Einfhrung
Introduction
PVC wird fr vielfltige medizinische Anwendungen eingesetzt. Die funktionalen Eigenschaften von PVC eignen
sich in hohem Mae fr eine Vielfalt von medizinischen
Produkten. Das von Natur aus sprde und harte PVC wird
dabei mit Substanzen wie Di-(2-ethylhexyl)phthalat
(DEHP) weichgemacht. Derartige Weichmacher haben
prinzipiell ein niedriges Molekulargewicht, da sie innerhalb der molekularen Struktur des Polymers beweglich
sein mssen, um die gewnschten Effekte zu erzeugen.
Als unabwendbare Konsequenz daraus resultiert eine signifikante Migration des Weichmachers in Kontaktmedien. Voraussetzung dafr ist jedoch, dass die Lslichkeit
des Weichmachers in den Kontaktmedien hoch genug ist.
Im Falle von enteralen Nahrungslsungen, die normalerweise ber einen gewissen Fettgehalt verfgen, kann
diese Migration von Weichmachern aus den Ernhrungssets zu einer betrchtlichen Weichmacherbelastung des
Patienten fhren, da in der Regel die berfhrungsschluche und manchmal zustzlich noch die Beutel aus
weichgemachtem PVC bestehen. Daher muss der Einsatz
von Weichmachern bei der medizinischen Versorgung
von Patienten bercksichtigt werden. Insbesondere bei
Frhgeborenen, die enteral ernhrt werden mssen, kann
die Belastung mit Weichmachern bezogen auf das Krpergewicht besonders hoch ausfallen.
Der derzeit in medizinischen Anwendungen noch am
hufigsten eingesetzte Weichmacher, das Di-(2-ethylhexyl)phthalat (DEHP), ist aufgrund seiner fruchtbarkeitsbeeinflussenden und fruchtschdigenden Wirkung im
Tierversuch als toxisch gekennzeichnet. Das europische
wissenschaftliche Komitee fr Nahrungsmittelsicherheit
(Scientific Committee on Food SCF) hat fr DEHP eine
maximal zulssige, tgliche Dosis (TDI) von 50 g pro kg
Krpergewicht festgelegt [1]. Das wissenschaftliche Komitee fr Medizinprodukte und medizinische Gerte (Scientific Committee on Medical Products and Medical Devices
(SCMPMD)) wollte sich aufgrund der Daten nicht auf eine
hchstzulssige tgliche Zufuhrmenge festlegen, sondern
betont stattdessen die Abwgung zwischen erzielbarem
Nutzen und mglichen Risiken[2]. Gleichwohl wird in der
Verffentlichung darauf hingewiesen, dass einige Gruppen einem hheren Risiko durch die Behandlung ausgesetzt sein knnen. Das SCMPMD forderte dringend dazu
auf, die Datenlage bei mglichen Alternativen zu verbessern.
Vor dem Hintergrund der anhaltenden Diskussion um
DEHP rcken Alternativen ins Blickfeld des ffentlichen
Interesses. Besonders interessant sind solche Alternativen,

expected, the migration increases when the stock solution bag also contains DEHP. According to the results of
these studies, the increase is constant at about 20% greater than the migration when using DEHP-free EVA bags.
It can be concluded that considerable quantities of plasticizer can transfer into the feeding solutions. However,
plasticizers are vital for the functional properties of the
products. It is not possible to completely do without pla-
