Techniques
Neuromuscular facilitation techniques are a group of techniques used to facilitate or
inhibit muscle contraction or responses.
Prolonged Stretch
Resistance

making them more ser.siiive to changes in velocity and length.
Muscle spindle provides input to spinal neurons (stretch reflex) and higher centers
(functional stretch response).
The response facilitates or enhances muscle contraction. It facilitates agonist patterns,
inhibits antagonists and facilitates synergists and reciprocal innervation effects.
Resistance also recruits both alpha and gamma motoneurons and additional motor
units. Resistance hypertrophies extrafusal muscle fibers and enhances kinesthetic
awareness.
Techniques include:
Manual resistance, adjusted to produce smooth, coordinated movement
Use of body weight and gravity
o Mechanical weights Comments: Light, tracking resistance is used to strengthen
weak muscles. With weak
hypotonic muscles, eccentric and isometric contractions are used before concentric
contractions (this enhances muscle spindle support of contraction with less spindle
unloading). Maximal resistance may produce overflow to other muscles. Resistance
also enhances patient awareness of movement. ' Adverse effects: Too much resistance
can easily overpower weak, hypotonic muscles
and prevent voluntary movement or encourage substitution. Excessive resistance may
increase spasticity. Breath-holding may accompany maximal resistance and should be
avoided.
Joint Approximation
The stimulus is compression of joint surfaces. Approximation activates joint
receptors (greatest effects on static, type I receptors).
The response facilitates postural extensors and stabilizers. Joint compression
enhances joint awareness.
Techniques include:
Joint compression, either manual or mechanical using weight cuffs or belts o
Bouncing while sitting on a Swiss ball
Comments: Compression is typically applied in extensor patterns, in middle to
shortened ranges while the patient is weight bearing. Resistance must be used to
maintain the contraction.
Adverse effects: Approximation is contraindicated in inflamed joints.
Joint Traction
The stimulus is distraction of joint surfaces. Traction activates joint receptors
(greatest effects on phasic, type II receptors).
The response facilitates agonists. Traction enhances contraction and joint awareness.
Technique: Manual distraction is used.
Maintained Touch
The stimulus is maintained contact or pressure. Maintained touch activates tactile
receptors with input into the CNS and parasympathetic division of the autonomic nervous
system.
The response is a calming effect and generalized inhibition. Maintained touch desensitizes skin receptors.
Techniques include: o Firm manual contacts o Firm pressure applied to the midline of
the abdomen, the back, lips, palms, and/or the
soles of the feet Firm rubbing
Comments: Maintained touch is useful for patients with high arousal and a hypersensitivity to sensory stimulation. Maintained touch can be applied to hypersensitive areas
to normalize responses, for example, in the patient with peripheral nerve injury and
paresthesias.
Adverse effects:
Brief touch stimuli should be avoided.
Maintained touch can be used in combination with other maintained stimuli.
How Stroking

The stimulus is slow stroking, applied to the paravertebral spinal region over the
posterior primary rami. Slow stroking activates tactile receptors in the
parasympathetic division of the autonomic nervous system.
The response is a calming effect and generalized inhibition.

Techniques o The patient is placed in a supported position such as prone, or sitting with
the head
and arms supported and resting on a table top.
The therapist uses a flat hand to apply firm, alternate strokes downward to the paravertebral region for approximately 3 to 5 minutes.
Comments: Slew stroking is useful with patients who demonstrate high arousal and
increased sympathetic (fight-or-flight) responses.
Manual Contacts
The stimulus is firm, deep pressure of the hands in contact with the body. Manual contacts activate tactile receptors and muscle proprioceptors.
The response is contraction in the muscle(s) directly under the hands. Manual contacts
provide sensory awareness and directional cues tc .T.ovcizcr.t. The to^t^U uldo piyviJt;
security and support to unstable body segments. Comments: Manual contacts can be used
with or without resistance. Adverse effects: Manual contacts are contraindicated over
spastic muscles and open wounds.

The stimulus is application of cold water or ice. Prolonged icing activates thermoreceptors.
The response is decreased neural and muscle spindle firing. Icing inhibits muscle tone
and painful muscle spasm. Prolonged icing also decreases the metabolic rate of tissues.
Techniques include: Immersion in cold water, ice chips 0 Ice towel wraps Ice packs
Ice massage
Comments: Effects should be monitored carefully.
Adverse effects: Prolonged icing can cause sympathetic nervous system arousal and
evoke protective withdrawal responses, including fight-or-flight responses.
Prolonged icing is contraindicated in patients with sensory deficits,
generalized arousal, autoinstability, or vascular problems. Exposure to skin
should be limited to avoid
Neutral Warmth
The stimulus is retention of body heat. Neutral warmth activates thermoreceptors in the
parasympathetic division of the autonomic nervous system. The response is generalized
inhibition of tone. Neutral warmth produces a calming effectrelaxation; it also can decrease
pain. Techniques include:
Wrapping the body or body parts with Ace wraps or towel wraps Applying snug-fitting
clothing (gloves, socks, or tights) or air splints o Immersing the body or body part in tepid
ba*,hs
Comments: Neutral warmth should be applied for approximately 10 to 20 minutes. It is useful
for patients with high arousal or increased sympathetic activity, including spasticity.
Adverse effects: Overheating should be avoided. Excessive warmth may produce rebound
effects (increased arousal or tone).

