BEFORE HOSPITALIZATION
Ang kalusugan para sakin ay ang
malusog ako at ang aking pamilya as
verbalized by the patient. According to
the patient she already had numerous
experienced of fever she usually takes
OTC during like paracetamol 500mg
every 4 hours until the fever subside.
And when she have cough and colds,
she usually drink a lot of warm water.
Patient J.M. usually eats 3 times a day,
every meal she eat 2-3 cups of rice,
more on meat and fish and often on
vegetables. She drinks 10-12 glasses
of water a day, every glass of water
contains 250ml. she does not have any
allergies reaction to any food and
medication. And according to her, her
weight is 79kg.

According to patient J.M. she voids 810 times a day with yellowish in color
of urine. She also defecates once or
twice times every after two days. Stool
is brown in color and semi formed.
She does not take any laxatives.
Patient J.M. does not have painful
sensation during urination and
defecation.
According to patient J.M., she does
household chores and goes to work.
She was able to bathe herself and have
her exercise sometimes by stretching
and walking. She does not take energy
vitamins. She feels uneasy when she
does not take a bath. Patient J.M. can
ambulate and perform activities of
daily living without assistance
According to patient J.M., she sleeps
for 7-8 hours in a day. She usually
wakes up at night to urinate. She does
not take medication for inducing sleep.
Her usual routine before sleeping is
taking a bath to feel more comfortable
when sleeping.

DURING HOSPITALIZATION
Ang kalusugan para sakin ngayon ay
kayamanan na dapat kong alagaan
para sa magandang kinabukasan ko at
ang aking pamilya as verbalized by
the patient. She managed her health
right now by following the doctors
order to her. And willing to accept and
listen to health teachings.
Before the operation, patient eats her
meals 3x a day with 1 cup of rice in
every meal but she was on NPO 12
hours before the surgery. After the
operation patient E.T does not eat any
food just GLEMCD but when she is
able to defecate the patient is able to
eat DAT. She eats three times a day,
every meal she eats two cups of rice,
one cup of vegetable. She also drinks
6-8 glasses of water a day.
Approximately 250 per glass. And her
weight is 48kg.
According to patient J.M. she voids 911 times and defecates 1-2 times a day
but lessen in amount. She does not
experience pain during urination and
defecation. Her urine color is light
yellow. Her stool is watery because
she take laxative as prescribe by her
doctor (Bisacodyl)
She was able to ambulate, lay down
herself in bed, bathe herself, and have
her meal without assisting before her
operation. After the operation the
patient cannot easily ambulate because
of her surgical incision but we told her
that she must move her body so that
she can easily to recover to the
surgery.
There is a change in patient J.M.
sleeping pattern. She usually sleeps
10-12 hours and she has her siesta.

According to patient J.M. she is a


grade 1 undergraduate of elementary.
She knows how to speak tagalong,
Iloko and Ibanag as verbalized by the
patient. She has no hearing deficit,
have a good vision, she can see
without eye glass. According to patient
J.M., she usually recalls events easily
from the past. She can respond to
stimuli verbally and physically
oriented and have her husband beside
her in decision-making.
Patient J.M., describe herself as a
silent type in terms of she do not make
chika to the other people life, she is
happy, tall, and skinny., she hates
when somebody is making chismis to
her. Her emotions affect her way of
communicating with others. According
to her, she limits herself towards
things she wants to provide more for
her family. She easily accepts changes
in her life. maging positibo lang tayo
lagi sa buhay as verbalized by the
patient.
According to patient J.M., she is
married and has children. Living with
her family and described it as a solid
family. She managed with her husband
for their family. Patient J.M. does not
avoid responsibilities given to her.
When we ask if she is disturbed by her
relationship, she verbalized hindi
naman madalas, may araw din na
hindi kame nagkakaintindihan. She
does not have multiple relationships.
When confronted with
misunderstanding with a family
member or anyone else, he verbalized,
dapat inaayos kaagad ang problema
para hindi lumala at magkaroon ng
alitan sa bawat isa.

The patient properly responds to the


questions being asked. She was able to
answer open-ended question concisely.
Patient J.M. is oriented of time, place,
and person. There are no changes
noted on his senses.

According to patient J.M., she was


scared for the scheduled operation.
She perceived if her operation was
done theres nothing to think about
and can focus herself doing things that
matters to her. She was still hopeful to
have positive operation although she
was worried but she was trying herself
to be calm.

Her relationship with her family


during hospitalization remains the
same. She maintains communication
with her husband and children because
she feels more relieved when talking
to her husband and children.

Patient J.M. had her menarche when


she was 13y/o. her usual menstruation
lasts for 3-5 days with no pain and
consumes 1-2 pads of napkin a day
and fully soaked. Her menstruation is
regular. She menopause at the age of
43 years old and also that age she
experience enlarging abdomen. She
also stated that her first coital with his
husband happened when she was
17y/o. They have sex 2 times a week.
Patient J.M. copes up with stress by
managing to pray and relax. When it
comes to financial problems and
others, her husband is beside her and
they both handle it helping each other
to resolve it.
According patient J.M., she is a
Roman Catholic but she doesnt attend
mass regularly. Sometimes she
believes in superstitious belief. And
according to her, having faith in God
is the most important and God is
always there from the trials they
encounter until they surpassed it.

According to patient E.T she has no


sexual activity and he also stated that
she is happy to have four children and
her husband in her life.

Patient J.M. usually pray, takes a nap


and rest. She talks to her family and
when tense she alleviates herself that
everything will be fine and she tries to
be an optimistic
According to patient J.M. her faith did
not change and remains to be strong.
Patient believes to have a successful
operation because she believed that
God is always there to help and also
she asks for the guidance of the Lord.
