, officers, agents, employees, representatives, volunteers, attorneys, assigns, and affiliates from
anyandallclaims and demandsof whateverkindornature,whetherknownor unknown,thatariseoutofor
are connected in any way whatsoever with my voluntary participationinthecompetition of the Christmas
Cup.

3.
ReleaseRe:MedicalTreatment
As furtherconsideration forbeingpermittedbyLarsNordang,MattKemper,andDrewMeinke,to participate
in their competition, I herebyrelease and foreverdischarge LarsNordang,MattKemper,andDrewMeinke,
their directors, officers, agents, employees, representatives, volunteers, attorneys, assigns, and affiliates
fromanyandallclaims and demandsof whateverkindofnature,whetherknownorunknown,thatariseout
or areconnectedinanyway whatsoeverwithanyfirstaid,medicaltreatmentorservicesrenderedmeduring
myparticipationinorinanywayrelatedtoChristmasCup.

4.
BindingEffect
Iunderstandand acknowledge thatthis Agreementis a binding legaldocumentthat affectsmylegalrights
and remedies. I further understand and acknowledge that this Agreement binds not only me but also my
spouse,children,heirs,representatives,distributes,guardiansandassigns.

6.
Scope
I understand and agree that this Agreement is intended to be interpreted and construed as broadly and
inclusivelyaspermittedunderthelawsoftheStateofWisconsin.Iflegalproceedingsare
filedIunderstandtheywillbetriedonlyintheStateofWisconsin.

7.
EntireAgreement/AmendmentOnlybyWriting

I understand and agree that this Agreement constitutes the entire agreement between me and Lars
Nordang, MattKemper,and DrewMeinke, concerning my participation in thecompetition ofthe Christmas
Cupand supersedesallnegotiationsand statements madepriorto orcontemporaneouswiththeexecution
ofthis Agreement. Ifurther understand and agree that thisAgreementmay onlybemodifiedor amendedin
writing signed both by me and by an authorized representativeofLars Nordang, MattKemper,andDrew
Meinke,andthatthisAgreementmaynotbeorallyamended.

8.
GoverningLaw/Forum
I understand and agree that this Agreement shall be governed by andinterpreted in accordance with the
laws of the State of Wisconsin. I hereby further agree that any litigation, administrative proceeding or
arbitration concerning this Agreement ormy participation in the competitionoftheChristmas Cup shallbe
brought andconductedintheSuperiorCourtofthestateofWisconsininandfortheCountyofWinnebago. I
agree to be subjectto personal jurisdictionand venuein thestateofWisconsin,CountyofWinnebago,and
herebywaiveanyright Imay have tocommenceanylitigation,administrativeactionorarbitration relatingto
this Agreement or my participation in the competition of the Christmas Cup in any form other than the
SuperiorCourtoftheStateofWisconsininandfortheCountyofWinnebago.

9.
InvalidityofAnyClause
Iunderstandandagree thatintheeventanyclause,sentenceorprovisionofthisAgreementshallbeheldto
be invalid orunenforceable by anycourtofcompetent jurisdiction, the invalidity orunenforceabilityofsuch
clause,sentenceorprovisionshallnotaffectthevalidityorenforceabilityoftheremainingprovisions.

10.
TermsContractual
Iunderstandand agree that the termsoftheAgreementarecontractualandareconditionsprecedent tomy
participationinthecompetitionoftheChristmasCupandarenotmererecitals.

11.
ReleaseofIdentity
Iherebyauthorizetheuseofmypicture,whethervideoorstill,and/orverbalstatementsmadebyme,to
LarsNordang,MattKemper,andDrewMeinke,orotherorganizationsalliedwithLarsNordang,Matt
Kemper,andDrewMeinke,innewsorpromotionalmaterialorvideo.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS AND
BINDING EFFECT. I ACKNOWLEDGE THAT I HAVE HAD THE OPPORTUNITY TO HAVE THIS
AGREEMENT REVIEWEDBYANATTORNEYPRIORTOTHETIMEISIGNED IT.IUNDERSTANDTHAT
THISAGREEMENTIS ALEGALCONTRACT BETWEEN MEAND Lars Nordang,MattKemper,andDrew
Meinke. THAT AFFECTS MY LEGAL RIGHTS.I REPRESENTTHATI AMSIGNINGTHIS AGREEMENT
KNOWINGLY,VOLUNTARILYANDOFMYOWNFREEWILL.
