The performance appraisal form is to be completed by the workplace supervisor in the professional
services firm in which the student of Sunway University is placed on Internship attachment. The
performance appraisal contributes 70% to the final assessment.
Please complete this evaluation at the end of the students work period. You are encouraged to
discuss the completed form with the intern to aid in their professional development. This Appraisal
Form aims to cover issues that relate to the overall development of the student.