INITIAL MOBILITY Strategies/Techniques to Improve Mobility
Initial mobility is characterized by movements that are not well controlled. They subserve
a large protective function and therefore have a large reflex base (for example, protective
withdrawal). Initial mobility is generally developed in non-weight-bearing postures such
as supine, sidelying, or prone positions.
STABILITY

control. Modified plantigrade position (weight bearing in standing position with
extended
elbowsupper and lower extremity support) promotes control of the trunk and
proximal extremities (upper and lower extremities). Standing promotes knee, foot, and
ankle control in addition to upright head, trunk, and
hip control.
Patients who demonstrate problems with stability control may be unable to hold a posture
for several reasons including tonal imbalances (hypotonia, hypertonia), impaired voluntary control (decreased strength, lack of stretch sensitMty), hypermobility (athetosis,
ataxia), sensory hypersensitivity (tactile-avoidance reactions), and/or increased arousal
(high sympathetic tone).
Emphasis should be placed on:
Isometric contractions in the shortened range (postural extensors), progressing to
midrange control (co-contraction)
Weight bearing and holding in antigravity postures
Specific motion deficits (for example, patients with hyperkinesia, including ataxia and
athetosis) may benefit from gradually decreasing ROM progressing to steady holding
Specific neuromuscular facilitation techniques may include:
Alternating isometrics (AI) or rhythmic stabilization (RS) Slow reversal-hold (SRH)
through decrements of ROM Placing and holding: hold after positioning
Sensory stimulation inputs may include:
Joint approximation (manual compression and weight bearing or gentle bounding while
sitting on a Swiss ball) to facilitate stabilizing muscles
Weights and Theraband or surgical tubing to increase proprioceptive loading Pool therapy
to provide antigravity support and proprioceptive loading via resistance of
water
CONTROLLED MOBILITY
Strategies/Techniques to Improve Controlled Mobility
The ability to move while maintaining a stable upright posture is referred to as controlled
mobility. It is characterized by the combined function of both mobilizing and stabilizing
components with smooth reversal of the antagonists. '
Closed-chain activities refer to those functional activities that involve a total body
movement with the distal end of the extremity fixed in space while the proximal segments
(trunk and proximal jointsshoulders and/or hips) are moving.
Examples of activities in which controlled mobility function is used include rocking,
weight shifting, or movement transitions (shifting from one posture to another) while
maintaining upright posture. For example, the patient is asked to rock slowly from side to
side in sitting or quadruped position, or the patient can be asked to assume a quadruped
position from a side-sitting position.
A variation of this level of control is termed static-dynamic control. While maintaining a
static posture, one limb is freed to move through space (for example,
reaching or stepping). From a quadruped position one upper limb is lifted,
then one upper limb and the opposite lower limb are lifted simultaneously.
This requires dynamic stability because of the reduction in the overall bnso
of support and the shift of the center of mass over the support limbs
Deficits in controlled mobility are associated with an inability to maintain a posture while
moving (dynamic stability). A number of factors can produce deficits in controlled
mobility, including tonal imbalances, ROM restrictions, impaired reciprocal actions of the
antagonists, or impairment of proximal stabilization.
Emphasis should be placed ^n:
Smooth movements and antagonist patterns (changing direction)
Carefully graded assistance using key movement elements only
Light, tracking resistance only, or antigravity resistance
Eccentric control to concentric control
Gradually increasing ROM
Functional activities (for example, sitting and reaching and supine-to-sitting transitions)
Steady holding of a posture while dynamic limb segments are moving (static-dynamic
control)
Repetition of movements
Specific neuromuscular facilitation techniques may include:
Slow reversals (SRs) through increments of range Slow reversal-hold (SRH)
Timing for emphasis/repeated contractions (TE/RCs) Agonist reversals (ARs)
Active-assistive to active movements and movement transitions
Swiss ball activities (for example, sitting on the ball and moving the ball while maintaining the sitting position) Stability techniques to facilitate static segments as needed (AI,
RS, and approximation)

