The Ex employee was under sick list from 01.04.2014 to 07.04.2014 he has
attended Health Unit BJP and taken fit certificate on 07.04.2014, As per Police case
dairy the ex employee was crossing the Railway Track on the BIJAPUR RAILWAY
Station while retuning home he got stuck to moving train and died on the spot .(copy
enclosed).
The name of the candidate does not tally with FCC and 9 th Statement of
marks , the name of the candidate in FCC copy is recorded Anjali and in statement
of Marks mentioned as ANJALEENA MaliyalI whereas in School certificate her
name is recorded as Anjeleena Malayali,the widow has submitted an affidavit
clarifying the discrepancies stating that after her marriage with Sri Novel Philip
Joseph she was known as Anjali consequent to marriage with ex-employee her

Diploma in Mechanical Engineering he has completed 2nd year Diploma, at Gadag.
Daughters of Late Sri Yellappa have submitted their no objection certificate stating that they
are not having any objection if their brother gets job opportunity, the same is enclosed.
The widow has submitted on Affidavit stating that , the date of births given by me are
only correct. I am not aware of the family composition certificate (F.C.C.) given by my late
husband in the office and came to know about it after his death only. I am clarifying the
discrepancies and variations. My information is correct to the best of my knowledge and
belief (Affidavit enclosed)

Special or any other particulars considered relevant in the case:

This is a case of two wives dispute for which settlement in favour of Smt. Parvathi (Jambavva)
w/o (L) Huchappa Ramappa Shirunde Ex Points Man under SS/O/Koppal, was kept pending for

want of succession certificate from the Court. The case came to be finally disposed on
09.01.2015 by Honble High Court, Circuit Bench, Dharwad.
From the proceedings of the Court case, it is seen that the above named Smt. Parvathi was
married to Late Huchappa Ramappa Shirunde on 04.05.1982.
The Honble Court has held that Smt. Parvathi her son(Yellappa Shirund ,CG candidate) and
2nd wife children are the legal heirs of deceased late Huchappa Ramappa Shirunde who are
entitled to receive the service benefits of the deceased.
The widow had however approached this office for settlement dues and appointment on
compassionate grounds vide her application dt. 08.06.2015 (copy enclosed) consequent on the
orders of Pr.Dist.& Sessions Judge, Dharwad.
The candidate has passed SSLC in March 2004 as a repeater as he could not pass the same at
first attempt during the year 2000.
The Widow has submitted an Affidavit for variations/differences in the surname of her son &
and in the ration card (enclosed in original) .The Ex employee was not executed the FCC in
favour of 1st wife and 1st wifes children , he has executed only 2nd wife s children.
Certified that the above information has been verified from records / spot enquiry and are
correct and no appointment to any ward / relative of the deceased employee has been given.