Feedback
Feedback is response-produced information acquired during or after performance; information is used to make error corrections. There are two main categories of feedback
intrinsic and augmented:
Intrinsic feedback is naturally occurring or inherent information derived from the patient's
own sensory and perceptual systems, such as visual, auditory, proprioceptive, and tactile
feedback.
Augmented feedback is supplemental or added feedback derived from external sources
such as verbal cues, electromyogram (EMG) biofeedback, and videotaped performance.
Augmented feedback is subdivided into two typesknowledge of results and knowledge
of performance:
Knowledge of results (KR) is augmented feedback related to the nature of the result or
outcome produced in terms of the environmental goal; for example, the patient with a
spinal cord lesion successfully transfers from wheelchair to mat without a transfer board
or assistance.
Knowledge of performance (KP) is augmented feedback related to the nature of the
movement characteristics produced; for example, the patient with a spinal cord lesion
successfully scoots to the edge of the chair, pushes up and lifts the body, and laterally
shifts the pelvis over onto the mat using reverse movements of the head and upper trunk.
Precise and accurate feedback should be used. The patient should be assisted in recognizing and pairing intrinsic feedback with movement responses. Consideration should
be given to videotaping and analyzing performance.
Feedback given after every trial improves performance; feedback given after several trials improves learning and retention. Consideration should be given to use of different
timing for feedback:
o Summed feedback is given after a set number of trials (for example, every third trial).
Fading feedback is given after every trial, then every third trial, every fifth trial,
and so forth, o Bandwidth feedback is given only if performance exceeds an expected
error range.
Early training should focus on visual feedback (the cognitive phase of learning); later
training should emphasize proprioceptive feedbackthe "feel of the movement" (the associated phase of learning).
Augmented feedback should be provided about knowledge of results and knowledge of
performance. The therapist should emphasize different aspects at different stages: o
During early learning, it is important to emphasize the correct aspects of performance.
During the middle and later stages of learning, errors should be emphasized only as
they become consistent.
Use of feedback should be carefully monitored. Feedback dependency should be
avoided with attention focused on reducing augmented feedback as soon as possible and
allowing the patient active introspection and decision making (participation improves
learning and retention).
Extraneous environmental stimuli must be reduced early in learning; practice should
occur in a closed'environment. During later learning the focus can be shifted to adapting
performance to varying environmental demands; practice should now occur in an open
environment.
Supportive feedback can be used to motivate the patient and shape (reinforce) behavior.
Practice
Constant practice is practice of a single motor skill repeatedly; repetitive practice.
Variable practice is practice of a motor skill in a context in which the patient is required
to make rapid modifications of the skill in order to match the demands of the situation.
Mass practice is practice in which the amount of rest time is small; rest time is less than
the total practice time. Mass practice may lead to decreased performance and fatigue.
Distributed practice is practice in which the rest time is relatively large; practice time is
less than the total rest time.
Mental px-actice is cognitive rehearsal of a motor skill without making overt physical
movements.
Practice order is the order in which practice of tasks is organized:
Blocked practice order is practice of the same task within the practice session; for
example, task A can be practiced repeatedly, in a blockAAAAAA. Blocked practice
order results in improved acquisition of a skill and performance.
Serial practice order is practice of a group or class of motor skills in a predictable order
within a series (for example, ABCABC).
Random practice order is practice of a group or class of motor skills in a random,
unpredictable order (for example, ACBBAC). Random practice order represents higher
contextual interference, resulting in improved retention of the skill.
A practice schedule should be established. Distributed practice is used when the task is
complex, long, or energy costly; when superior performance is desired; when motivation
is low; or when the patient has a short attention span, poor concentration, or fatigues
easily. Mass practice is used when the task is highly meaningful and the patient has an
adequate attention span, concentration, high motivation, and endurance.
Practicing tasks within a class of different but related skills provides contextual interference and increases the depth of cognitive prorr-ssing; for example, a serial
or random
practice order results in improved retention of the skill. Variable practice will promote
generalizability of skills to different environments.
Mental practice can be used to improve learning; the patient verbalizes task components
and critical task elements required for performance.
Mental practice is effective when the task has a large cognitive component and cognition is intact.
Mental practice can help to decrease anxiety or fear.
Transfer refers to the effect (gain or loss) of previous practice of a skill on learning a new
skill, or on the performance in a new context. Transfer may be either positive (assisting
learning) or negative (hindering learning).
Part-whole transfer is a learning technique in which the motor task is broken down into
its component parts for separate practice before practice of the integrated whole.
Bilateral transfer occurs when improvement in movement skill performance with one
limb results from practice with the opposite limb.
Practice of parts to whole is used when the task is complex, with highly independent parts
(for example, a transfer). Part-whole transfer should also be used when the patient has
limited memory or attention or difficulty with a particular component of a task. The
practice session should always include both the parts and the integrated whole; practice of
the whole should not be delayed. Practice of parts to whole is not as effective when the
task has highly integrated, dependent components such as gait.
Practice of bilateral movements (bilateral transfer) should stress identical movements
from one side to the other and the similarity of task components. Practice of tasks should
occur in similar performance situations.
o Instruction and demonstration are used carefully. Verbal, visual feedback, and manual
guidance are provided as needed. Progression is made from passive to
active to resistive movement.
